The choice of appropriate treatment of recurrent lateral patellar dislocation (RPD) is the source of constant debate because of unacceptable treatment outcomes. The aim of this study is to assess the outcome of operative treatment of RPD with the use of proximal realignment method by Insall. 94 patients (109 knees), who were present at the final examination, after the average follow-up 6.8 years, and met the inclusion criteria, were operated with the Insall procedure due to RPD. There were 63.3% of excellent and good results and 11% poor in the final outcome score. The outcomes of treatment of

A detailed examination was undertaken of hospitalized car occupants who sustained a lower limb injury in a frontal crash. The assessment included an analysis of the type, severity and causes of these injuries and mechanisms involved in lower limb fractures. The findings showed that fractures and dislocations occurred in 88% of lower limb injury cases, that more than half were from crashes < 48 km hour-1 and that the number of fractures was directly proportional with delta V. Ankle dislocations and foot fractures from the floor and toe pand were the most common injury-source combination overall

INTRODUCTION: Patellofemoral instability (PFI) is a common knee injury in the pediatric population, and medial patellofemoral ligament (MPFL) reconstruction is common surgery to minimize risk of chronic PFI. Medial patellotibial ligament (MPTL) reconstruction may be subsequently performed, providing more lateral stability to reduce rotational instability and tilt. Recent literature has described MPTL reconstruction in adults and nonpathologic pediatric patients; however, little literature has discussed this procedure in pediatric patients with underlying abnormal patellofemoral morphology.

PURPOSE: Abnormal bearing movements, including bearing contact with or separation from the vertical wall (VW) of the tibial component, can cause bearing dislocation or spin-out in Oxford mobile-bearing unicompartmental knee arthroplasty. This study aimed to evaluate the influence of varus placement of the tibial component on the bearing orbit in Oxford unicompartmental knee arthroplasty.
METHODS: This retrospective study included 75 knees that underwent Oxford unicompartmental knee arthroplasty. The conventional group comprised 43 knees where tibial cuts were made orthogonally to the tibial a

BACKGROUND: The management of first-time patellar dislocation remains variable, with limited evidence to support or compare different operative and nonoperative modalities. The primary aim was to establish consensus-based guidelines for different components of nonoperative treatment following a first-time patellar dislocation. The secondary aim was to develop guidelines related to management after failed nonoperative treatment. The tertiary aim was to establish consensus-based guidelines for the management of first-time patellar dislocation with a concomitant osteochondral fracture.

BACKGROUND: Isolated Patellar Aplasia Hypoplasia is a very rare autosomal dominant disorder. Its treatment depends on the clinical manifestations that can vary widely. The lack of active extension, which can be responsible for frequent falls due to a knee instability, is the most frequent and disabling manifestation. We report an original technique that is a modification of the Galeazzi technique for recurrent dislocation of the patella to gain active extension in case of PTLAH.
CASE REPORT: A 7-year-old Caucasian boy with isolated Patellar Aplasia Hypoplasia and an extension lag of the right

BACKGROUND AND AIMS: The most common mechanisms of the knee injuries are distortions and contusions. Fairly often they will result in hemarthrosis. The purpose of this study was to evaluate the findings in patients with acute traumatic hemarthrosis of the knee and based on these findings estimate the necessity of acute artroscopic examination with therapeutic procedure.
MATERIAL AND METHODS: In the years 2007 and 2009 a total of 266 patients underwent acute arthroscopic examination in Department of Traumatology the Faculty Hospital Brno after the acute knee trauma. Patients with other intraar

INTRODUCTION: Extensor mechanism disruption following total knee arthroplasty (TKA) is an uncommon but serious complication. Surgical management is often complex, particularly in cases of failed reconstruction or persistent instability, and may ultimately necessitate amputation.
CASE PRESENTATION: We report a 79-year-old female with a history of TKA and prior patellar tendon repair, who developed a fixed flexion deformity and periprosthetic knee dislocation. Given the failure of conventional interventions and compromised soft tissues, she underwent a through-knee amputation. The femoral compo…

We produced a dynamic model of experimental hip dysplasia in growing puppies by cast immobilization of a single hind leg with the knee in extension. Serial roentgenographic studies demonstrated a progressive acetabular dysplasia with subluxation and eventually dislocation of the femoral head. A consistent finding in the dysplastic hip was significant hypertrophy of the ligamentum capitum femoris, both in length and in cross-sectional area. Quantitative studies of blood flow in the femoral head by the hydrogen-washout technique in normal and dysplastic hips demonstrated a significant decrease i

Anterior knee pain is a frequent symptom in young athletes. Symptomatic patellofemoral cartilage defects can occur after trauma, especially after patellar dislocation. Numerous cartilage repair methods are currently available. Due to co-pathologies, the outcome after patellofemoral cartilage repair is inferior to the treatment of cartilage defects of the tibiofemoral joint. Adequate addressing of coexisting pathologies is essential for treatment success. This review provides an overview of the different techniques of patellofemoral cartilage repair.

The treatment of post-traumatic dislocation of the patella depends on careful examination. One should propose surgery in most cases, for not only one may leave a foreign body within the joint, but above all correction of the extensor apparatus is necessary in more than 50 p. cent of cases, to avoid a recurrence. The quality of the results depends on the preoperative analysis of the knee. Our study of 29 cases confirmed this.

CHST3-related chondrodysplasia with congenital joint dislocations (CDCJD, #MIM 143095), is a rare genetic skeletal disorder caused by biallelic loss of function variants in CHST3. CHST3 is critical for the sulfation of chondroitin sulfate. This study delineates the clinical presentation of nine individuals featuring the key symptoms of CDCJD; congenital joint (knee and elbow) dislocations, short trunk short stature progressive vertebral anomalies, and metacarpal shortening. Additional manifestations include irregular distal femoral epiphysis, supernumerary carpal ossification centers, bifid hu

Congenital dislocation of the patella is a disorder that presents with dysfunction in extensor mechanism. Although congenital dislocation of the patella mostly occurs in children with genetic disorders, it may also occur in totally healthy children, despite rarely. In this article, we report a 16-year-old male patient who referred to our clinic with complaints of gait disturbance, frequent falls, and muscular weakness in lower extremity. The patient had no complaints during walking, but had difficulty in running and walking up and down the stairs. It was observed that the range of motion of th

BACKGROUND: Traumatic patellar dislocation (TPD) may be associated with other injuries requiring additional treatment. The superficial localization of the knee extensor apparatus and knee articular cartilage make them accessible to sonographic evaluation (ultrasonography [USG]).
PURPOSE OF THE STUDY: Prospective analysis of the value of USG in TPD.
METHODS: In 21 patients, 22 knees were examined by USG after clinical and routine radiographic evaluations. Based on USG findings, operative or nonoperative treatment was done (15 and 7 cases, respectively).
RESULTS: In 17 knees, lesions of the m

Knee dislocations are complex cases requiring careful initial management to prevent devastating consequences caused by limb ischemia from vascular injury. Initial treatment includes gentle reduction and neurovascular assessment including an arteriogram and vascular reconstruction when necessary. Evaluation by examination and MR imaging will help define all ligamentous and associated pathology. All combined instabilities are repaired or reconstructed at the time of surgery. Early intervention within 3 weeks is preferred, as acute reconstruction is technically easier and more predictable than ch…

The incidence of traumatic chondral and osteochondral fractures and their role in the development of joint degeneration are not fully elucidated. While assessing traumatic knee injuries, one important criterion for the diagnosis of chondral fractures is to remember the possibility of a chondral or osteochondral fracture. Symptoms in osteochondral fractures are more obvious and cause severe pain and difficulty in movement of knee with hemarthrosis. The presence of hemarthrosis facilitates the diagnosis of an osteochondral fracture. Chondral and osteochondral fractures may be associated with oth
