Positive correlation between blood alcohol level and ISS in road trauma.

To assess the relationship between blood alcohol concentration (BAC) and injury severity in an unselected population of road accident victims, case notes of 820 consecutive road accident victims presenting to hospital in a 12 month period were reviewed retrospectively. Five hundred and thirty-five of these were eligible for BAC analysis and results, obtained from the Victoria Police and the Road Safety and Traffic Bureau were available in 429 cases. These were expressed as mg/100 ml (per cent). Injury severity was quantified using the Injury Severity Score (ISS). BAC was compared with injury s

Orthopaedic injuries in children associated with the use of off-road vehicles.

Two hundred and thirty-three children who had musculoskeletal injuries related to the use of off-road vehicles were admitted to acute-care hospitals in the two largest urban centers in Manitoba between April 1979 and August 1986. There were 190 boys and forty-three girls, and the ages ranged from two to seventeen years. Ninety-three accidents involved the use of a minibike or dirtbike; seventy-two, a snowmobile; fifty-nine, a three-wheeled all-terrain vehicle; and nine, a four-wheeled all-terrain vehicle. The injuries in 73 per cent of the children occurred in a rural setting. The use of alcoh

A Case of Displaced Cricoid Cartilage Fracture Successfully Treated With Non-Surgical Intervention.

Laryngeal trauma, particularly cricoid fractures, is rare but potentially life-threatening and often requires invasive airway management. However, factors contributing to successful conservative treatment remain unclear. We report the case of a 40-year-old woman diagnosed with a displaced cricoid cartilage fracture following a motorcycle accident. Despite the risk of upper airway obstruction due to a massive hematoma observed on fiberoptic laryngoscopy and a displaced cricoid cartilage fracture, the patient was successfully managed conservatively without tracheotomy or endotracheal intubation

Delayed Presentation of Subclavian Artery Pseudoaneurysm Following Blunt Thoracic Trauma: A Case Report.

INTRODUCTION: Subclavian artery pseudoaneurysms are a rare complication of blunt thoracic trauma with high mortality and incidence of long-term disability. CASE REPORT: We describe a 49-year-old female who suffered a midshaft clavicle fracture after a motorcycle collision who presented five weeks later with right arm weakness, paresthesias, and persistent clavicle pain and swelling. She was diagnosed with a subclavian artery pseudoaneurysm on point-of-care ultrasound performed in the emergency department, which was confirmed with computed tomography angiography. She underwent endovascular ste

Pure Open Ankle Dislocation Leading to Isolated Lateral Ligament Complex Tear: A Rare Case.

Pure open ankle dislocation is a rare orthopedic emergency characterized by the absence of associated bony lesions, necessitating prompt and immediate management to prevent disastrous complications. This article details a distinctive case of open pure ankle dislocation in an 18-year-old female following a motorcycle accident, resulting in a 6-cm wound and a dislocated left ankle with a concurrent tear of the anterior talofibular ligament and calcaneofibular ligament. Immediate reduction under sedation was performed, followed by intensive debridement and ligament repair in the operating room. P

Surgical treatment for acute longitudinal radioulnar dissociation with humeral capitellar fracture: A case report.

BACKGROUND: The forearm connects the wrist and elbow joints, and its stability depends on the two articulations, the proximal and distal radioulnar joints, and the interosseous membrane complex. The aim of this case report is to present an acute case of longitudinal radioulnar dissociation with humeral capitellum fracture. CASE PRESENTATION: A 48-year-old man suffered longitudinal radioulnar dissociation with humeral capitellar fracture following a motorcycle accident. Surgical treatment consisted of open reduction with an iliac bone graft using a buttress posterolateral plate for capitellar

Delayed diagnosis of proximal ureter injury after a blunt abdominal trauma in Korea: a case report.

Traumatic ureteral injuries account for fewer than 1% of traumatic urologic injuries, and those caused by blunt trauma are even rarer than those caused by penetrating trauma. The symptoms associated with ureteral injury may be subtle, with or without hematuria, making it difficult to diagnose. We report the case of a 31-year-old man with a delayed diagnosis of proximal ureter injury after abdominal blunt trauma sustained in a motorcycle traffic accident. The patient underwent emergency laparotomy on admission for liver injury, mesenteric injury, and resultant hemoperitoneum. On postoperative d

Right-sided luxation of the heart caused by blunt traumatic pericardial rupture.

Cardiac luxation is a rare condition in cases of blunt thoracic trauma, yet it is quite fatal. We present a case of a 28-year-old man, admitted to the emergency room after a motorcycle accident in a hemodynamically unstable condition and radiographic presentation of multiple rib fractures, bilateral pneumothorax, pneumomediastinum, and significant dislocation of the heart to the right. After performing emergency bilateral tube thoracostomy and achieving hemodynamic stability, a CT scan was performed and the patient was diagnosed with pericardial rupture with right-sided luxation of the heart.

Hybrid approach to complex vascular injury secondary to blast induced scapulothoracic dissociation.

Scapulothoracic dissociation is a rare but devastating injury complex involving high velocity blunt trauma to the osseous, muscular, neurologic, and vascular structures of the shoulder girdle. Often seen following a motor vehicle or motorcycle accident, this injury complex presents with vascular trauma in over 80% of cases. We present a unique case of scapulothoracic dissociation secondary to a self-inflicted shotgun wound to the shoulder, not previously reported in the literature. The patient presented in hemorrhagic shock, with an open wound to the chest, and a flaccid, pulseless left upper

The fat embolism syndrome as a cause of paraplegia.

The fat embolism syndrome is a well-known complication in trauma patients. We describe a rare case of traumatic fat embolism that leads to paraplegia. A 19-year-old male motorcycle accident victim was presented to our hospital. After stabilization and trauma survey, he was diagnosed with bilateral femur fractures, a spleen laceration and a tear in the inferior vena cava, for which damage control surgery was performed. Post-operatively, the patient became paraplegic and developed a fluctuating consciousness, respiratory distress and petechiae. Fat embolism syndrome was considered as the most pl

[An example of excellent functional adaptation in a patient with a four-digit amputation: case report].

We present the case of an 84-year-old man who shortly after the 2nd World War lost 4 digits (including the thumb) of his left hand due to an explosion. In spite of severe mutilation he completed school education, passed the secondary school leaving examination, and was employed in the mechanical industry. He perfectly adapted to his disability: he performed tasks in mechanical workshops and achieved car as well as motorcycle driving licences. He had been employed full time until retirement. Clinical examination of the affected hand revealed total grip strength of 10 kG (31% of the other hand),

Management of large segmental tibial defects using locking IM nail and absorbable mesh.

Tibia accounts for the majority of open fractures with long segmental defects. The management of such defects imposes several challenges due to their associations with extensive soft tissue damage and high infection risk. In this report, we describe a new technique for the reconstruction of tibia in a young patient with 10 cm defect. The patient had a history of Gustilo Anderson type IIIB open fracture following a motorcycle accident. He was initially managed with multiple debridements and application of an external fixator. The defect was treated with the insertion of an antibiotic-coated int

Fracture of the shoulder girdle in multiply injured patients - an imperative for a high level of suspicion for associated neurovascular injuries.

BACKGROUND: The combination of a bony injury to the shoulder girdle and damage to the brachial plexus and the subclavian vessels is a rare finding. The cases of this combined injury pattern described in the literature are most notably reported in multiply-injured patients after high velocity trauma. FINDINGS: Three cases were admitted to our hospital after motorcycle accidents resulting in a combination of severe bony injuries to the shoulder girdle, to the subclavian artery and a lesion to the brachial plexus. Based on these three clinical cases the patterns of injury, as well as primary and

Irreducible metatarsophalangeal joint dislocation of the lesser toes: a case report.

Irreducible metatarsophalangeal joint dislocation of the lesser toes is a rare injury. We present a 37-year-old man who was injured in a motorcycle accident and dislocated the first to third metatarsophalangeal joints and fractured the fourth metatarsal head. The left first metatarsophalangeal joint was reduced successfully through the closed method, but multiple attempts at closed reduction under local anesthesia failed to reduce the dislocated second and third metatarsophalangeal joints. We performed a dorsal incision between the second and third metatarsals, and the metatarsal heads were fo

Long-term vaginal sequelae secondary to pediatric pelvic fracture.

BACKGROUND: Pediatric pelvic fractures often appear radiographically benign despite significant injury to viscera. Vaginal injuries are rare, and therefore require a high index of suspicion. CASE: A 26-year-old female patient who sustained a pelvic fracture in a motorcycle accident at the age of 10 presented with vaginal stenosis precluding sexual intercourse. Patient menstruated through a fistula in the distal vaginal vault. Radiographic and dye studies showed almost complete vaginal obstruction with heterotopic ossification. An extensive 30-year review of the literature was conducted lookin

[Management of maxillofacial injuries in an emergency unit: our experience].

Maxillofacial injuries are relatively frequent. These may be complicated by cranioencephalic injuries or large facial wounds. We report our experience with the initial management of these lesions in an emergency unit setting. In 2007, we observed 105 patients with maxillofacial injuries. Maxillofacial injuries were associated with cranio-encephalic injuries in 69 patients (65.7%) and with polytrauma in 31(29.5%), while in 5 cases (4.8%) they were isolated. The main causes of trauma were motorcycle accidents (60%). All patients were treated in accordance with the Advanced Trauma Life Support gu

Unilateral reduction of the cervical response in median SEP due to a vertebral fracture, unrecognized in plain films.

We report on a patient presenting with hypaesthesia in first, second and third finger of the right hand following a motorcycle accident. Conventional X-ray showed only a mild dislocation in C6/C7 segment. Cervical MRI in order to prove a root avulsion, was reported to be normal. Somatosensory evoked potentials (SEP) revealed a reduced amplitude of the cervical response on right median nerve stimulation. Needle-EMG showed a mild reduced recruitment pattern in triceps brachii muscle compatible with an anterior root lesion. Reviewing MRI, a signal loss in the course of C7 root was suspicious for
