The Cause of Acute Respiratory Failure Predicts the Outcome of Noninvasive Ventilation in Immunocompromised Children.

BACKGROUND: Noninvasive ventilation (NIV) may be superior to conventional therapy in immunocompromised children with respiratory failure. M … ive for NIV failure. We observed catecholamine treatment in 14/41 (34%), pneumothorax in 2/41 (5%), mediastinal emphysema in 3/41 (7%), a life threatening nasopharyngeal hemorrhage and need for resuscitation during intubation in 5/41 (12%) NIV-episodes. CONCLUSIONS: The prognosis of ARF in immunocompromised children remains guarded independent of initial success or fa …

Pneumomediastinum as a complication to treatment of mediastinal (thymic) large B-cell lymphoma.

Mediastinal (thymic) large B-cell lymphoma (Med-DLBCL) is a subtype of diffuse large B-cell lymphomas (DLBCL) with a typical radiological appearance of bulky anterior mediastinal mass, often with areas of necrosis. We report a case of Med-DLBCL with unusual radiological findings and clinical development. Computed tomography (CT) obtained at presentation revealed a huge anterior mediastinal tumor with an axial diameter of 180 mm. Nineteen days after the first cycle of chemotherapy, chest radiography and CT revealed large areas of tumor necrosis and pneumomediastinum with air-fluid levels. To ou

Colon Perforation by Compressed Air: A Case Report and Short Review of Literature.

Barotrauma-associated perforation of the colon is not common and usually occurs due to the passage of compressed air through the anus. Most of the cases are accidental and done for fun often at the victim's workplace. Therefore, it is necessary that the workers should be made aware of the dangers of the equipment they regularly use at their workplace. Here, we describe one such case where a rice mill worker died when compressed air through an air pump pipe entered his rectum. His chief complaint was abdominal pain and breathing difficulty. Computed tomography scan of the abdomen and thorax sho

[Spontaneous pneumomediastinum: about 18 cases].

Spontaneous pneumomediastinum is defined as the presence of air in the mediastinum in the absence of traumatic or iatrogenic cause. Diagnosis is based on chest X-ray. Other paraclinical examinations, such as chest tomodensitometry or bronchial or gastrointestinal endoscopy, are sometimes necessary. Outcome is most often favorable. We report 18 cases of spontaneous pneumomediastinum whose data were collected in the Department of Respiratory Diseases at the Ibn Rochd University Hospital in Casablanca between 2006 and 2017. The study involved 13 men and 5 women, with an average age of 24 years. C

A 4-year-old child presenting morning onset of spontaneous tracheal rupture due to bronchial mucous plug occlusion during the nighttime sleep: a case report.

BACKGROUND: Coughing is the most efficient mechanism for clearing mucus and fluid secretions from the airways and its reflex can be suppressed by sleep. Spontaneous tracheal ruptures are believed to result from raised intratracheal pressure against a closed glottis, such as for severe coughing. This is the first reported case of tracheal rupture presented on morning awakening after bronchial mucous plug formation during the nighttime sleep because of an ineffective cough reflex. CASE PRESENTATION: An otherwise healthy white 4-year-old child presented morning onset of dyspnea, chest pain and d

Bilateral pneumothorax following acute inhalation injury.

INTRODUCTION: Inhalation injury can be thermal and/or chemical. We report bilateral pneumothorax following acute inhalation injury. CASE REPORT: A male worker in an upholstery factory was confined in the tanning machine for 15 min. The device was used to contain sodium sulfate, sulfur dioxide, and sulfuric acid. On admission, he was confused with Glasgow coma scale score as 9. His vital signs were as follows: blood pressure, 80/58 mmHg; pulse rate, 114 bpm; respiratory rate, 30 bpm; temperature, 37.1 degrees C; and oxygen saturation, 48%. He was intubated. Physical examination disclosed exten

Intracardiac thrombus and pulmonary embolism with cavitation and pneumomediastinum in a patient with protein S deficiency.

We present a case of intracardiac thrombus with pulmonary embolism and pulmonary infarction in a patient with protein S deficiency. Secondary infection of the pulmonary infarction resulted in pneumomediastinum. Thrombus in an unusual location in a young patient without known predisposing conditions is a characteristic feature of a hereditary thrombophilic disorder. Cavitation within a lesion adjacent to the mediastinal pleura can result in pneumomediastinum.

Paraquat poisoning: findings on chest radiography and CT in 42 patients.

The purpose of this study is to describe the radiologic manifestations of paraquat-induced pulmonary damage, with special emphasis on the sequential changes seen with this condition. Paraquat is a herbicide that has toxic effects on the lungs, liver, and kidneys. Progressive respiratory failure is a frequent cause of death. We analyzed retrospectively 42 patients with a history of paraquat ingestion and abnormal findings on chest radiographs. Radiographic changes during the first week after ingestion included diffuse consolidation (26/39), pneumomediastinum with or without pneumothorax (15/39)

Pediatric tracheostomy and associated complications.

A retrospective analysis of 123 pediatric tracheostomies reveals an overall complication rate of 33%. Immediate complications were present in 12% or 15 patients. The most frequent immediate complications were pneumomediastinum and pneumothorax. Delayed complications occurred in 24% or 30 patients. The most frequent delayed complications were subglottic stenosis, fused vocal cords, and tracheal granuloma. Four patients died because of tracheostomy-related complications. Age, underlying disease, and prior endotracheal intubation had a high degree of correlation with complications. The use of a m

Spontaneous pneumomediastinum and/or pneumothorax in anti-MDA5 dermatomyositis: a refined staging system.

BACKGROUND: Spontaneous pneumomediastinum and/or pneumothorax (SPM/P) have been Linked to unfavorable outcome in anti-melanoma differentiation-associated gene 5 positive dermatomyositis (MDA5+DM) patients. This study attempted to investigate the prognostic value of SPM/P and identify the predisposing factors of SPM/P in patients with interstitial lung disease (ILD) complicated with MDA5+DM. METHODS: This study was conducted in a large inception cohort of MDA5+DM-ILD (from 2014 to 2023) at Shanghai Renji Hospital. Baseline demographic data, pulmonary function tests, high-resolution CT imaging,

Pneumomediastinum, pneumatosis intestinalis and pneumoperitoneum in a patient with polymyositis: case-based review.

Pneumomediastinum (PnM), pneumatosis intestinalis (PI), and pneumoperitoneum (PP) are rare complications of inflammatory myositis. We present a 59-year-old polymyositis (PM) patient who experienced all three complications simultaneously. The patient who presented with proximal muscle weakness, dysphagia, and weight loss was diagnosed with PM due to elevated muscle enzymes and consistent electromyography and muscle biopsy with inflammatory myopathy. On the 45th day of her immunosuppressive treatment, PnM, PI, and PP were detected incidentally in 18F-fluorodeoxyglucose (FDG) positron emission to

A Fire Extinguisher Death: The Macklin Effect.

We present the case of an adult white man found dead in a psychiatric institution with fine white powder (monoammonium phosphate) deposited over the entire face after he insufflated the contents of a dry chemical fire extinguisher. Fine white powder was present within the mouth and sinuses and lined the upper airways. On opening the thoracic cavity, approximately 500 g of fine white powder was present within the right thoracic cavity. The esophagus was ruptured. Traumatic emphysema of the posterior sternum wall was present (pneumomediastinum). The ethmoid bones were fractured by the barotrauma

Occult pneumomediastinum in blunt chest trauma: clinical significance.

INTRODUCTION: Thoracic injuries are potentially responsible for 25% of all trauma deaths. Chest X-ray is commonly used to screen patients with chest injury. However, the use of computed tomography (CT) scan for primary screening is increasing, particularly for blunt trauma. CT scans are more sensitive than chest X-ray in detecting intra-thoracic abnormalities such as pneumothoraces and pneumomediastinums. Pneumomediastinum detected by chest X-ray or "overt pneumomediastinum", raises the concern of possible aerodigestive tract injuries. In contrast, there is scarce information on the clinical s

[Tracheal rupture from blunt chest injury in a child].

Tracheobronchial lesions after blunt chest injury are rare. Diagnosis and therapy can prove to be challenging for the attending physician. Nonrecognition of this injury may entail severe complications. Herein we report the case of a 9-year-old girl with rupture of the trachea due to a fall against the handlebar of her kickboard. The clinical, radiological, and features of the therapy performed are discussed.

The diagnosis of potentially fatal asthma.

The concept of life-threatening asthma is not new. Reviews of fatal asthma often describe retrospectively identified factors that were associated with or contributed to the patient's demise. We report 55 patients considered to have the diagnosis of potentially fatal asthma who were managed for 4.8 +/- 4.3 years per patient. The diagnostic criteria of potentially fatal asthma included at least one of the following four potentially fatal asthma events: 1) mechanical ventilation for respiratory arrest or failure, 2) acute respiratory acidosis that did not necessitate mechanical ventilation, 3) tw
