Effect of open wedge high tibial osteotomy on the lateral tibiofemoral compartment in sheep. Part III: analysis of the microstructure of the subchondral bone and correlations with the articular cartilage and meniscus.

PURPOSE: First, to evaluate whether medial open wedge high tibial osteotomy (HTO) induces alterations of the microstructure of the lateral tibial subchondral bone plate of sheep. Second, to test the hypothesis that specific correlations exist between topographical structural alterations of the subchondral bone, the cartilage and the lateral meniscus. METHODS: Three experimental groups received biplanar osteotomies of the right proximal tibiae: (a) closing wedge HTO (4.5° of tibial varus), (b) opening wedge HTO (4.5° tibial valgus; standard correction) and (c) opening wedge HTO (9.5° of valgus…

Primary Fixation and Cyclic Performance of Posterior Horn Medial Meniscus Root Repair With Knotless Adjustable Suture Anchor-Based Fixation: A Human Biomechanical Evaluation Over 100,000 Loading Cycles.

BACKGROUND: Recent biomechanical evidence for adjustable suture anchor (ASA)-based posterior medial meniscus root (PMMR) fixation has shown promising results compared with conventional transtibial pull-out repair (TPOR). However, ASA fixation has not been evaluated in human tissue to 100,000 cycles. HYPOTHESIS: ASA repair would lead to increased primary fixation strength and less cyclic displacement than conventional TPORs. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 32 human medial menisci were used, 8 of which were intact specimens and served as native controls. For the…

Anterior Cruciate Ligament Rupture with Medial Collateral Ligament Tear with Lateral Meniscus Posterior Root Tear with Posterolateral Tibia Osteochondral Fracture: A New Injury Tetrad of the Knee.

INTRODUCTION: Injuries to the knee ligaments, menisci, and cartilage are possible in high-velocity trauma as in road traffic accidents. Similarly, these structures can be disrupted in proximal tibia fractures. We present a series of three cases which had a previously undescribed injury combination. CASE PRESENTATION: The first and second patients presented primarily to us following fall from motorbikes. Both these patients had injuries of the anterior cruciate ligament (ACL), medial collateral ligament (MCL), lateral meniscus body and posterior root tear, and osteochondral fracture of postero…

Comparison of two simple stitches and modified Mason-Allen suture for medial meniscus posterior root tear based on the progression of meniscal posterior extrusion: A retrospective cohort study.

Purpose: Medial meniscus (MM) posterior root (PR) tear leads to severe MM posterior extrusion (PE), resulting in rapid knee cartilage degeneration. MMPR repairs are recommended to reduce MMPE, especially during knee flexion. However, the difference in MMPE between different repair techniques remains unknown. This study aimed to investigate preoperative and postoperative MMPE following several pullout repair techniques. We hypothesized that a technique using two simple stitches (TSS) would be more useful than FasT-Fix-dependent modified Mason-Allen suture (F-MMA) to prevent the progression of M…

Shared versus independent tibial tunnels for lateral meniscus posterior root repair with ACL reconstruction: Comparable outcomes with shorter operative time in the shared tunnel technique at 2 years.

PURPOSE: Lateral meniscus posterior root (LMPR) tears frequently accompany anterior cruciate ligament (ACL) injuries and can compromise knee biomechanics and stability if left unrepaired. The primary research question was whether LMPR repair performed using a shared ACL tibial tunnel provides clinical, patient-reported and magnetic resonance imaging (MRI) outcomes equivalent to those of an independent transtibial pullout technique. METHODS: A retrospective comparative study included 50 patients who underwent arthroscopic ACL reconstruction with concomitant LMPR repair between June 2019 and Ju…

Biomechanical Comparison of an All-Inside Meniscal Repair Device Construct Versus Pullout Sutures for Arthroscopic Transtibial Repair of Posterior Medial Meniscus Root Tears: A Matched-Pair Cadaveric Study.

BACKGROUND: Meniscus root repairs are important for restoring knee function after a complete meniscus root tear. Various suturing patterns have been proposed for the root repair. The 2-simple-stitches (TSS) method is currently the preferred technique, as it is simplest to perform and allows the least displacement of the meniscus root. PURPOSE: To compare the biomechanical properties of a posterior medial meniscus transtibial root repair consisting of an all-inside meniscal repair device (AMRD) construct with the TSS pullout suture pattern. STUDY DESIGN: Controlled laboratory study. METHODS:…

Comparison of Functional Recovery Between Unicompartmental Knee Arthroplasty and Medial Meniscus Posterior Root Tear Repair in Patients Aged over 50 Years: A One-Year Propensity Score-Weighted Study.

BACKGROUND: Medial meniscus posterior root tear (MMPRT) accelerates knee osteoarthritis progression. Meniscal root repair (MRR) in patients older than 50 years is controversial due to unpredictable healing and mild to moderate cartilage degeneration, which may lead surgeons to consider unicompartmental knee arthroplasty (UKA). This study compared clinical outcomes of UKA versus MRR in patients older than 50 years who had MMPRT and Kellgren-Lawrence (KL) grade ≤ 3 arthritis. METHODS: We retrospectively reviewed patients aged 50 to 79 years who had magnetic resonance image-confirmed MMPRT who u…

Meniscus Reconstruction Using Autologous Tendon Combined With Open-Wedge High Tibial Osteotomy: A Technique to Achieve Rigid Fixation and Avoid Interference Between Locking Screws and Tibial Bone Tunnel.

Meniscal posterior root tears reportedly result in accelerated cartilage degeneration and spontaneous insufficiency fractures of the knee. While meniscus repair combined with open-wedge high tibial osteotomy is an optional method, the healing rate is not sufficiently high. Therefore, this Technical Note describes a technique for meniscal reconstruction combined with open-wedge high tibial osteotomy that may offer improvements in meniscal healing rates and clinical results.

Medial meniscus injury changed plantar pressure distributions and decreased posture stability especially in those with varus alignment: a cross-sectional study based on a wearable smart plantar pressure system.

BACKGROUND: Medial meniscus (MM) injuries are common and often contribute to knee osteoarthritis (KOA). While studies focus on joint degeneration, the role of extrinsic factors such as postural control remains underexplored. This study investigated how MM injuries affected postural control, particularly plantar pressure distribution, with an emphasis on lower limb alignment. METHODS: 83 participants were recruited: 29 healthy subjects, 29 MM patients with neutral alignment (-3°< hip-knee-ankle angle (HKA) ≤ 3°), and 25 MM patients with varus alignment (HKA > 3°). Plantar pressure was measured…

Anterior cruciate ligament reconstruction simultaneously with trans-tibial pull-out fixation of avulsion fracture of medial and lateral meniscus posterior roots: a case report and eight-year follow-up.

We report a rare case of anterior cruciate ligament (ACL) tear combined with acute avulsion fracture of the medial and lateral meniscal posterior roots in a 23-year-old male patient. The management strategy was focused on how to do arthroscopic trans-tibial pull-out fixation of the avulsed fracture fragment and ACL reconstruction simultaneously, avoiding the possibility of the tibia tunnel interference during drilling. At the 8-year postoperative follow-up, the patient demonstrated excellent knee function and returned to non-competitive football activities, and manual knee laxity tests were ne
