Implications of endoscopic endonasal surgery for the treatment of postoperative maxillary mucoceles.

Recently, endoscopic endonasal surgery has been widely used to treat chronic sinusitis with good results being reported by many investigators. Endoscopic endonasal surgery is a technique available not only for chronic sinusitis but also for other sinus diseases including postoperative maxillary mucoceles. In this report, the indications and limitations of endoscopic endonasal surgery for the treatment of postoperative maxillary mucoceles are discussed based on our experience treating 26 such mucoceles at our clinic. The indications for endoscopic endonasal surgery include mucoceles in close co

Efficacy of sinus irrigation versus sinus irrigation followed by functional endoscopic sinus surgery.

Sinus irrigation is the traditional treatment for chronic maxillary sinusitis. Functional endoscopic sinus surgery (FESS) restores aeration and allows secretions to be removed from an infected sinus. This study compares the efficacy of sinus irrigation with that of sinus irrigation followed by FESS in 89 patients. We measured the effects by way of sinus radiographs, nasendoscopic findings, and patient complaints. When we analyzed the data in terms of intent to treat, we found significantly favorable results for sinus irrigation followed by FESS at the end point, though only for loss of smell a…

[Orbital and endocranial complications in acute sinusitis in childhood].

Acute infection the paranasal sinus is a rather frequent pathology in children. On the contrary, local orbital complications are rare. The employment of antibiotics reduces the incidence of complications but can sometimes conceal their appearance so that diagnosis is delayed. Orbital as well as intracranial complications of ethmoidal and maxillary sinusitis are most often encountered in childhood. Within a brief time span, 8 patients with complicated frontal and ethmoid-maxillary sinusitis were diagnosed and treated: 4 cases of periorbital cellulitis, 2 of subperiorbital abscess and 2 of orbit…

Sinonasal Cholesteatoma Presenting Within the Maxillary Sinus: A Case Report.

OBJECTIVES: Epidermal inclusion cysts are a common type of benign cutaneous cyst. When located in the mastoid cavity, they are more commonly known as cholesteatomas. However, it is extremely rare to be present within the paranasal sinuses, with 20 known cases involving the maxillary sinus. This is a case presentation of an epidermal inclusion cyst of the maxillary sinus, describing its clinical presentation, identifying pathological correlations, and highlighting the importance of early recognition and management. STUDY DESIGN: A case report. METHODS: A retrospective review of the medical re

Evaluation of the posterior superior alveolar artery and the maxillary sinus with CBCT.

Assessment of the maxillary sinus anatomy before sinus augmentation is important for avoiding surgical complications, because of the close anatomical relationship between the posterior maxillary teeth and the maxillary sinus. The posterior superior alveolar artery (PSAA) is the branch of the maxillary artery that supplies the lateral sinus wall and overlying membrane. We evaluated the location of the PSAA and its relationship to the alveolar ridge and maxillary sinus using cone beam computed tomography (CBCT). The study group consisted of 135 CBCT scans (270 sinuses) obtained from the archive

[Revision endoscopic sinus surgery--long-term follow up and operative findings].

Among 170 cases (301 sides) with chronic sinusitis who underwent endoscopic sinus surgery, 15 cases (25 sides) were reoperated on because of unsatisfactory results. We examined 12 such cases, periodically, with an endoscope following the first operation. Postoperative endoscopic management, including removal of polyps in the middle nasal meatus and irrigation of the maxillary sinus with sterilized saline water through the enlarged opening, had been done if necessary. However, stenosis of the middle nasal meatus had gradually progressed and mucous or purulent discharge had increased despite pos

Computed tomography scan findings in refractory acute rhinosinusitis.

Several sinonasal anatomical variants may be related to the etiology of acute rhinosinusitis. The same is true with intrusion of teeth into the maxillary sinus cavity. Patients with acute rhinosinusitis not responding to maximal medical treatment were prospectively enrolled. Deviation of the nasal septum towards the infected sinus may be associated with the pathogenesis of refractory acute rhinosinusitis. Intrusion of teeth into the maxillary sinus is a common finding and is not necessarily associated with the formation of sinusitis.

[Silent sinus syndrome. Clinical case].

CLINICAL CASE: A 53 year-old man presented with a progressive enophthalmos without any sinus or nasal symptoms. There was no history of a trauma. The ophthalmology examination showed enophtalmos and hypoglobus. The computerized tomography (CT) showed a collapsed maxillary and frontal sinus and a lateral deviation of the nasal septum that led us to the diagnosis. DISCUSSION: The clinical features of silent sinus syndrome are described, as well as the need to distinguish it from maxillary sinusitis.

[X-Ray and clinical study of the nose, sinuses and maxilla in patients with severe iron deficiency disease (author's transl)].

A definite relation between ozaena and iron deficiency disease could not be verified. However, the examination of 88 patients with severe iron deficiency disease, mostly of juvenile age, revealed that X-ray pictures of the nose and the paranasal sinuses showed abnormal alterations indicating the existence of a non-inflammatory process. There were few symptoms from the nose and the neighbouring structures as well as insignificant clinical findings by inspection of these structures. The increased opacity of the maxillary sinuses on the roentgenograms and some other changes are considered to be p

[Maxillary sinus infection by Bacillus licheniformis: a case report from Djibouti].

Aerobic, spore-forming gram-positive Bacillus spp infections are rare and reported mainly in immunocompromised hosts. We report a case of acute unilateral maxillary sinusitis, caused by Bacillus licheniformis, in a 35-year-old French soldier stationed in Djibouti. It was easily identifiable due to its typical culture and resistance profile. This case is interesting for two reasons: first, it is, to our knowledge, the first case of sinusitis attributed to this microbe, and second, it has rarely been described in immunocompetent patients without altered skin or mucous membranes.

[Clinical and experimental rationale of policatan administration in upper respiratory tract inflammation].

Experimental and clinical trials of policatan in ENT inflammation have established that maximal antiinflammatory effect of the drug manifests in experimental serotonin, histamine and corragenin edema, less pronounced--in formaline-induced edema. The drug accelerated healing of infected burn ulcer in rabbit nose by 37%. This may be explained not only antiinflammatory effect of policatan, but also by stimulation of macrophage phagocytosis and bacteriostatic potential. Policatan proved effective in acute and chronic maxillary sinusitis, chronic frontal sinusitis and rhinitis, chronic hyperplastic…

Lymphocyte subsets of maxillary mucosa in chronic inflammation.

Subsets of infiltrating lymphocytes within maxillary sinus mucosae of patients with chronic sinusitis were investigated by immunoperoxidase staining of frozen sections with the use of monoclonal antibodies. The most commonly observed infiltrating cell type was suppressor/cytotoxic T cells (CD8+) and smaller subpopulations of lymphocytes were helper/inducer T cells (CD4+) and B cells (CD20+). Variable numbers of HLA-DR+ cells were commonly observed in the lamina propria. The fibrous type of chronic sinusitis was found to have more suppressor/cytotoxic T cells (CD8+) and lower CD4/CD8 ratio than

Chronic sinusitis in children with respiratory allergy: the role of antimicrobials.

We evaluated the role of antimicrobials in the treatment of chronic maxillary sinusitis in children with respiratory allergy. Night and day cough, nasal obstruction, rhinorrhea, postnasal seen. Eighty-four children were treated in a double-blind manner with either amoxicillin, erythromycin, trimethoprim-sulfamethoxazole, or an antihistamine decongestant (carbinoxamine maleate-pseudoephedrine HCl). Radiographic and clinical responses were best with amoxicillin, but trimethoprim-sulfamethoxazole was an adequate alternative. This study demonstrates that allergic children with chronic sinusitis wi…

Kartagener syndrome with abnormalities of cilia.

Kartagener syndrome of sinusitis, bronchiectasis and situs inversus has been so called since his classical description in 1933. This report presents two further cases, a brother and sister. They were extensively studied and the findings are reported. A unique finding was that in both patients, both in the maxillary sinuses and in the bronchi, abnormal cilia were discovered on electron microscopy. After a search of the literature the authors conclude that this finding is probably congenital rather than acquired and may explain the development of the sinusitis and bronchiectasis.

Postmortem CT analysis of paranasal sinuses using an experimental model of drowning.

Fluid-filled paranasal sinuses are suggested to be a valuable tool to distinguish between drowning and non-drowning postmortem, yet the mechanisms governing fluid entry remains unknown. We investigate if fluid-filled paranasal sinuses are caused by a passive influx from submersion or an active aspiration mechanism during drowning. The ovine nasal cavity and maxillary sinuses are remarkably similar anatomically to humans, and have been used for endoscopic surgical training in recent decades. We submerged 15 decapitated ovine heads from agricultural waste at a depth of 2 m in flowing water for 1
