The time course and phenotype of Uhthoff phenomenon following optic neuritis.

As there is little information regarding the recovery from Uhthoff phenomenon (UP) in the multiple sclerosis (MS) literature, the objective of this study was to assess the phenotypes of UP. A one-page questionnaire was sent to 80 consecutive optic neuritis (ON) patients seen in a tertiary neuro-ophthalmology clinic. Of the 48 responders to the questionnaire, 52% reported experiencing UP, with a range of follow-up from 1 to 20 years. Only 16% had resolution of UP, all this occurred within 8 weeks of the onset of ON. Of the MS patients with UP, 88% experienced non-visual heat-related phenomenon …

Hyperbaric oxygen therapy and optic neuritis: case report and literature review.

A history of optic neuritis has long been considered a relative contraindication to hyperbaric oxygen therapy. However, the published medical literature regarding the use of hyperbaric oxygen therapy in patients with previous optic neuritis is very limited, and patients who might benefit from hyperbaric oxygen therapy may be denied its benefit without adequate consideration of the risk-benefit ratio. We present a case report of a patient with a history of optic neuritis who underwent comprehensive ophthalmologic evaluation before and after 40 treatments with hyperbaric oxygen therapy, with no …

[Optic neuritis, multiple sclerosis-related or not: structural and functional study].

INTRODUCTION: Multiple sclerosis (MS) is an inflammatory demyelinating disease with axonal degeneration. Optical coherence tomography (OCT) is a noninvasive technique that quantifies the thickness of the retinal nerve fiber layer (RNFL). The objectives of this study are: to determine the thickness of the RNFL in MS patients with or without previous optic neuritis (ON) and in patients with ON and to determine the relationship between the structural damage and functional alterations in visual acuity (VA) and visual field (VF). METHODS: Three groups were studied: G1 - patients with ON, G2 - pati…

Optic neuritis associated with Q fever: case report and literature review.

Optic neuritis (ON) is a rare association of Q fever. We report the first case of ON associated with Q fever in Australia and review all previously reported cases in the medical literature. The impact of therapy with antibiotics and steroids on outcome is discussed. Q fever should be considered in the differential diagnosis of patients presenting with apparent acute idiopathic ON.

Pediatric optic neuritis: brain MRI abnormalities and risk of multiple sclerosis.

BACKGROUND: Optic neuritis is often the initial presentation of multiple sclerosis (MS). As established by the Optic Neuritis Treatment Trial, an abnormal baseline brain MRI is a strong predictor of MS after isolated optic neuritis in adults. However, the rate of conversion to MS after optic neuritis in children based upon brain MRI findings is unknown. METHODS: We reviewed the medical records of children (<18 years) presenting with optic neuritis between 1993 and 2004 at the Children's Hospital of Philadelphia. Children with a history of demyelinating disease or prior optic neuritis were exc…

Acute inflammatory demyelinating optic neuritis: evidence-based visual and neurological considerations.

BACKGROUND: Optic neuritis (ON) is an acute inflammatory demyelinating disorder of the optic nerve that occurs most often in young adults. It can be a monophasic or polyphasic disease isolated to the optic nerve(s) or can be associated with a more widespread demyelinating disorder of the central nervous system such as multiple sclerosis (MS) or neuromyelitis optica. Advances in therapeutics that modify the risk of progression to MS have emphasized accurate diagnosis and risk assessment of patients with ON. REVIEW SUMMARY: ON usually presents with acute unilateral visual loss associated with o…

Multifocal visual evoked potential analysis of inflammatory or demyelinating optic neuritis.

OBJECTIVE: To determine the sensitivity of multifocal visual evoked potentials (mVEP) in optic neuritis of an inflammatory or demyelinating nature. DESIGN: Cross-sectional study. PARTICIPANTS: Sixty-four patients participated who had a confirmed diagnosis of optic neuritis (ON) (past and acute). Based on the McDonald multiple sclerosis (MS) criteria, 25 patients (27 eyes with ON) were deemed to have isolated optic neuritis and thus not have MS (i.e., the not-MS group), and 19 patients (24 eyes with ON) had a diagnosis of MS (i.e., the MS group). The remaining 20 patients (25 eyes with ON) we…

Isolated bilateral anterior optic neuritis following chickenpox in an immunocompetent adult.

Chickenpox may lead to several different neurological complications, but optic neuritis has rarely been described; in particular, only one case of isolated bilateral anterior optic neuritis (AON) in an immune-competent adult has so far been reported. We describe a second case of this type and consider similarities and differences between our patient and all other cases of AON following chickenpox. Then, we discuss the therapeutic role of steroids and advance the hypothesis of different pathogenetic pathways in immune-competent and immune-compromised subjects.

Adaptive cortical plasticity in higher visual areas after acute optic neuritis.

The ability to distinguish adaptive cortical reorganization may help to target future therapeutic strategies after neurological insult. We investigated cortical plasticity by prospectively applying visual functional magnetic resonance imaging (fMRI) and optic nerve MRI to 20 patients with acute optic neuritis at baseline, 1, 3, 6, and 12 months. We performed three types of correlation analyses to investigate the relationships between fMRI activity, clinical function, and optic nerve structure. The first analysis directly correlated the fMRI response to clinical function or optic nerve structur…

Bilateral optic neuritis in a child following Epstein-Barr virus infection.

A rare case of bilateral optic neuritis is presented in a child with no light perception. Ophthalmic examination revealed dilated pupils without reaction to the light, swollen optic discs with small peripapillary hemorrhages in both eyes. Serology revealed evidence of recent Epstein-Barr virus infection. After treatment with high dose of corticosteroid visual acuity gradually improved. After four months visual acuity was normal despite complete pallor of the optic disc. Ebstein-Barr virus infection should be considered in the differential diagnosis of bilateral optic neuritis in a child with s…

Multimodality visual evoked potentials in evaluating visual dysfunction in optic neuritis.

Visual information is processed via multiple, parallel channels. The present study examined the clinical feasibility of multimodality visual evoked potentials (VEPs) in optic neuritis. We recorded transient VEPs to 30' checkerboard patterns, chromatic and achromatic sinusoidal gratings and apparent motion, and steady-state VEPs to achromatic gratings in 15 normal controls and 14 patients. VEPs to 30' checks were abnormal in 10 eyes (7 patients); however, considering all five modalities, abnormal responses were seen in 20 eyes (12 patients). Abnormality rates were not equal among the visual sti…
