Thermoablative Techniques to Treat Excessive Central Airway Collapse.

Excessive central airway collapse (ECAC) is a condition characterized by the excessive narrowing of the trachea and mainstem bronchi during expiration, which can be caused by Tracheobronchomalacia (TBM) or Excessive Dynamic Airway Collapse (EDAC). The initial standard of care for central airway collapse is to address any underlying conditions such as asthma, COPD, and gastro-esophageal reflux. In severe cases, when medical treatment fails, a stent-trial is offered to determine if surgical correction is a viable option, and tracheobronchoplasty is suggested as a definitive treatment approach. Radiation-induced damage in the lower gastrointestinal tract: Clinical presentation, diagnostic tests and treatment options.

Radiation therapy is an important ally when treating malignant lesions in the pelvic area, but it is not exempt of adverse events. There are some measures that can be taken to reduce the possibility of these effects, but some are non-modifiable factors related to previous treatments, location of the lesions or comorbidities. There is a wide variety of clinical presentations that can be of an acute or chronic onset that go from mild to severe forms or that can have a great impact in the quality of life. Medical available therapies as metronidazole, sucralfate, mesalizine or probiotics, can be o

Portal hypertensive gastropathy and gastric antral vascular ectasia.

Portal hypertensive gastropathy (PHG) and gastric antral vascular ectasia (GAVE) are gastric mucosal lesions that mostly present as chronic anemia and rarely cause the acute gastrointestinal hemorrhage. Despite similar clinical manifestations, their pathophysiology and management are entirely different. PHG is seen exclusively in patients with portal hypertension, but GAVE can also be observed in patients with other conditions. Their diagnosis is endoscopic, and although generally each of them has a characteristic endoscopic appearance and distribution, there are cases in which the differentia

Surgical management of life-threatening epistaxis in Osler-Weber-Rendu disease.

Hereditary hemorrhagic telangiectasia is a systemic disorder of blood vessels. It often leads to severe epistaxis that is refractory to conventional therapy. Three patients with severe epistaxis caused by hereditary hemorrhagic telangiectasia unresponsive to laser coagulation and to septal dermoplasty and requiring multiple transfusions underwent extirpation of nasal mucosa through a rhinotomy approach, with a median forehead flap and split-thickness skin graft coverage of the mucosal defects over the floor and lateral walls of the nose. All patients had reduced frequency and severity of bleed…

[The results of removing a transparent lens in Marfan's syndrome].

In 112 eyes of 65 patients a clear lens in the Marfan's syndrome was removed through the approach in the flat part of the ciliary body. Before operation, in all patients who had peripheral degeneration of the retina peripheral prophylactic argon laser coagulation of the retina was made. At the time of operation, in 12 eyes some pieces of lenticular substance penetrated into anterior segments of the vitreous body, which in all cases was removed by means of a lens vitreotome. One month after lensectomy, in 1 patient retinal detachment developed that totally reattached after surgical intervention…

Inhibitory mechanism of thrombin (FIIa) by garlic-derived H2S: persulfidation cleavage of the Cys42-Cys58 disulfide bond triggers FIIa cascade inactivation.

Thrombin (FIIa) is a pivotal serine protease in the coagulation cascade. Its aberrant activation drives pathological thrombosis, myocardial infarction, and stroke. Although garlic-derived sulfur compounds are known for cardiovascular protection, their redox-dependent regulation of key coagulation proteases remains largely undefined. In this study, we used a multi-scale approach combining enzymatic assays, structural characterization, molecular dynamics, and functional validation to elucidate how garlic-derived hydrogen sulfide (H2S) inhibits FIIa. Real-time activity assays demonstrated that ga

Argon plasma coagulation (APC) in gastroenterology: experimental and clinical experiences.

BACKGROUND: Diathermy procedures are indispensable in interventional endoscopy. Argon beam coagulation is an innovative no-touch electrocoagulation technique in which high-frequency alternating current is delivered to the tissue through ionized argon gas. METHOD AND PATIENTS: Before clinical application, we conducted in-vitro studies to investigate the depth and diameters of tissue coagulation in fresh operative specimens from the stomach, small intestine and colon. Five different power/gas flow settings between 40 and 155 W and 2 and 7 l/min were used. The impact time (1-10s) and the inciden

Novel flapless esthetic procedure for the elimination of extended gingival metal tattoos adjacent to prosthetic teeth: Er:YAG laser micro-keyhole surgery.

PURPOSE: This article describes a novel, minimally invasive procedure called Er:YAG laser micro-keyhole surgery (EL-MIKS) that removes metal or amalgam tattoos in the gingiva adjacent to prosthetic teeth without gingival deformity and recession. We aimed to evaluate the clinical efficacy of EL-MIKS for removing metal tattoos and present its long-term treatment outcomes. METHODS: The EL-MIKS procedure consists of several steps that are all performed under a microscope. First, a micro-keyhole (1-2 mm diameter) was prepared using an Er:YAG laser in the center of the metal pigmentation. The conne

DYRK1A retinopathy.

We present a case of retinopathy in a 4-year-old girl with DYRK1A syndrome. On external examination, she had short stature, cognitive delay, microcephaly, and iris coloboma of the right eye. On fundus examination of both eyes, she was found to have lattice degeneration and areas of avascularity in the retinal periphery, with overlying hyaloidal organization. Widefield fluorescein angiography showed vascular pruning and vascular arborization with leakage. Given the risk for progression to retinal detachment, laser photocoagulation was applied to areas of peripheral avascular retina. To our know

PURPOSE: To present a unique case of optic disk pseudo-duplication with proliferative diabetic retinopathy. METHODS: Case report. RESULTS: A 63-year-old white diabetic man presented with an apparent duplicated optic disk in the superonasal midperiphery of his left eye. A large flat frond of neovascularization fanned out from this structure. Optical coherence tomography scanning showed a noncolobomatous scar with a large plume of blood vessels sprouting from the choroid, thru the retina and branching out into the vitreous. Magnetic resonance imaging scanning revealed a normal left globe and o

Outcomes of Subliminal Transscleral Cyclophotocoagulation treatment in glaucoma.

BACKGROUND: Subliminal transscleral cyclophotocoagulation (SL-TSCPC) is a new alternative therapy to reduce intraocular pressure (IOP) safely and effectively. However, there are few studies regarding SL-TSCPC by Supra 810 laser machine and limited data regarding its effectiveness in moderate severity glaucoma that still has good preservation of vision. This study was conducted to evaluate the outcome of SL-TSPCPC in various types of glaucoma including patients with good vision. METHODS: A retrospective, non-comparative, analytical case series of all patients who received SL-TSCPC treatment fr
