Cerebellar theta burst stimulation in stroke patients with ataxia.

Evidence for effective improvement of the symptoms of cerebellar stroke is still limited. Here, we investigated the effects of repetitive transcranial magnetic stimulation (rTMS) applied over the injured cerebellar hemisphere in six patients with posterior circulation stroke. We applied a two-week course of cerebellar intermittent theta burst stimulation (iTBS). Before and after the iTBS treatment, paired-pulse TMS methods were used to explore: i) the functional connectivity between the cerebellar hemisphere and the contralateral primary motor cortex (M1), by means of the cerebellar brain inhi

Spectral and level effects in auditory signal enhancement.

Auditory enhancement refers generally to the increased perceptual salience of a spectral region when that region is preceded by its spectral complement, e.g., reinserting a missing component in a harmonic complex makes that component "pop out." One manifestation of enhancement is the increased detectability of a signal in certain spectro-temporal configurations. In the present experiments, detection thresholds were measured for a 2-kHz signal that was masked by an inharmonic complex with a spectral notch centered at 2-kHz. When the masker was preceded by a precursor/adaptor with a spectral gap

Modulation of the disturbed motor network in dystonia by multisession suppression of premotor cortex.

Daily sessions of therapeutic transcranial brain stimulation are thought to prolong or amplify the effect of a single intervention. Here we show in patients with focal hand dystonia that additional, new effects build up progressively over time, making it difficult to predict the effect of long term interventions from shorter treatment sessions. In a sham-controlled study, real or sham continuous theta burst stimulation (cTBS) was given once daily for five consecutive days to dorsolateral premotor cortex (PMd). Five days of real, but not sham, premotor cTBS improved intracortical inhibition in

Cortical effects of repetitive finger flexion- vs. extension-resisted tracking movements: a TMS study.

While the cortical effects of repetitive motor activity are generally believed to be task specific, the task parameters that modulate these effects are incompletely understood. Since there are differences in the neural control of flexor vs. extensor muscles, the type of muscles involved in the motor task of interest may be one important parameter. In addition, the role each muscle plays in the task, such as whether or not it is the prime mover, is another potentially important task parameter. In the present study, use-dependent cortical plasticity was examined in healthy volunteers performing

Selective loss of AMPA receptor subunits at inhibitory neuron synapses in the cerebellum of the ataxic stargazer mouse.

AMPA receptor subunits (GluA1-4) are trafficked to membrane synaptic sites by transmembrane AMPA receptor regulatory proteins (TARPs). In the stargazer mutant mouse, expression of TARP-γ2 (stargazin) is severely reduced, resulting in cerebellar ataxia. Stargazer granule cells (GCs) have a complete loss of functional AMPARs, as γ2 is their main TARP; hence mossy fiber (MF)-GC synapses are silent. The aim of the current study was to investigate how the stargazin deficit affects expression levels of AMPAR subunits at output synapses from GC parallel fibers (PF) onto inhibitory neurons in the mole

Difference in intracortical inhibition of the motor cortex between cortical myoclonus and focal hand dystonia.

OBJECTIVE: The short interval intracortical inhibition (SICI) of the motor cortex (M1) is reduced in both cortical myoclonus and focal hand dystonia. This reduction has been attributed to the dysfunction of GABAergic system within the motor cortex. However, the precise mechanisms underlying the reduction may not be entirely identical in these two disorders, being due to primary pathological involvement in M1 or secondary to functional changes outside M1. The aim of this study was to elucidate possible differences in intracortical inhibition between these two disorders. METHODS: Subjects were

Long-term plasticity of visually evoked potentials in humans is altered in major depression.

BACKGROUND: Long-term synaptic plasticity is a ubiquitous form of neuronal plasticity that regulates the strength of synaptic transmission in many brain areas. However, most data on long-term potentiation and long-term depression rely on research in animal brain slices. The role of synaptic plasticity in physiology and pathology of the functioning human brain remains obscure. Considering recent studies that provided evidence for a dysfunction of brain plasticity as the neurobiological basis of affective disorders, we assessed neural transmission and its plastic modulation in the visual pathway

Effect of subject task on contralateral suppression of click evoked otoacoustic emissions.

Contralateral suppression of click evoked otoacoustic emissions (CEOAEs) is widely used as a non-invasive measure of the activity of the (uncrossed) medial olivocochlear bundle (MOCB). There is evidence that the uMOCB receives descending input from the cortex, potentially mediating top-down control during higher order processing. This study investigated whether the contralateral suppression measure is affected by top-down influences during different tasks performed by the participants during recording. Suppression of CEOAEs evoked at 50 and 60dB SPL was measured under four different task condi

The effects of sleep deprivation on pain inhibition and spontaneous pain in women.

Impaired central pain modulation is implicated in the pathophysiology of chronic pain. In this controlled experiment, we evaluated whether partial sleep loss altered endogenous pain inhibition and reports of spontaneous pain. Thirty-two healthy females were studied polysomnographically for 7 nights. On Nights 1-2 (Baseline), subjects slept undisturbed for 8 hours. After Night 2, subjects were randomized to Control (N = 12), Forced Awakening (FA, N = 10), or Restricted Sleep Opportunity (RSO, N = 10) conditions. Controls continued to sleep undisturbed. FA underwent 8 forced awakenings (one per

Phasic spike-timing-dependent plasticity of human motor cortex during walking.

Research into mechanisms for inducing long-lasting increases in human motor system excitability holds great promise for two prime reasons. First, the research may provide a better understanding of the role of cortex in human movement. Second, an induced increase in the excitability of the motor system during walking re-training following neural injury may emerge as a valuable therapeutic adjunct. The present study applied paired associative stimulation (PAS) to healthy humans during walking. PAS is based on the principles of spike-timing-dependent plasticity. Our hypothesis predicted that PAS

Intraoperative monitoring of motor symptoms using surface electromyography during stereotactic surgery for movement disorders.

The authors present practical evidence for the usefulness of intraoperative monitoring with surface electromyograms (sEMGs) from the affected muscles to assist electrode implantation and lesioning in patients with movement disorders. In 22 consecutive patients with various movement disorders, sEMGs were monitored in selected muscles during stereotactic surgery that involved either lesioning or electrode implantation. The electromyograms related to major motor symptoms such as tremor, rigidity, myoclonus, dystonia, and chorea were monitored and characterized on-line by both amplitude and freque

Early maternal deprivation retards neurodevelopment in Wistar rats.

A single 24 h period of maternal deprivation (MD) in rats has been shown to induce, in adulthood, a number of abnormalities in brain and behaviour that also occur in patients with schizophrenia. However, the short-term behavioural effects of MD have not been studied in detail. Since patients with schizophrenia are characterized by a retardation of normal development, we aimed in the present study to investigate the development of control rats and rats that were exposed to MD on postnatal day 9. Compared to control animals, MD rats showed (1) a reduction in body weight, (2) an increased in reve

Clinical findings in sporadic Creutzfeldt-Jakob disease correlate with thalamic pathology.

The pathogenesis underlying the typical findings in Creutzfeldt-Jakob disease (CJD) such as periodic EEG changes or myoclonus is not fully understood. The thalamus possesses a high density of inhibitory neurones and serves as a crucial pacemaker of rhythmic EEG activity. As inhibitory neurones expressing parvalbumin (PV) are reduced in the cerebral cortex and hippocampus in sporadic CJD (sCJD), we studied the distribution and number of PV-immunoreactive neurones in sCJD thalami in order to determine whether damage to them could account for certain clinical findings. Immuno histochemical analys

Altered cortical excitability in obsessive-compulsive disorder.

OBJECTIVE: To assess cortical inhibitory and excitatory mechanisms in obsessive-compulsive disorder (OCD). BACKGROUND: Transcranial magnetic stimulation (TMS) studies have found decreased neuronal inhibition and a reduced cortical silent period in the primary motor area in Tourette's syndrome, focal dystonia, and other disorders believed to involve dysfunction of subcortical structures, including the basal ganglia. Dysfunction of the basal ganglia and linked regions also has been implicated in OCD, which has significant clinical and familial overlap with tic disorders. METHODS: We applied th

Are jaw and facial reflexes modulated during clinical or experimental orofacial pain?

A variety of jaw and facial reflexes can be evoked by orofacial mechanical or electrical stimuli. Because of its possible diagnostic utility in the management of pain and dysfunction of the masticatory system, the exteroceptive suppression that can be evoked in the masseter and temporalis muscles has been particularly investigated. A review of the different studies emphasizes the crucial importance of the area stimulated and the type of stimulation used to evoke the reflex. More recent studies have applied the necessary standardization of stimulus intensity, clenching levels, recording procedu

Presyncope during progressive hypovolaemia simulated by lower body negative pressure is not prevented by high-dose naloxone.

BACKGROUND: The haemodynamic response to progressive hypovolaemia, whether simulated by lower body negative pressure (LBNP) or head-up tilt, or induced by haemorrhage or haemodialysis, has a typical biphasic pattern: a first, sympathoexcitatory, phase of vasoconstriction, tachycardia, and stable blood pressure, and a second, sympathoinhibitory, phase of vasodilatation, bradycardia, and hypotension. The opioid system is involved in this response, since animal studies showed that opioid antagonism by naloxone can attenuate hypovolaemic hypotension. In humans, this finding could not be confirmed.

Ipsilateral cortico-cortical inhibition of the motor cortex in various neurological disorders.

We used a paired-pulse magnetic stimulation technique to study ipsilateral cortico-cortical inhibition of the motor cortex in 48 patients with various neurological disorders and in 20 normal volunteers. In the normal subjects, the first subthreshold conditioning stimulus suppressed responses to the second suprathreshold test stimulus at interstimulus intervals (ISIs) of 1-5 ms (inhibition at short intervals), and facilitated them at ISIs of 8-15 ms (facilitation at long intervals). Patients with motor neuron disease, except those in whom brain stimulation produced control responses that were g

Inhibitory effects of centrally and peripherally induced anosmia on mounting behavior in the female rat.

Adult ovariectomized female rats received a testosterone-propionate (TP)-filled silastic implant and were tested for mounting behavior seven days later. Exposure to estrous female urine but not to urine from ovariectomized females increased the number of TP-treated females that displayed mounting behavior. Olfactory bulb removal on its own and peripheral anosmia induced by intranasal ZnSO4 application impaired mounting behavior. These results are discussed with respect to the effects of anosmia on copulatory behavior in the male.

Conduction block in neuropathies with necrotizing vasculitis.

Thirty-two patients with a mononeuropathy multiplex associated with a systemic necrotizing vasculitis were studied. The main abnormality was a loss of motor and sensory axons confirmed by electrophysiological and histological methods. A conduction block was observed in five patients, but only one was at a usual site of compression. Based on previous pathological studies and the experimental data in human and animals, the mechanism of the block is proposed to be ischemic. It is suggested that a conduction block in one nerve in a neuropathy with two or more individual nerves affected and with el

Chronic cerebroventricular infusion of hypertonic sodium chloride in rats reduces hypothalamic sympatho-inhibition and elevates blood pressure.

To determine whether or not salt loading restricted to the brain would elevate blood pressure, chronically implanted osmotic minipumps were used to infuse hypertonic sodium chloride solutions made in artificial cerebrospinal fluid into a jugular vein intravenously or the third cerebral ventricle, intracerebroventricularly, for 11 days in awake rats. During intracerebroventricular infusion of hypertonic sodium chloride, tail-cuff systolic pressures began to rise on day 4 and were significantly elevated by day 9. In contrast, infusions of either artificial cerebrospinal fluid alone, intracerebro
