Giant Mesencephalic Dilated Virchow Robin Spaces Causing Obstructive Hydrocephalus Treated by Endoscopic Third Ventriculostomy.

BACKGROUND: Virchow Robin spaces (VRSs) are perivascular spaces that act as drainage pathways for interstitial fluid. Dilatation of VRSs is visible on magnetic resonance images in asymptomatic individuals. However, giant dilatation of VRSs (dVRSs) is very rare. Such giant dVRSs may produce a pressure effect on surrounding structures and can be confused with more sinister conditions such as cystic neoplasm. CASE DESCRIPTION: We describe a 30-year-old man who presented with complaints of headache, poor attention and concentration, forgetfulness, polyuria, urinary incontinence for past 3 years a

Familial perimesencephalic subarachnoid hemorrhage: two case reports.

INTRODUCTION: Non-aneurysmal spontaneous subarachnoid hemorrhage is characterized by an accumulation of a limited amount of subarachnoid hemorrhage, predominantly around the midbrain, and a lack of blood in the brain parenchyma or ventricular system. It represents 5% of all spontaneous subarachnoid hemorrhage cases. In spite of extensive investigation, understanding of the mechanisms leading to perimesencephalic non-aneurysmal subarachnoid hemorrhage remains incompletely defined. A growing body of evidence has supported a familial predisposition for non-aneurysmal spontaneous subarachnoid hemo

The great imitator--still today! A case of meningovascular syphilis affecting the posterior circulation.

Cerebral ischemia due to meningovascular syphilis is rare and more frequently affects the anterior circulation than the posterior circulation. We describe clinical features and imaging studies of a 50-year-old patient with Parinaud syndrome and a syphilitic dorsal midbrain infarction. Brain magnetic resonance imaging indicated vasculitis of the posterior circulation. The diagnosis of meningovascular syphilis was established by serum and cerebrospinal fluid examinations. Although rare, because of the high impact on treatment, clinicians should always be aware of meningovascular syphilis in the

The cerebral representation of scratching-induced pleasantness.

Itch is an unpleasant sensation with the desire to scratch. Although it is well known that scratching itchy skin is pleasurable, the cerebral mechanisms underlying this phenomenon are poorly understood. We hypothesized that the reward system is associated with scratching-induced pleasantness. To investigate this hypothesis, a functional magnetic resonance imaging study was performed in 16 healthy subjects. Pleasantness was evoked by scratching the wrists where itch stimuli were applied, while scratching the dorsal forearms, far from itch stimuli, did not evoke pleasantness. Interestingly, plea

Pupil-sparing complete third nerve palsy from cryptogenic midbrain stroke in an otherwise-healthy young adult with patent foramen ovale.

Although pupil-sparing in acute unilateral complete third nerve palsy is often a sign of ischemic nerve injury, it is not specific for injury outside of the midbrain. This report documents acute pupil-sparing complete third nerve palsy in an otherwise healthy young adult with patent foramen ovale and associated atrial dilatation who suffered cryptogenic focal midbrain stroke, presumably from a paradoxical embolism. The patent foramen ovale was surgically closed. Over the next several months neurological recovery was complete except for diplopia and relatively comitant hypotropia, which respond

Mesencephalic enlarged Virchow-Robin spaces in a 6-year-old boy: a case-based update.

BACKGROUND: Perivascular spaces or Virchow-Robin spaces are normal anatomical structures within the brain, typically less than 5 mm in diameter. Rarely, they can reach much larger sizes and adopt bizarre single or multi-cystic configurations, as was the case in the patient that we report on. When there is such markedly gross dilatation, the phenomenon is known as Giant or tumefactive perivascular space enlargement. CASE REPORT: We report a 6-year-old boy with a multi-cystic mesencephalic lesions presenting with obstructive hydrocephalus secondary to obstruction of the aqueduct of Sylvius due

Radiolabeling of morphine with (131)i and its biodistribution in rats.

This study was conducted to determine the possible radiopharmaceutical potential of morphin labeled with (131)I. Morphine was extracted from dry capsules of the opium poppy (Papaver somniferum L.), purified by high-performance liquid chromatography, and characterized with nuclear magnetic resonance and infrared spectroscopy. The purified compound was labeled with (131)I. Male Albino Wistar rats (18) were used for receptor blockage and unblockage biodistribution studies. Tissue distribution studies showed that radiolabeled morphine had higher uptake in lung, liver, small intestines, large intes

[Convergence nystagmus and vertical gaze palsy of vascular origin].

A case of convergence-retraction nystagmus with upward vertical gaze paralysis and skew deviation (right hypotropia), without any other neurological signs, is reported. The probably vascular lesion was located at the mesodiencephalic junction, lying between the right border of the posterior commissure, the right interstitial nucleus of Cajal and the periaqueductal grey matter, accounting for the three ocular motor signs. The particular interest of this case is due to the relative smallness of the lesion.

Microsurgical approaches to the perimesencephalic cisterns and related segments of the posterior cerebral artery: comparison using a novel application of image guidance.

OBJECTIVE: To describe the exposure obtained through six approaches to the perimesencephalic cisterns with an emphasis on exposure of the posterior cerebral artery and its branches. METHODS: Dissections in 12 hemispheres exposed the crural, ambient, and quadrigeminal cisterns and related segments of the posterior cerebral artery. A Stealth Image Guidance workstation (Medtronic Surgical Navigation Technologies, Louisville, CO) was used to compare the approaches. RESULTS: The transsylvian approach exposed the interpeduncular and crural cisterns. The subtemporal approach exposed the interpedunc

Isolated relative afferent pupillary defect secondary to contralateral midbrain compression.

BACKGROUND: Relative afferent pupillary defects are typically related to ipsilateral lesions within the anterior visual pathways. OBJECTIVE: To describe a patient who had a workup for headache and was found to have an isolated left relative afferent pupillary defect without any other neurological findings. DESIGN: We review the neuroanatomy of the pupillary light reflex pathway and emphasize the nasotemporal bias of decussating fiber projections, which accounts for the relative afferent pupillary defect contralateral to the described lesion. Midbrain venous angioma with obstructive hydrocephalus.

A rare case of a mid brain venous angioma with obstructive hydrocephalus is described. A dilated draining vein from the lesion in the aqueduct as the cause of the hydrocephalus is highlighted, and interesting features of the pathology of venous angiomas and associated cavernous hemangioma are described. The management of this interesting condition is discussed.

An MRI study of midbrain morphology in patients with schizophrenia: relationship to psychosis, neuroleptics, and cerebellar neural circuitry.

BACKGROUND: The midbrain contains the perikarya of all the dopamine neurons in the human brain. Although other neurochemicals may well be involved, dopamine dysregulation is central in the pathophysiology of psychosis. Despite this, few imaging studies have evaluated the morphology of the midbrain. METHODS: Using high-resolution magnetic resonance imaging, morphology of three posterior fossa and brain stem structures were measured: midbrain, pons, and medulla. The patient sample consisted of 50 men with schizophrenia, matched by gender and age to 50 healthy control subjects. RESULTS: Patient

The putative role of extra-synaptic mesolimbic dopamine in the neurobiology of nicotine dependence.

A majority of habitual tobacco smokers find it very difficult to quit the habit because they become addicted to the nicotine present in tobacco smoke. Nicotine, like other psychostimulant drugs of abuse, increases dopamine release in the principal terminal field of the mesolimbic system, the nucleus accumbens, and there is evidence that this mediates the 'rewarding' properties of the drug, which reinforce its self-administration. This review focuses on the working hypothesis that addiction to nicotine, and other psychostimulant drugs, depends upon their ability to evoke a sustained increase in

Hyperthermia in brain hemorrhage.

Hemorrhage in the midbrain and/or pons in patients is often associated with increased metabolism, resulting in hyperthermia. We have recently reported that hyperthermia develops in anesthetized rats following prepontine knife-cuts or procaine microinjections into the midbrain or upper pontine region. It was concluded that the hyperthermia in the animals was caused by the removal of a tonic inhibitor mechanism of heat production that exists in the lower midbrain. The present paper proposes a new hypothesis that the hyperthermia in patients with brainstem hemorrhage is caused by disinhibition of

Hypothalamic grafts induce the recovery of lordosis in female hamsters with lesions of the ventromedial hypothalamus.

Previous studies have documented the ability of neural grafts to stimulate the recovery of lordosis from neurochemical deficits. However, it was unclear if grafts also could reverse deficits in lordosis caused by lesions at critical points in the neural circuit controlling this response. To address this question, female hamsters were subjected to unilateral lesions of the ventromedial hypothalamus (VMN), a structure well known for its mediation of hormonal effects on lordosis. The effects of these lesions were described by noting the ability of manual stimulation of one flank to reinstate a de

On the distribution of cholecystokinin B receptors in monkey brain.

In view of recent evidence for a role for the B subtype of cholecystokinin (CCKB) receptor in panic and anxiety, the distribution of CCKB receptors in the forebrain of a Rhesus macaca monkey was examined by receptor autoradiography employing [125I]D-Tyr25(Nleu28,31)-CCK25-33S. CCKB receptors were widely and topographically distributed in cortex. Other structures with notable labelling included the basal ganglia, presubiculum, amygdala, mamillary bodies, cerebellar cortex and pineal gland. The distribution of CCKB receptors further supports roles for this peptide in behavioural processes.

Auditory input to the pedunculopontine nucleus: I. Evoked potentials.

The pedunculopontine nucleus (PPN) has been implicated in sleep-wake control, arousal responses, and motor functions. The PPN also has been implicated in the generation of the P1 middle-latency auditory-evoked potential. The present study was undertaken to determine the topographical distribution, threshold, and response properties of depth-recorded potentials following auditory click stimulation. Experiments were conducted in both decerebrate cat and rat, with a view towards determining the presence of P1-like middle-latency auditory-evoked potentials in the midbrain of both species. These re

Cells of origin of the spinoparabrachial fibers in the rat: a study with fast blue and WGA-HRP.

The internal lateral (IL) subnucleus of the parabrachial nucleus (PB), which is one of the seven lateral subnuclei of the PB, receives information from the spinal cord. The IL subnucleus perhaps relays nociceptive signals to the intralaminar nuclei of the thalamus, apparently being implicated in the motivational-affective component of pain reaction. However, cells of origin of spinal fibers to the IL subnucleus have not been investigated sufficiently. We intended to clarify these cells by injection of fast blue or wheat germ agglutinin-conjugated horseradish peroxidase into the IL subnucleus a

[Positron-emission tomographic study of the dopaminergic system in a case of secondary unilateral tremor after mesencephalic hematoma].

A 30 year-old woman developed a postural and rest tremor of the left hand following a right peduncular post-traumatic hematoma. Two years later, positron emission tomography showed a marked decrease in [18F] fluorodopa uptake contrasting with a normal [76Br] bromolisuride uptake in the right striatum. This suggests that: 1) chronic unilateral dopaminergic striatal denervation may occur without persistent D2 dopaminergic receptor upregulation in humans; and 2) symptomatic mesencephalic tremor may be, at least in part, related to dopaminergic striatal denervation.

Female sexual behaviors in male rats with dorsal raphe nucleus lesions: treatment with p-chlorophenylalanine.

The effects of the serotonin-synthesis inhibitor, p-chlorophenylalanine (PCPA) on female sexual behaviors were examined in male rats with or without lesions (DRL) of the dorsal raphe nucleus, which contains a large number of serotonergic cell bodies. Estrogen-primed castrated males without brain surgery (control) showed extremely low levels of lordosis compared with females. On the other hand, DRL males displayed lordosis response more frequently than control males, but the lordosis quotient (LQ) in this group was lower than that in females. As well as DRL males, all PCPA-treated males showed
