Clinical outcomes following open reduction and internal fixation without primary cruciate ligament reconstruction or avulsion fixation for Schatzker types IV-VI tibial plateau fractures: a retrospective cohort study.

BACKGROUND: Schatzker types IV-VI tibial plateau fractures (TPFs) are frequently complicated by soft tissue injuries. This study aimed to evaluate the clinical and functional outcomes of Schatzker types IV-VI TPFs treated with ORIF alone, and to compare outcomes between patients with and without concomitant anterior or posterior cruciate ligament (ACL or PCL) injuries. METHODS: 316 Schatzker types IV-VI TPFs patients with a mean age of 51.5 ± 13.2 years old who underwent open reduction and internal fixation (ORIF) were enrolled. There were 152, 56 and 108 patients with type IV, type V, and ty

Revision arthroplasty versus open reduction and internal fixation in Vancouver type B2 and B3 periprosthetic femoral fractures: results from a matched pairs analysis of the registry for geriatric trauma of the DGU®.

BACKGROUND AND OBJECTIVES: Owing to a lack of evidence, the appropriate surgical treatment strategy for geriatric patients with Vancouver type B2 or B3 periprosthetic femoral fractures (PFFs) remains unclear. Data from a large international geriatric trauma registry were analyzed to investigate the medical care situation of such patients, as well as to examine the outcomes related to revision arthroplasty (RA) or open reduction and internal fixation (ORIF). MATERIALS AND METHODS: Datasets from the Registry for Geriatric Trauma of the German Trauma Society (Deutsche Gesellschaft für Unfallchir

Elbow hemiarthroplasty versus open reduction and internal fixation for AO/OTA type 13 C2 and C3 fractures of distal humerus in patients aged 50 years or above: a randomized controlled trial.

BACKGROUND: Intraarticular distal humeral fractures of AO/OTA type 13 C2 and C3 pose a surgical challenge despite the evolution of surgical implants and techniques. Open reduction and internal fixation (ORIF) is often preferred as the first choice of treatment, but the results vary and are sometimes disappointing. Total elbow arthroplasty (TEA) has been widely used for fractures that are not amenable to ORIF in elderly patients, but the mechanical complications remain a challenge, especially in active patients. Elbow hemiarthroplasty (EHA) provides a modern alternative that might avoid the mec

Weekend open reduction and internal fixation of distal radius fractures associated with higher complication and readmission rates: a nationwide analysis of two hundred and sixty six thousand, three hundred and seventy eight patients.

PURPOSE: The "weekend effect" suggests that patients operated upon weekends may experience worse outcomes. This study evaluates whether the day of the week impacts outcomes and costs of open reduction and internal fixation (ORIF) surgery for distal radius fractures (DRF) in a large nationwide cohort. METHODS: A retrospective cohort study was conducted using data from the PearlDiver Mariner M165Ortho dataset, which covers a U.S. population of over 165 million from 2010 to 2022. Patients who underwent ORIF surgery for DRF were categorized into weekday and weekend groups. Demographics, comorbidi

Comparison of anesthesia efficacy between subclavius muscle plane block and interscalene brachial plexus block for open reduction and internal fixation of clavicle fractures: study protocol for a randomized non-inferiority clinical trial.

BACKGROUND: Interscalene brachial plexus block (ISBPB) combined with supraclavicular nerve block or superficial cervical plexus block has been reported to be a reliable, efficacious anesthesia technique for clavicle fracture surgery. However, several serious adverse events impeded the development of this technique. Herein, we propose a novel nerve block technique-the subclavius muscle plane block (SMPB) combined with supraclavicular nerve block-that selectively targets the sensory nerves innervating the surgical region of the clavicle. We hypothesize that SMPB may achieve non-inferior anesthes

How good is good enough? Lessons learned from review of outcomes of 50 patients following extraoral open reduction and internal fixation of condylar neck and base fractures at a regional major trauma centre.

The management of mandibular fractures is routine owing to their frequency, particularly those of the mandibular condyle, which are invariably common. Notwithstanding disparities in the literature, the international consensus towards which fractures of the mandibular condyle benefit from open surgical intervention is almost universally accepted. Closed reduction or non-operative management may be utilised in certain circumstances. Whilst outcome research has addressed complications with respect to operative side effects, outcomes related to the quality of bone repair and the setting in which t

Open Reduction and Stable Internal Fixation Using a 3.5-mm Locking Hook Plate for Isolated Fractures of the Greater Tuberosity of the Humerus: A 2-Year Follow-up Study Using an Innovative Fixation Method.

BACKGROUD: The best treatment for isolated greater tuberosity (GT) fractures is still controversial. Although previous studies have suggested surgical options, they are either unable to provide firm fixation or present with a variety of complications. METHODS: We retrospectively studied the records of patients with isolated GT fractures who underwent open reduction and internal fixation using a 3.5-mm locking hook plate between January 2016 and January 2018. The surgical indication was an at least 5-mm displacement of the GT as observed in either simple radiography or three-dimensional comput

A Prospective Study Comparing the Functional Outcomes of Complex Tibial Plateau Fractures Using Computed Tomography-based Three-column Theory Versus X-Ray-based Schatzker Classification Treated with Open Reduction with Locking Compression Plates.

INTRODUCTION: Tibial plateau fractures are complex intra-articular injuries that require precise classification for optimal management. The widely used Schatzker classification, based on X-rays, provides a two-dimensional assessment, while the computed tomography (CT)-based three-column theory offers a more detailed three-dimensional analysis. This study aimed to compare the functional outcomes of complex tibial plateau fractures classified using these two systems and treated with locking compression plates (LCP). MATERIALS AND METHODS: A prospective, observational study was conducted involvi

[Nasal Fracture].

Nasal fractures are the most prevalent among facial fractures, constituting 40-50% of all such injuries. They can be isolated or accompany multi-system trauma, including damage to soft tissues or other fractures. When addressing a patient with a suspected nasal fracture, anamnesis is paramount. It is crucial to discern the mechanism and timing of the trauma. The latter is essential as the most recent data indicates a 14-day "treatment window" for closed reduction, a procedure that does not require general anesthesia. Contrary to past practices, imaging studies are now deemed unnecessary for di

Open Versus Closed Management for Gunshot Wounds of the Mandible - Which More Frequently Achieves Satisfactory Bony Union? A Systematic Review and Meta-Analysis.

PURPOSE: Closed management of high-energy mandibular injuries has been considered the gold standard for many years, but open management is preferable for its shorter overall treatment time. At present, there is a paucity of evidence on which to base management decisions, and as such there is a priority to review the literature to define the optimal treatment for mandibular gunshot wounds (GSWs). METHOD: The study design was a systematic review with meta-analyses and the protocol was designed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis statement.
