Community emergency medical services approaches to fall prevention: a systematic review.

BACKGROUND: Falls are a leading cause of morbidity and mortality among older adults in the USA. Current approaches to fall prevention often rely on referral by primary care providers or enrolment during inpatient admissions. Community emergency medical services (CEMS) present a unique opportunity to rapidly identify older adults at risk for falls and provide fall prevention interventions in the home. In this systematic review, we seek to assess the efficacy and qualitative factors determining success of these programs. METHODS: Studies reporting the outcomes of fall prevention interventions d

Toward an evidence-based approach for cholangitis diagnosis.

BACKGROUND AND AIMS: Despite improvements in imaging and laboratory medicine, consensus criteria for the diagnosis of cholangitis are lacking. Although ERCP is an effective treatment for cholangitis, it should be reserved for those patients with a high probability of the diagnosis, given the morbidity associated with the procedure. METHODS: A comprehensive literature search of PubMed (from 1898 to present), Web of Science (1900 to July 15, 2019), Embase (1943 to July 15, 2019), and the Cochrane library (1898 to July 15, 2019) was performed to identify studies that reported on diagnostic parad

Systematic review of the effect of implementing enhanced recovery after surgery on selected attributes of surgical recovery in gynecology.

This study aimed to systematically review the available literature on enhanced recovery after surgery (ERAS) following gynecologic procedures performed either as an open surgery or as a minimally invasive gynecological surgery (MIGS) in terms of outcomes. This review revealed the results of published literature and assessed the benefits and diverse outcomes of ERAS implementation in patients undergoing MIGS or other gynecologic surgeries. In this review, we sought to examine the efficacy of entire ERAS protocols, faithfully performed, to determine whether they were successful in improving indi

Immunity certification for COVID-19: ethical considerations.

Restrictive measures imposed because of the coronavirus disease 2019 (COVID-19) pandemic have resulted in severe social, economic and health effects. Some countries have considered the use of immunity certification as a strategy to relax these measures for people who have recovered from the infection by issuing these individuals a document, commonly called an immunity passport. This document certifies them as having protective immunity against severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), the virus that causes COVID-19. The World Health Organization has advised against the impl

A simulation and video-based training program to address adverse childhood experiences.

Adverse childhood experiences (ACEs) are 10 categories of childhood abuse and maltreatment, which have a dose-response relationship with common adult health concerns seen in primary care including health risk behaviors, chronic disease, and mental illness. Many of the ACEs-associated biopsychosocial risk factors are modifiable. However, physicians may not address these issues for fear of opening "Pandora's Box", that is, a source of extensive problems for which they are not sufficiently prepared with training, resources, or time. Residents need training in how to conduct trauma-focused convers

Pakistan: contraceptive prevalence rate up.

Pakistan's low sociodemographic profile has changed dramatically since the 1994 International Conference on Population and Development, said the Minister for Population Welfare and Science and Technology, HE Begum Sayda Abida Hussain, during her statement at the Forum. Based on the population census of 1998, Pakistan has an annual population growth rate of 2.3%, she said. The decline may be attributed to changes in married patterns and a rapid reduction in marital fertility, she added. The contraceptive prevalence rate has gone up from only 2% in the early 1990s to about 27% in 1998. Meeting t

Interventions to address social and structural conditions for individuals with select complex, chronic, disabling conditions: a scoping review protocol.

OBJECTIVE: The objective of the review is to describe features of interventions that address social and structural conditions for individuals with select complex, chronic conditions: multiple sclerosis (MS), inflammatory arthritis, diabetes, or frailty. INTRODUCTION: Despite advances in health care over the past 30 years, substantial heterogeneity in health outcomes persists across people with complex, chronic conditions. Social and structural conditions affect this heterogeneity and contribute to social vulnerability, marginalization and equity-seeking status. ELIGIBILITY CRITERIA: We will

Ethics in the trenches: a multifaceted analysis of the stem cell debate.

The increasing understanding of stem cell biology has opened up the possibility of using cell transplantation to treat a large variety of diseases. The medical need to identify optimal therapies is being challenged, however, by some members of society who seek to impose on this scientific quest their views--generally associated with particular religious beliefs--of what constitutes allowable research. This conflict mirrors earlier battles, extending over 150 years, between those implementing inoculation and vaccination to protect against smallpox and those who felt this to be unethical for rel

Experiences of midwives and nurses when implementing abortion policies: A systematic integrative review.

INTRODUCTION: Policy implementation can be affected by what individuals believe to be right and wrong. When implementing abortion policies, providers' moral beliefs can be relevant in the success of the implementation process. Considering that midwives and nurses are direct providers of abortion care, exploring their experiences related to abortion policy implementation could provide helpful information to prevent policy failure. METHODS: Systematic integrative review. The studies were identified through an electronic search strategy and the screening of the reference lists of all selected ar

School health initiatives in low-income and middle-income countries: a scoping review protocol.

BACKGROUND: Schools are key settings for implementing school health initiatives to prevent health problems facing learners. Initiatives exist to help learners build health literacy, adopt positive behaviours and improve their health and academic outcomes. Yet, school health outcomes have not been achieved in many settings, particularly in low-income and middle-income countries. This failure is attributed to mismatches between the initiatives and the problems being addressed, or the ill implementation of school health initiatives. Mapping these initiatives to school health problems and other sc

Interventions as experiments: Connecting the dots in forecasting and overcoming pandemics, global warming, corruption, civil rights violations, misogyny, income inequality, and guns.

This essay applies the "ultimate broadening of the concept of marketing" for designing and implementing interventions in public laws and policy, national and local regulations, and everyday lives of individuals. The ultimate broadening of the concept of marketing: Marketing is any activity, message, emotion, or behavior by someone, firm, organization, government, community, or brand executed consciously or nonconsciously that may stimulate an observable or non-observable activity, emotion, attitude, belief, or thought by someone else, group, organization, firm or community. The broadening defi

Assessment of health surveys: fitting a multidimensional graded response model.

The multidimensional graded response model, an item response theory (IRT) model, can be used to improve the assessment of surveys, even when sample sizes are restricted. Typically, health-based survey development utilizes classical statistical techniques (e.g. reliability and factor analysis). In a review of four prominent journals within the field of Health Psychology, we found that IRT-based models were used in less than 10% of the studies examining scale development or assessment. However, implementing IRT-based methods can provide more details about individual survey items, which is useful

Implications of the Use of Artificial Intelligence Predictive Models in Health Care Settings : A Simulation Study.

BACKGROUND: Substantial effort has been directed toward demonstrating uses of predictive models in health care. However, implementation of these models into clinical practice may influence patient outcomes, which in turn are captured in electronic health record data. As a result, deployed models may affect the predictive ability of current and future models. OBJECTIVE: To estimate changes in predictive model performance with use through 3 common scenarios: model retraining, sequentially implementing 1 model after another, and intervening in response to a model when 2 are simultaneously implem

Kafka's beautiful eyes: Forensic intelligence utilisation of phenotypic information.

Franz Kafka had beautiful eyes. So striking, that many of the famous author's friends and peers commented on them - but quite variously ('dark', 'brown', 'grey' & 'blue'). Eye colour as perceived by an observer is subjective, being influenced by physiological, environmental, and even sociocultural factors. In a policing context, this does not mean that trait information such as eye colour is not valuable (far from it), but that it must be managed carefully. The Australian Federal Police has recently implemented a forensic DNA phenotyping (FDP, aka. physical trait prediction or PTP) capability,

A Risk Assessment and Risk-Based Approach Review of Pre-Use/Post-Sterilization Integrity Testing (PUPSIT).

In January 2023, ICH Q9 was updated to include expanded guidance on risk-based decision-making, emphasizing its application in informing science-driven and strategic decisions. The revised guidance highlights that while quality risk management can aid decision-making, it does not eliminate the industry's obligation to comply with regulatory requirements. This article introduces a framework for evaluating the risks and benefits of pre-use/post-sterilization integrity testing (PUPSIT) using risk management principles. It provides a structured approach to assess the acceptability of alternative m
