Intestinal obstruction due to sigmoid colon volvulus during pregnancy is a rare complication but associated with significant fetomaternal mortality. We describe a case of sigmoid volvulus in a patient with 37 wk pregnancy causing huge dilation of left colon. Patient developed rectovaginal fistula following nonmedical method to relieve distention by inserting stick as told by patient.

The association of congenital aganglionic megacolon and imperforate anus is rare. Our patient was a male neonate who initially presented with intestinal obstruction due to imperforate anus without fistula. Intraoperative findings and colonic biopsy revealed the presence of concurrent Hirschsprung's disease (HD). A suspicision that these two disorders may coexist is mandatory for early correct diagnosis.

Polymicrobial anaerobic meningitis is a rare event secondary to a contiguous infection in the head or neck. Anaerobic meningitis due to a meningo-intestinal fistula is a rare event with only two cases reported in the literature. We describe a new case of adult polymicrobial anaerobic meningitis after colorectal surgery and radiotherapy and review the previous two cases.

The Hartmann situation is a temporary intestinal diversion to treat diseases of the rectosigmoid. In case of benign rectosigmoidal disorders the entirely laparoscopic performance of the colon disconnection and reconstruction is an advantage due to its minimal invasive nature. A patient suffering from chronic anorectal fistula with temporary relief exclusively performed by laparoscopic approach is reported.

In 12 children colostomy wound closure was undertaken with liberal extraperitoneal mobilisation of the bowel. In all the cases adequate length of the colonic loops could be achieved for resection of the colostomy and end to end anastomosis. There was only minor wound infection in 3 cases. There was no faecal fistula nor any postoperative intestinal obstruction.

Intraductal papillary mucinous neoplasms (IPMNs) of the pancreas present more commonly in the elderly. This report describes a case of IPMN in a 36-year-old man who presented with obstructive jaundice and weight loss. A case of reno-colic-cutaneous fistula is presented. Of several diagnostic methods, fistulography combined with computerized tomography was the most useful to locate the precise anatomical site of involvement. Two cases of colothoracic fistulae are reported. The first one is secondary to pulmonary actinomycosis, the second to complicated colic diverticulitis. The etiologies of colothoracic fistulae are multiple and can be divided in intrathoracic or intraabdominal origin. The treatment includes: effective intravenous antibiotherapy, colic resection of the concerned portion and thoracic drainage.

A case of fatal erosion of the abdominal aorta by gallstones is reported. The patient presented with a six-day history of melaena and died suddenly 39 hours after admission to hospital, with massive blood loss per rectum. At post-mortem examination a saccular cavity containing gallstones was found at the lower end of the common bile duct. The cavity communicated in front with the duodenum and behind with the aorta. This case appears to be the first of its kind to be reported.

INTRODUCTION: Enteric fistulas are a recognised complication of various diseases and surgical interventions. Non-operative medical management will result in closure of 60-70% of all fistulas over a six- to eight-week period, those that fail non-operative management will require operative intervention if they are to close.
