HYPOTHESIS: High-resolution magnetic resonance imaging (MRI) at 1.5 T preferentially enhances the perilymph over endolymph after administration of contrast with gadodiamide, which allows for differentiation of the membranous labyrinth. Furthermore, this imaging allows for the detection of endolymphatic hydrops.
BACKGROUND: Endolymphatic hydrops is believed to be associated with a number of ear diseases, including Ménière's disease. Although the pathologic changes of overaccumulation of endolymph in the inner ear are obvious on postmortem histologic examination of the temporal bone, they have

Arrhythmogenic cardiomyopathy (ACM) epitomises a genetic anomaly hallmarked by a relentless fibro-fatty transmogrification of cardiac myocytes. Initially typified as a right ventricular-centric disease, contemporary observations elucidate a frequent occurrence of biventricular and left-dominant presentations. The diagnostic labyrinth of ACM emerges from its clinical and imaging properties, often indistinguishable from other cardiomyopathies. Precision in diagnosis, however, is paramount and unlocks the potential for early therapeutic interventions and vital cascade screening for at-risk indivi

Development of the video head impulse test (vHIT) assessing all three semicircular canals in both labyrinths has uncovered the existence of new vestibular failure patterns and made bilateral posterior canal dysfunction detection possible. We conducted a retrospective analysis of 41 patients with bilateral posterior semicircular canal failure and compared results to 37 controls, with normal posterior semicircular canal function. Mean calculated gain showed significant difference between patients and controls in right [0.54 (SD 0.016)] and left [0.57 (SD 0.014)] posterior semicircular canals. Th

Meniere's Disease (MD) is a complex disorder associated with an accumulation of endolymph in the membranous labyrinth in the inner ear. It is characterized by recurrent attacks of spontaneous vertigo associated with sensorineural hearing loss (SNHL) and tinnitus. The SNHL usually starts at low and medium frequencies with a variable progression to high frequencies. We identified a novel missense variant in the PRKCB gene in a Spanish family with MD segregating low-to-middle frequency SNHL. Confocal imaging showed strong PKCB II protein labelling in non-sensory cells, the tectal cells and inner

We describe a mouse model of fetal loss in factor V Leiden (FvL) mothers in which fetal loss is triggered when the maternal prothrombotic state coincides with fetal gene defects that reduce activation of the protein C anticoagulant pathway within the placenta. Fetal loss is caused by disruption of placental morphogenesis at the stage of labyrinth layer formation and occurs in the absence of overt placental thrombosis, infarction, or perfusion defects. Platelet depletion or elimination of protease-activated receptor 4 (Par4) from the mother allows normal placentation and prevents fetal loss. Th

HYPOTHESIS: The study of infant temporal bones with tuberculosis (TB) of the middle ear and mastoid could provide information to assist with clinical diagnosis in this population.
BACKGROUND: The TB pandemic has become a critical global public health problem. With the rising incidence of the disease, otolaryngologists might encounter an increased frequency of otologic TB. Pediatric temporal bone reports of TB are rare.
METHODS: Light microscopic examination was performed on both temporal bones from an infant who died as a result of miliary TB.
RESULTS: The tympanic membranes were thickened

Cats with the cochlear aqueduct patent were placed in a pressure chamber and exposed for 10 min to hypobaric pressures of 5.1 and 6.8 kPa relative to atmospheric pressure. The experiments were designed according to a program used for treatment of Meniere's disease. The perilymph, middle ear, cerebrospinal fluid (CSF), venous, arterial and chamber pressures were recorded. The results demonstrated that hypobaric effects on the labyrinth were mediated via pressure changes in the middle ear and not via a systemic vascular or CSF influence. A reduction in chamber pressure induced a relative increas

HYPOTHESIS: Untreated otitis media (OM) may associate with progressive histopathological changes in the peripheral vestibular system structures in the chinchilla model.
BACKGROUND: Previous studies have suggested a link between OM and vestibular pathology. However, the mechanisms underlying this relationship are not well understood.
OBJECTIVES: To evaluate the progression of peripheral vestibular pathology in chinchilla temporal bones with OM.
METHODS: Chinchilla temporal bones, both control and Streptococcus pneumoniae -inoculated, were obtained from the Paparella Otopathology & Ear Pathog

Lassa fever (LF), caused by Lassa virus (LASV) infection, typically leads to mild symptoms in humans, but some survivors experience audiovestibular problems. Here we present vestibular histopathological insights in our LF model mice. We observed (1) hemorrhage within the vestibular ganglion and stroma beneath the sensory epithelium, (2) preserved hair cells and supporting cells, (3) LASV antigen presence in the vestibular ganglion cells and the stroma beneath the sensory epithelium, and (4) CD3-positive T-lymphocyte infiltration in the vestibular ganglion and the stroma underlying the sensory

Sudden unilateral loss of vestibular function is the most severe condition that can occur in the vestibular system. The clinical syndrome is caused by the physiologic properties of the vestibulo-ocular reflex (VOR) arc. In the normal situation, the two peripheral vestibular end organs are connected to a functional unit in coplanar pairs of semicircular canals working in a push-pull mode. "Push-pull" mode means that, when one side is excited, the other side is inhibited, and vice versa due to two mechanisms. First, first-order vestibular afferents are bipolar cells. They have a tonic firing rat

Certain peripheral vertigos undoubtedly do not raise from neuro-sensorial structures but rather of the secretory structures and can be called "endolymphatic". They deserve to be individualized because concerning different pathogenic mechanisms and certainly are relevant for a specific treatment. We hope that in the next years new investigation tools will allow to explore in a noninvasive way the endolymphatic contents and dysfunctions of these structures.

Autoimmune vestibulo-cochlear disorders (AVCD) represent a group of syndromes with overlapping clinical features, manifesting as sensorineural hearing loss, often associated with vertigo, tinnitus, and aural fullness, and believed to be caused by an autoimmune mechanism. Although definitive evidence of a classic "autoimmune process" is still lacking, substantial indirect evidence has accumulated to strongly indicate such a pathogenesis. Rapidly progressing AVCD is analogous to rapidly progressive glomerulonephritis in that inner ear inflammation progresses to severe, irreversible damage within

Recently Meniere's disease is believed to show a pathological features of endolymphatic hydrops, but the etiology of this disease has not yet been ascertained. Many investigators tried to create animal models with endolymphatic hydrops by obliterating endolymphatic sac and duct. However, these methods have not been adequate to explain the mechanism of development of Meniere's disease, because it gives mechanical damage to destroy endolymphatic sac and duct. On the other hand, Meniere's disease is thought to develop even as a result of stress. The present paper reported the conduction of an acu

Insertion of a sound amplification device into the round window niche (extracochlear implant) or into the coils of the cochlea (intracochlear implant) can give significant benefits to some carefully selected, severely deaf patients. Imaging has an essential role in selective and pre-operative assessment. Severe otosclerosis and post-meningitic labyrinthitis ossificans are common causes of deafness in these patients and can be demonstrated by computed tomography (CT). The most suitable side for operation can be assessed. We describe our experiences with 165 patients, 69 of whom were found suita

Multiple-frequency rotatory testing was performed in a series of 12 normal subjects and 27 patients who had significant labyrinthine paralysis. Fourteen of the patients had total loss of labyrinthine function. Phase leads of the responses were much greater in the lower-frequency measurements and measured gains of the responses to ampullopetal stimulation were qualitatively greater than those of the responses to ampullofugal stimulation. However, many patients' responses continued to fall within the normal range because of the large variability of the responses of normal subjects. The value of

Multi-frequency tympanometry brings into evidence, in the normal subject, three types of tympanogram of acoustic impedance. The frequency of the tone probe showing a W tympanogram seems to be the key element. This frequency is close to that of the resonance of the ear, which is altered in the case of pathology of the tympano-ossicular system and of increase in the pressure of the endolabyrinthine fluids. Multi-frequency tympanometry facilitates the differential diagnosis between ankylosis and a discontinuity of the ossicular chain. It also objectifies the variations in the pressure of the inne
