Short-segment pedicle stabilisation, transpedicular disc resection and autologous cancellous bone grafting is performed in trauma patients being transferred to our trauma centre with thoracolumbar fracture/dislocations. A follow-up examination of 88 patients, who were operated during the years of 1985-1992, took place after an average time of 5.6 years. Our results suggest that the earlier operative decompression and spine stabilisation takes place, the better is the recovery rate in patients with neurologic deficits. The highest neurologic recovery rates were found in patients operated within

Spontaneous cervical epidural hematoma (SCEDH) is a rare disease, but can cause severe neurologic impairment. We report a case of a 68-year-old female who presented with sudden onset, posterior neck pain, right shoulder pain, and progressive right hemiparesis mimicking stroke with no trauma history. Initial brain CT and diffusion MRI performed to rule out brain lesion did not show any positive findings. Laboratory examination presented only severe thrombocytopenia (45,000/mm(3)). Subsequent cervical MRI revealed a cervical epidural mass lesion. We confirmed that it was pure hematoma through C5

Most adolescent headaches are related either to an acute sensory systemic illness on an acute basis or to episodic tension in a more chronic recurrent pattern. A careful history and physical and neurologic examination of these adolescents with nonmigraine headache are necessary to discern the need for additional testing. Additional testing will determine if the headaches are symptomatic of a progressive neurologic disorder or a chronic recurrent process such as tension-type headache. Only after a correct diagnosis is made can a therapeutic regimen be applied or, in the uncommon occurrence of a…

Extracorporeal membrane oxygenation (ECMO) is associated with a high risk of neurologic injury in pediatric patients, and early detection of injury is often challenging because the clinical examination is frequently limited by illness, medication, and developmental factors. Quantitative electroencephalography (qEEG), derived from standard continuous electroencephalography (cEEG), offers a bedside, continuous, and objective approach to neuromonitoring. We present a case series of two pediatric patients supported on ECMO who underwent cEEG monitoring within 6 hours of cannulation. Quantitative e

INTRODUCTION: We present a case of a foramen magnum meningioma that highlights the importance of the neurologic exam when evaluating a patient with dysphagia. A 58-year-old woman presented with an 18-month history of progressive dysphagia, chronic cough and 30-pound weight loss. Prior gastroenterologic and laryngologic workup was unrevealing.
RESULTS: Her neurologic examination revealed an absent gag reflex, decreased sensation to light touch on bilateral distal extremities, hyperreflexia, and tandem gait instability. Repeat esophagogastroduodenoscopy was normal, whereas laryngoscopy and vide…

OBJECTIVE: This study was undertaken to investigate long-term neurodevelopmental outcome of children born after intrauterine laser coagulation for severe twin-twin transfusion syndrome.
STUDY DESIGN: One hundred sixty-seven surviving infants treated between June 1997 and September 1999 were investigated at a median age of 3 years and 2 months. All children underwent a detailed standardized physical and neurologic examination and a standardized developmental test (Griffiths' Developmental Test Scales and Snijders-Oomen Non-Verbal-Intelligence Test).
RESULTS: One hundred forty-five infants (86…

In order to study the long-term course after herpes simplex virus type 2 (HSV-2) meningitis and/or myeloradiculitis the records of 40 consecutive patients were studied. During the year following the acute phase, verified or suspected neurologic recurrences were noted in nearly half of the patients: 1 or more episodes of recurring meningitis were noted in 8 patients; new episodes of myelitis or radiculitis in 3; distinct attacks of headache in 4; and diffuse neurologic complaints impairing daily life in 3. Recurring mucocutaneous symptoms were observed in 16 patients. Eleven patients experience

Neurologic complications can occur during the course of brucellosis. In a 25-year-old patient admitted July 3, 1984 because of loss of weight (10 kg) brucellosis (Brucella melitensis) was diagnosed on the basis of splenomegaly, positive agglutination and positive blood and bone marrow cultures. Therapy with cotrimazol was performed for eight weeks. On July 31, 1984 the patient was readmitted because of weakness in both legs. Neurologic examination revealed slight weakness of the hip extensors and a marked weakness of the knee flectors and foot extensors without loss of sensation. The cerebrosp…

Functional neurologic disorder (FND), although neglected for much of the 20th century, is among the most common conditions encountered by neurologists across clinical settings. High prevalence rates and limited provider expertise in FND have created a considerable need to develop educational initiatives and practical suggestions to guide neurologists in training working with this population. To help avoid diagnostic errors, trainees should keep in mind that (1) marginally positive functional examination signs have low specificity; (2) FND can coexist with other neurologic comorbidities; and (3

PURPOSE: Symptoms of cauda equina syndrome due to ependymoma in the conus medullaris or filum terminale develop slowly. However, hemorrhagic change inside spinal tumors can induce acute neurologic decline. Here, we report a case of posttraumatic hemorrhage inside a filum terminale myxopapillary ependymoma presenting as acute neurologic decline, which had a positive prognosis after surgical resection.
METHODS: A 28-year-old man presented with buttock pain, sensory disturbance, and motor weakness of bilateral lower extremities after falling on ice during smelt fishing. Magnetic resonance imagin

We retrospectively analyzed the medical documentation of 194 children infected with Epstein-Barr virus. The diagnosis was based on clinical symptoms and the presence of the viral capsid antigen IgM antibody. Patients with severe neurologic complications also underwent neurologic examination, magnetic resonance imaging (MRI), and electroencephalography (EEG). There were 2 peaks in incidence of infection; the first one in young children aged 1 to 5 years represented 62.0% of cases. The second peak (24.6% of patients) occurred in teenagers. Febrile seizures were confirmed in 3.1% of affected chil…

BACKGROUND Neurological assessment is a critical component of prehospital emergency care, particularly in time-sensitive conditions including stroke and traumatic brain injury. Neurological documentation practices in this setting remain insufficiently characterized. This study aimed to evaluate self-reported completeness of neurological documentation among paramedics, then examine its associations with neurological training and professional experience. MATERIAL AND METHODS A cross-sectional study was conducted between June 2025 and March 2026 among 348 paramedics working in emergency medical s

OBJECTIVES: to explore the association between cerebral small-vessel diseases (CSVDs) and motor symptoms in Parkinson's disease (PD).
METHODS: 137 PD patients were recruited into the study. Detailed motor symptoms, including tremor, rigidity, bradykinesia, and axial impairment, were evaluated using Unified Parkinson's disease Rating Scale (UPDRS). Non-motor symptoms, including cognition, anxiety, and depression, were evaluated using Montreal Cognitive Assessment (MoCA), Hamilton anxiety scale (HAMA), and Hamilton depression scale (HAMD). Brain MRI was used to assess the subtypes of CSVDs, inc

Optokinetic nystagmus was recorded and measured in 101 subjects comprising six diagnostic categories: 1) normal, screened for otologic disease, 2) chronic unilateral labyrinthectomy, 3) unilateral Menière's disease, 4) neurologically confirmed focal brainstem lesion, 5) brainstem-cerebellar syndrome, and 6) focal unilateral supratentorial lesion. For the OKN test, each subject looked at a translucent screen onto which a field of parallel black and white bars was back-projected. The array of bars could be projected vertically or horizontally to allow for study of nystagmus beating right and lef

PURPOSE: The purpose of this study was to describe the phenotype (and corresponding genotype) of adult patients with late-onset Tay-Sachs disease, a clinical variant of the GM2-gangliosidoses.
METHODS: A comprehensive physical examination, including neurological assessments, was performed to establish the current disease pattern and severity. In addition, the patients' past medical histories were reviewed. The patients' alpha-subunit mutations (beta-Hexosaminidase A genotype) were determined and correlated with their corresponding clinical findings and disease course.
RESULTS: Twenty-one pat

We prospectively studied the evaluation of dementia in 107 unselected outpatients; 83 had so-called "irreversible" dementias, including 74 who had an Alzheimer-type dementia. Fifteen patients had potentially reversible dementias, of which hypothyroidism and drug toxicity were the commonest causes. Distinguishing features of reversible dementia were shorter duration, use of more prescription drugs, and less severe dementia. Almost half of the patients had other previously unrecognized treatable medical diseases. Most diagnoses were made from patient history and physical and mental status examin
