Asystole Due to Oculocardiac Reflex during Surgical Repair of an Orbital Blowout Fracture.

Bradycardia and asystole due to oculocardiac reflex (OCR) are potential intraoperative complications of periocular surgery. We report a case of asystole due to OCR that occurred during surgical repair performed 40 hours after an orbital blowout fracture. The patient had vomiting, bradycardia, and ocular motility disorder before the operation. During the operation, asystole occurred when the entrapped tissue was grasped with forceps. After administration of anticholinergic drugs, the heart promptly resumed beating, and no sequelae due to asystole were observed after surgery. Before and during p

Mutations in the β-tubulin gene TUBB5 cause microcephaly with structural brain abnormalities.

The formation of the mammalian cortex requires the generation, migration, and differentiation of neurons. The vital role that the microtubule cytoskeleton plays in these cellular processes is reflected by the discovery that mutations in various tubulin isotypes cause different neurodevelopmental diseases, including lissencephaly (TUBA1A), polymicrogyria (TUBA1A, TUBB2B, TUBB3), and an ocular motility disorder (TUBB3). Here, we show that Tubb5 is expressed in neurogenic progenitors in the mouse and that its depletion in vivo perturbs the cell cycle of progenitors and alters the position of migr

Progressive supranuclear palsy and primary lateral sclerosis secondary to globular glial tauopathy: a case report and a practical theoretical framework for the clinical prediction of this rare pathological entity.

Globular glial tauopathy (GGT) is a rare 4-repeat tauopathy characterized by the accumulation of tau globular inclusions in astrocytes and oligodendrocytes. Several clinical phenotypes have been associated with GGT, making the prediction of this rare pathological entity difficult. We report the case of a patient with eye-movement abnormalities and gait instability, reminiscent of progressive supranuclear palsy-Richardson's syndrome (PSP-RS), who later developed upper motor neuron symptoms suggestive of primary lateral sclerosis (PLS). Neuropathological assessment revealed GGT type III patholog

Eye movement deficits in X-linked dystonia-parkinsonism are related to striatal degeneration.

BACKGROUND: X-linked dystonia-parkinsonism (XDP) is characterized by the unique transition of dystonia to parkinsonism and striatal degeneration. Slowing of saccades on clinical examination has been taken as suggestive of a progressive supranuclear palsy (PSP) phenotype. OBJECTIVES: To elucidate whether eye movement abnormalities in XDP patients reflect striatonigral impairment or deficits in the brainstem saccade generator as present in PSP. METHODS: Eye movements of 18 male XDP patients from the Philippines and 16 ethnically and age-matched, healthy control participants were analyzed and t

Developmental visual perception deficits with no indications of prosopagnosia in a child with abnormal eye movements.

Visual categories are associated with eccentricity biases in high-order visual cortex: Faces and reading with foveally-biased regions, while common objects and space with mid- and peripherally-biased regions. As face perception and reading are among the most challenging human visual skills, and are often regarded as the peak achievements of a distributed neural network supporting common objects perception, it is unclear why objects, which also rely on foveal vision to be processed, are associated with mid-peripheral rather than with a foveal bias. Here, we studied BN, a 9 y.o. boy who has norm

Psychogenic Ophthalmologic Movement Disorders.

In this study, the authors retrospectively identified 11 patients with psychogenic ophthalmologic movement disorders (POMDs) (6%) among 182 patients with psychogenic movement disorders (PMDs), using medical charts and video reviews. The phenomenology included oculogyric crises (N=7), opsoclonus (N=5), and ocular flutter (N=1). No statistically significant differences were observed in gender and PMD distribution between patients with and without POMDs, although a trend for younger age at onset was observed in patients with POMDs. Seven patients showed improvement with psychotherapy, whereas two

Improvement of fine motor skills in children with visual impairment: an explorative study.

In this study we analysed the potential spin-off of magnifier training on the fine-motor skills of visually impaired children. The fine-motor skills of 4- and 5-year-old visually impaired children were assessed using the manual skills test for children (6-12 years) with a visual impairment (ManuVis) and movement assessment for children (Movement ABC), before and after receiving a 12-sessions training within a 6-weeks period. The training was designed to practice the use of a stand magnifier, as part of a larger research project on low-vision aids. In this study, fifteen children trained with a

Congenitally dysplastic inferior rectus muscle.

The authors report an unusual presentation of an idiopathic congenitally dysplastic inferior rectus muscle that responded well to surgical correction. Isolated unilateral enlargement of extraocular muscles is rare in children, and there is no definite logical explanation for its cause. A 20-month-old child presented with a congenitally enlarged posterior part of the right inferior rectus muscle with prominent hypotropia and enophthalmos since 10 months of age. Systemic disease work-up, ultrasound B-scan, computed tomography of the orbit and brain, and inferior rectus muscle biopsy were perform

Autosomal recessive ataxia, slow eye movements and psychomotor retardation.

Two consanguineous Arab families with an autosomal recessive form of hereditary ataxia with slow eye movements and psychomotor retardation are reported. The ataxia presented in the first decade, was severely disabling and was associated with a spectrum of eye movements abnormalities as well as psychomotor retardation and sensory neuropathy. MRI studies of the brain showed a significant degree of cerebellar and brainstem atrophy. These 2 families support a previous report of a similarly affected consanguineous Arab family. The syndrome of autosomal recessive ataxia and slow or even absent sacca

Eye movement abnormalities in Parkinson's disease motor subtypes: a video-oculographic study.

INTRODUCTION: Eye movement dysfunction has been described in Parkinson's disease (PD), but differences between tremor-dominant (TD) and postural instability/gait difficulty (PIGD) PD motor subtypes remain poorly understood. The aim of this study was thus to compare video-oculographic (VOG) features between PD motor subtypes. METHODS: Two hundred and four PD patients and 55 age-matched healthy control subjects (HC) were enrolled in this study. PD patients were stratified into PIGD and TD motor subtype groups. VOG amplitude, peak velocity of upward, downward, and vertical saccades, and square w

Unusual presentations of Hashimoto's encephalopathy: trigeminal neuralgiaform headache, skew deviation, hypomania.

Hashimoto's encephalopathy (HE) is a term used to describe an encephalopathy of presumed autoimmune origin characterized by high titers of antithyroid-peroxidase antibodies. We describe three patients showing unusual clinical presentations like trigeminal-neuralgia, skew deviation, hypomania associated with HE. The purpose of this article is to describe the patients with unusual clinical pictures of HE, something that has not been reported in elsewhere in medical literature.

[Joubert's syndrome. Presentation of two adult siblings with favorable evolution].

We present the case of two siblings with Joubert's syndrome. They had breathing problems of unknown origin during the neonatal period. Their evolution has been followed-up from the first years of age until the present. They are now 26 and 22 years old, respectively. Both patients have a "borderline" mental level, are apparently normal, but have had progressive evolution and psychic and motor improvement. They can read, write and use the four operations in mathematics and can work with the computer. Both siblings are working in the familial agriculture, can play football at an elemental level w

CT of orbital trauma.

In a patient with acute orbital trauma, visual acuity and extraocular muscle motility are the two most important ophthalmologic functions to be evaluated emergently. The assessment of these capabilities may sometimes be difficult due to the severity of the head injury, the extent of periorbital soft tissue edema, inadequate cooperation in alert patients, and a reduced level of consciousness in obtunded individuals. Consequently, computed tomography (CT) has come to play a major role in the orbital examination of acute trauma patients. In this study, in conjunction with clinical evaluation, we

Optometric findings in velocardiofacial syndrome.

Velocardiofacial syndrome (VCFS) is a congenital genetically determined condition with skeletal, heart, and facial anomalies. Almost all children have learning problems, despite an often normal or above average IQ. We have examined 10 children with a prior diagnosis of VCFS. All were mildly to moderately hyperopic. Most had poor accommodation, and convergence was clinically poor. About 30% had tortuous retinal vessels. VCFS may be detected in the optometry clinic, and optometric management may ameliorate the learning problems in some cases.

A case of chorioretinal coloboma in a patient with achondroplasia.

Achondroplasia is a congenital disorder resulting from a specific disturbance in endochondral bone formation. The ophthalmic features reportedly associated with achondroplasia are telecanthus, exotropia, inferior oblique overaction, angle anomalies and cone-rod dystrophy. This is first report of chorioretinal coloboma in achondroplasia. An 8-year-old female was diagnosed with a developmental delay, known as achondroplasia, seven months after birth. Upon her initial visit, visual acuity was 0.3 in both eyes. The patient had telecanthus but normal ocular motility. Findings were normal upon anter
