Delipidified cell components (DCC) of Mycobacterium leprae obtained as an insoluble material consist of several proteins. This preparation, DCC, has ability to differentially bind to sera from lepromatous leprosy patients and antibodies to this complex get reduced as patients improve under chemotherapy. Non-tuberculous mycobacterial infections are usually found in the respiratory tract: infection confined to the genito-urinary system is rare. We report two patients with isolated genito-urinary infections, one due to Mycobacterium chelonae and the other to Mycobacterium avium-intracellulare.

A 29-year-old woman who had successfully used extended-wear soft contact lens for 7 years was found to have a corneal ulcer due to Mycobacterium chelonei. Unique to this presentation was the lack of a history of severe corneal trauma, prior surgery or immunocompromised tissue. Despite in-vitro resistance of the organism to amikacin sulfate, topical therapy with this antibiotic led to a complete cure without the need for surgical intervention.

A 34-year-old woman had wide-spread panniculitis due to a disseminated infection with Mycobacteria avium-intracellulare. The patient had previously received treatment with high dosages of corticosteroids. Suppurative lesions teeming with acid-fast bacilli and without formation of granulomas were found in many organs, including the skin, mediastinum, spleen, liver, and gastrointestinal tract. Both the appearance of disseminated M avium-intracellular infection resembling panniculitis and the involvement of mediastinum have not previously been reported, to our knowledge.

A patient on systemic steroid therapy developed a cutaneous papule while histologically resembled lepromatous leprosy. Cultures of this lesion grew Mycobacterium avium. Since there was no evidence of disseminated infection the lesion was excised and the patient continues to do well.

Skin tests were conducted on 3 lepromatous, 3 dimorphous and 6 tuberculoid leprosy patients and 3 others not suffering from leprosy with lepromin, purified protein derivative from M. leprae of armadillo origin and tuberculin. Results show that a delayed hypersensitivity reaction could be produced with PPD in 72 hours on all Mitsuda positive cases, with one anomalous exception, without cross reaction to tuberculin. The results were promising from the point of view of substituting lepromin with PPD in usual tests.
