Anesthesia for microlaryngeal surgery: the case for subglottic jet ventilation.

Although the techniques for surgery on the endolarynx using suspension and the operating microscope have been fully developed, the safest, and least obtrusive anesthetic technique has yet to be manifested, as evidenced by more than 200 references to anesthesia for microlaryngoscopy in the world literature. This study reviews the physiology, physics, and problems of each anesthetic technique. In light of this review, animal and human studies are reported demonstrating the utility and safety of subglottic ventilation when provided with proper monitoring using an automatic ventilator. Upper limb muscle reflexes in real and virtual environments: Insights into sensorimotor adaptations.

INTRODUCTION: The mechanisms influencing neuromuscular adaptations in the upper limb within dynamic environments remain understudied, especially when exposed to altered visual and emotional conditions such as those simulated in virtual reality (VR). Here we utilize VR to manipulate visual feedback, inducing motion sickness and modulating sympathetic arousal, while assessing adaptations in sensorimotor integration in the upper extremity using electrically evoked and muscle stretch reflexes. METHODS: Eighteen healthy young adults experienced four experimental conditions while sustaining submaxi

Recovery Stages After Anterior Crucuiate Ligament Reconstruction.

Postoperative rehabilitation after anterior cruciate ligament (ACL) surgery is essential for restoring strength and function and promoting return to sport while minimizing risk of reinjury. The recovery process typically spans 9 to 12 months, delineated in a phased, criterion-based approach to monitor milestones, restore function and confidence, and promote return to sport. The early phase (weeks 0-6) focuses on reducing pain and swelling while restoring knee range of motion (ROM) and quadriceps strength (≥60% limb symmetry index [LSI]) with the use of modalities such as neuromuscular electric

Changes in Quadriceps Rate of Torque Development After Anterior Cruciate Ligament Reconstruction and Association to Single-Leg Hop Distance.

BACKGROUND: Quadriceps neuromuscular function is negatively affected after anterior cruciate ligament reconstruction (ACLR). The specific effect that the ACLR has on the quadriceps femoris rate of force production and its impact on functional recovery is unknown. HYPOTHESIS: The anterior cruciate ligament (ACL) limb would present persistent deficits in the rate of torque development (RTD), when compared with the non-ACL limb before ACLR until 9 months (9M) post-ACLR. STUDY DESIGN: Prospective cohort study. LEVEL OF EVIDENCE: Level 3. METHODS: Twenty-eight participants performed quadriceps

Postextubation Respiratory Muscle Recovery After Sugammadex or Neostigmine: A Randomized Controlled Ultrasonographic Trial in Laparoscopic Cholecystectomy.

BACKGROUND: Sugammadex minimizes the residual neuromuscular block risk. By enhancing the function of expiratory muscles, it improves the clearance of secretions and helps prevent lung collapse, leading to a reduction in early postoperative respiratory issues. The study aims to compare the efficacy of sugammadex in promoting the recovery of expiratory muscle power to that of neostigmine following extubation. STUDY OBJECTIVES: The main objective of this study was to evaluate the effectiveness of sugammadex and neostigmine as a reversal of neuromuscular block, assessed through ultrasonographic e

Postoperative Respiratory Depression and Other Postoperative Pulmonary Complications in Veterinary Practice: A Narrative Review.

Postoperative respiratory depression (PORD) and other postoperative pulmonary complications (PPCs) represent significant contributors to perioperative morbidity and mortality in companion animal practice. The Confidential Enquiry into Perioperative Small Animal Fatalities (CEPSAF) established that cardiovascular and respiratory causes account for approximately 74% and 72% of perioperative deaths in dogs and cats, respectively, with 47% of canine and 61% of feline anesthetic-related deaths occurring in the postoperative period. PORD arises from a convergence of opioid-induced respiratory depres

Latrodectism in Europe (1973-2025): A systematic review of clinical manifestations, management and outcomes.

BACKGROUND: Latrodectus tredecimguttatus, the Mediterranean black widow spider, accounts for most clinically significant spider bites in Europe. Although its toxidrome has long been recognized, contemporary evidence is fragmented, and the evolving clinical spectrum - including emerging cardiac complications - has not been systematically reviewed. METHODS: We performed a systematic review of PubMed/MEDLINE and Scopus (inception-September 2025) following PRISMA 2020 guidelines. Eligible studies included case reports, case series, and observational datasets reporting confirmed or presumed L. tre

Advances in Real-Time Electrochemical Monitoring of Neurotransmitter Dynamics in Drosophila.

Drosophila melanogaster is a genetically tractable model organism with a compact yet functionally rich nervous system, making it ideal for investigating neural mechanisms and modeling neurological diseases. However, its small brain size poses challenges for real-time measurement of neurotransmitter dynamics. Recent advances in electrochemical methodologies, particularly fast-scan cyclic voltammetry (FSCV) and amperometry with carbon fiber microelectrodes, enable high spatiotemporal resolution measurements in vivo. The integration of electrochemistry with optogenetic tools further enhances expe

Investigating Countermovement and Horizontal Jump Asymmetry in Female Football Players: Differences Across Age Categories.

Background: Lower limb asymmetry is a critical factor influencing performance and injury risk in football players. Despite the increasing popularity of women's football, limited research has examined how asymmetry varies across different age categories. This study aimed to investigate countermovement jump (CMJ) and horizontal jump (HJ) asymmetry in female football players across four age categories (U14, U16, U18, and +18). Methods: Seventy-six female football players from the same club participated in the study. Players performed unilateral CMJ and HJ tests to assess inter-limb asymmetry. A o

The Validity and Reliability of the Force Plates and the Linear Position Transducer in Measuring Countermovement Depth and Velocity During Countermovement Jump.

Countermovement jump (CMJ) is a key test for evaluating lower-limb neuromuscular function, and accurate measurement of countermovement depth (CMD) and countermovement velocity (CMV) is critical for determining optimal performance. However, the measurement validity and reliability of CMD and CMV-particularly when obtained from force plates (FP) and linear position transducers (LPT)-have remained uncertain. This study determined the validity and reliability of FP and LPT for measuring CMD and CMV. Twenty-eight male recreational athletes performed the CMJ test, and the variables were synchronousl

Disrupted sensorimotor control after ACL injury: from mechanoreceptor degeneration to neuroplasticity-oriented rehabilitation.

BACKGROUND: Anterior cruciate ligament (ACL) injury is one of the most common sports-related injuries, often resulting in not only mechanical instability but also long-term proprioceptive dysfunction. The ACL is richly innervated with mechanoreceptors that contribute to sensorimotor control. Injury-induced degeneration of these receptors leads to disrupted afferent signaling and maladaptive central nervous system (CNS) responses, which can compromise functional recovery and increase the risk of reinjury. OBJECTIVE: This review aims to summarize current knowledge on ACL-associated propriocepti

Case Series of Anesthetic Management of Gene Therapy in Children With Aromatic L-Amino Acid Decarboxylase Deficiency.

BACKGROUND: Aromatic L-amino acid decarboxylase (AADC) deficiency is a rare life-threatening inborn error of neurotransmitter biosynthesis. It is characterized by deficient biosynthesis of neurotransmitters dopamine and serotonin, leading to catecholamines deficiency and sympathetic deprivation, while the parasympathetic system remains functional. Since 2012, gene therapy has led to clinical improvements in symptoms and motor function with a severe phenotype. General anesthesia of children with AADC deficiency is challenging. AIM: Describe prolonged anesthesia management of children with arom

Managing Refractory Hypoxemia in Acute Respiratory Distress Syndrome Obese Patients with Veno-Venous Extra-Corporeal Membrane Oxygenation: A Narrative Review.

Veno-venous extracorporeal membrane oxygenation (vvECMO) is a life-saving intervention for severe respiratory failure unresponsive to conventional therapies. However, managing refractory hypoxemia in morbidly obese patients poses significant challenges due to the unique physiological characteristics of this population, including hyperdynamic circulation, elevated cardiac output, and increased oxygen consumption. These factors can limit the effectiveness of vvECMO by diluting arterial oxygen content and complicating oxygen delivery. Refractory hypoxemia in obese patients supported by vvECMO oft

Exploring psychosocial experiences in clinically stable generalised Myasthenia Gravis: A thematic analysis.

Myasthenia Gravis (MG) is a chronic autoimmune neuromuscular condition that significantly impacts patients' lives. Whilst psychosocial challenges are increasingly recognised as important in understanding the lived experience of patients, insight into these experiences during periods of clinical stability remains underexplored. This qualitative study explores the psychological and social aspects of living with MG through thematic analysis of semi-structured interviews with eight adults from a specialist MG care clinic in London, all considered clinically stable for at least three months.
