Nephrogenic diabetes insipidus: a comprehensive overview.

Nephrogenic diabetes insipidus (NDI) is characterized by the inability to concentrate urine that results in polyuria and polydipsia, despite … ts may developcomplications leading to high morbidity and mortality, such as severe dehydration and hypernatremia. In that sense, the treatment of NDI consists in decreasing the urine output, while allowing appropriate fluid balance, normonatremia, and ensuring an acceptable quality of life. Therefore, therapeutic options include nonpharmacological intervention …

Conn's Syndrome: An Unusual Cause of Periodic Paralysis.

Primary hyperaldosteronism, also known as Conn's syndrome, is characterized by an independent and excessive aldosterone production in the adrenal cortex, causing hypernatremia, arterial hypertension, and, in some cases, potentially severe hypokalemia can occur. We report a case of a 45-year-old Caucasian woman, with a history of obesity and hypertension, who presented to the emergency room with a four-week evolution history of myalgia and ascending muscle weakness eventually resulting in tetraparesis. The initial blood analysis showed severe hypokalemia (1.5 mmol/L). Further laboratory studies…

Diabetes Insipidus: Pathogenesis, Diagnosis, and Clinical Management.

Diabetes insipidus (DI) is an endocrine condition involving the posterior pituitary peptide hormone, antidiuretic hormone (ADH). ADH exerts … ng, requires discontinuation of medications as well as maintaining a renal-friendly diet to prevent hypernatremia. Treatment of dipsogenic DI is mainly focused on behavioral therapy aimed at regulating water intake and/or administration of antipsychotic pharmaceutical therapy. Central and nephrogenic subtypes of DI share a paradoxical treatment in thiazide diur …

Strategies to reduce readmissions for hyponatremia after transsphenoidal surgery for pituitary adenomas.

PURPOSE: Disorders of water balance, particularly hyponatremia from altered antidiuretic hormone (ADH) secretion, are a common post-operativ … ponatremia. Importantly, the mild fluid restriction did not result in any hospital readmissions for hypernatremia. CONCLUSIONS: Mild fluid restriction (to 1.5 liters daily), in addition to a single post-operative serum sodium level, is an effective approach to preventing readmission for hyponatremia after TSS for pituitary adenomas.

Management of concomitant metabolic encephalopathy and meningioma with vasogenic edema and impending herniation.

BACKGROUND: Altered mental status describes impaired mental functioning ranging from confusion to coma and indicates an illness, either metabolic or structural in nature. Metabolic causes include hypothyroidism, hyperuremia, hypo/hyperglycemia, hypo/hypernatremia, and encephalopathy. The structural causes include tumors, brain hemorrhage, infection, and stroke. To our knowledge, this is the first case in which a patient presented with altered mental status from both metabolic (myxedema coma) and structural diseases (frontal meningioma) with vasogenic edema and midline shift. CASE DESCRIPTION:…

The patient rapidly developed polyuria and hypernatremia with an inappropriate decrease in urinary osmolality. [Urinary excretion of aquaporin-2 in water metabolism disorders].

Water metabolism plays an essential role in the homeostasis of body fluids in animals and humans. It is regulated by arginine vasopressin (AVP), renal function and water drinking. Disorders of water metabolism result in an increase or decrease in a body water or fluid, which manifest as hyponatremia, hypernatremia, polyuria, dehydration or edema. In the pathogenesis of such pathological conditions AVP is either directly or indirectly involved. Aquaporin-2 (AQP-2) is an AVP-dependent water channel in renal collecting duct cells. Approximately 3% of AQP-2 is excreted into the urine, which is mea…

None of the dogs developed hypernatremia or hypokalemia or any clinical signs suggestive of hypoadrenocorticism during the study. Metabolic alkalosis was induced in 10 clinically normal horses by administration of furosemide (1 mg/kg of body weight, IM) followed 4.5 hou … bility, as evidenced by muscle fasciculations, and 5 of the horses developed diaphragmatic flutter. Hypernatremia was transiently induced, but it resolved as the horses were allowed access to water. Salt and fluid balance in newborn infants.

About 5 years ago it was widely thought that preterm infants should be supplemented with extra sodium to avoid hyponatremia. It has, however, to be pointed out that VLBW infants during the first postnatal week run a risk of developing hypernatremia which makes it difficult to give general recommendations for sodium supplementation. Since the serum sodium level depends not only upon the balance between sodium intake and sodium excretion but also upon the water balance which may vary depending on the clinical condition of the baby, it seems advisable to monitor the serum sodium level in all pret…

The clinical physiology of water metabolism. Part III: The water depletion (hyperosmolar) and water excess (hyposmolar) syndromes.

Hyperosmolality occurs when there are defects in the two major homeostatic mechanisms required for water balance-thirst and arginine vasopressin (AVP) release. In this situation hypotonic fluids are lost in substantial quantities causing depletion of both intracellular and extracellular fluid compartments. Patients with essential hypernatremia have defective osmotically stimulated AVP release and thirst but may have intact mechanisms for AVP release following hypovolemia. Hyperosmolality can also be seen in circumstances in which impermeable solutes are present in excessive quantities in extra…

UNLABELLED: Temozolomide (TMZ), an alkylating agent used in glioblastoma multiforme and anaplastic astrocytoma treatment, is generally associated with side effects like myelosuppression and nausea. This case report describes a rare occurrence of nephrogenic diabetes insipidus (NDI) in a 45-year-old male with grade IV astrocytoma undergoing TMZ therapy. Following TMZ discontinuation due to haematological toxicity, the patient developed significant polyuria and hypernatremia. Central diabetes insipidus was initially suspected, but limited improvement with desmopressin and normal endocrine invest…
