Carpal tunnel syndrome (CTS) is a common neuromuscular condition and a major cause of work-related disability. As healthcare in the United States transitions toward a value-based system from fee-for-service, quality measures assume importance in the evaluation of care provided. This report from the American Association of Neuromuscular & Electrodiagnostic Medicine Quality Improvement Committee provides an introduction to quality measures and outlines a quality measurement set for the electrodiagnosis of CTS. The measures attempt to standardize technical requirements for electrodiagnostic (EDX)

Guillain-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy, and pathophysiologically classified into acute inflammatory demyelinating polyneuropathy (AIDP), acute motor axonal neuropathy (AMAN), and acute motor and sensory axonal neuropathy (AMSAN). The main pathophysiological mechanism is complement-mediated nerve injury caused by antibody-antigen interaction in the peripheral nerves. Antiglycolipid antibodies are most pathogenic factors in the development of GBS, but not found in 40% of patients with GBS. One of the principal target regions in GBS is the node of Ranvier

Postpartum neuropathies of the lower extremities are possible complications after delivery, with an estimated incidence of 1%. There are a few case reports about sciatic neuropathy after a caesarean section. However, a sciatic neuropathy after normal vaginal delivery has not been reported till date. This is a case report of a 30-year-old woman who visited our department with a chief complaint of left lower extremity weakness after normal vaginal delivery (at gestational age of 38 + 5 weeks) one month prior. The delivery took place under epidural anesthesia for about 6 h, with no dystocia or pe

Hereditary neuropathy with liability to pressure palsies (HNPP) is characterized by recurrent sensory and motor neuropathy in individual nerves starting in adolescence or young adulthood, focal conduction abnormalities at entrapment sites on nerve conduction studies, and sausage-like swellings (tomacula) of the myelin sheaths by nerve biopsy. It is characterized genetically by the deletion of the chromosome 17p11.2-p12 region including the peripheral myelin protein-22 gene in the overwhelming majority of cases. HNPP may be frequently underdiagnosed or misdiagnosed owing to the heterogeneity of

Carpal tunnel syndrome (CTS) is a clinical syndrome diagnosis. There is no universal reference test for diagnosing CTS. In case of a clinical presentation consistent with the typical characteristics of CTS, the diagnosis can be made without additional examinations. In that case, the physician may immediately proceed with a discussion of treatment options with the patient, using the CTS consultation card. Use of clinical provocation tests, questionnaires and hand diagrams is not advised due to their limited diagnostic value. Additional examinations are indicated when a patient does not have all

Different subtypes of Guillain Barré Syndromes (GBSs) are defined by their electrophysiological characteristics, acute inflammatory demyelinating neuropathy (AIDP), and acute motor/motor-sensory axonal forms (AMAN/AMSAN) with either reversible nerve conduction failure (RCF) or axonal degeneration. Our aim was to describe initial clinical and electrophysiological characteristics of axonal forms of GBS in a pediatric population and their short- and long-term evolution. Electroneuromyogram (ENMG) results were collected at diagnosis and at two months of evolution and interpreted using the recently

Different neurophysiological methods such as evoked potentials (EP), testing of the autonomic nervous system (ANS) or polysomnography have the potential to detect clinically silent lesions or to confirm the existence of an association between a clinical symptom and multiple sclerosis (MS); previously undetected by MRI. Therefore, in the most recent MRI criteria for the diagnosis of MS (MAGNIMS consensus guidelines), neurophysiological confirmation of optic nerve dysfunction (slowed conduction on visual EP), support dissemination in space and, in patients without concurrent visual symptoms, dis

The severity of Guillain-Barré Syndrome (GBS) varies among subtypes, which can be mainly Acute Inflammatory Demyelinating Polyneuropathy (AIDP), Acute Motor Axonal Neuropathy (AMAN), Acute Motor Sensory Axonal Neuropathy (AMSAN) and Miller-Fisher Syndrome (MF). In this study, we use a real dataset that contains clinical, serological, and nerve conduction tests data obtained from 129 GBS patients. We apply C4.5 decision tree, SVM (Support Vector Machines) using a Gaussian kernel, and kNN (k Nearest Neighbour) to predict four GBS subtypes. Accuracies were calculated and averaged across 30 10-fol

Electrodiagnosis of subclinical diabetic neuropathies by nerve conduction studies remains challenging. The question arises about which nerves should be tested and what the best electrodiagnostic protocol to make an early diagnosis of diabetic neuropathies would be. On the basis of our findings and other evidence, which highlighted the remarkable prevalence of electrophysiological abnormalities in nerve conduction studies of the upper limbs, often in the presence of normal lower limb conduction parameters, we suggest that both ulnar and median nerves, in their motor and sensitive component, sho

We conducted a study to determine the change in nerve conduction testing after steroid injection in patients with carpal tunnel syndrome (CTS). One hundred forty-five patients with suspected CTS were targeted for this study. Twenty-seven patients underwent testing before and after injection. Repeat studies were performed 4 to 6 weeks after injection. All data from the electrodiagnostic studies were entered into a database and used for comparison. Before injection, mean (SD) distal motor latency (DML) was 5.01 (0.9) ms, and mean (SD) peak sensory latency (PSL) was 5.01 (0.88) ms. After injectio

Martin-Gruber anastomosis (MGA) is the most common nerve anastomosis in the upper extremities and it crosses from the median nerve to the ulnar nerve. Proximal MGA is an under recognized anastomosis between the ulnar and median nerves at or above the elbow and should not be missed during nerve conduction studies. We presented two patients with ulnar neuropathy mimicking findings including numbness and tingling of the 4th and 5th digits and mild weakness of intrinsic hand muscles. However, both cases had an apparently remarkable conduction block between the below- and above-elbow sites that was

In some anti-ganglioside antibody-mediated neuropathies, human and experimental data suggest a common pathogenic mechanism of dysfunction/disruption at the node of Ranvier resulting in a pathophysiologic continuum from transitory nerve conduction failure to axonal degeneration. The traditional classification of polyneuropathies into demyelinating or axonal may generate some confusion in the electrophysiological diagnosis of Guillain-Barré syndrome subtypes associated with anti-ganglioside antibodies. The axonal forms show, besides axonal degeneration, promptly reversible nerve conduction failu
