Unilateral congenital dislocation of the knee and hip: a case report.

We report our experience with a unilateral congenital dislocation of the knee associated with a developmental dysplasia of the hip on the same side. Our case is a good example of congenital dislocation of the knee caused by abnormal intrauterine pressure leading to this type of congenital postural deformity. To our knowledge this is the first case of congenital dislocation of the knee reported after cervical cerclage of an incompetent cervix to prevent a pre-term delivery. The Pavlik Harness was used to treat the knee and the hip at the same time with a satisfactory result after 20 months of f

Orthotic intervention and postural stability in participants with functional ankle instability after an accommodation period.

CONTEXT: Most protocols established to treat patients with functional ankle instability (FAI) have focused on taping the ankle. Orthotic intervention is a different treatment protocol that may have a positive effect on these patients, especially after an accommodation period. OBJECTIVE: To determine whether the use of a prefabricated orthotic affects postural stability in patients with FAI and a control group. DESIGN: Randomized controlled clinical trial. SETTING: Research laboratory. PATIENTS OR OTHER PARTICIPANTS: Forty patients with unilateral FAI. INTERVENTION(S): Postural stability w

Early observations of the effects of lateral wedge orthoses on lower limb muscle length and potential for exacerbating spasticity.

Lateral wedge orthoses are often prescribed to correct varus deformity after stroke. Spasticity is implicated in varus deformity and is caused by velocity-related muscle length changes, so a lateral wedge may affect spasticity by manipulating foot and ankle kinematics which, in turn, may alter the length of these muscles. We sought to test this theory in healthy participants. Eight volunteers walked with no wedge and with lateral wedges of 5 and 8.5 degrees in both shoes. Qualysis motion capture system collected kinematic data and SIMM musculoskeletal modeling software calculated muscle tendon

Effects of foot orthoses and valgus bracing on the knee adduction moment and medial joint load during gait.

BACKGROUND: Lateral shoe wedges and valgus knee braces are designed to decrease the force acting in the medial knee compartment by reducing the external adduction moment applied at the knee. The biomechanical changes introduced by these orthoses can be relatively small. Computer modeling and simulation offers an alternative approach for assessing the biomechanical performance of these devices. METHODS: A three-dimensional model of the lower-limb was used to calculate muscle, ligament, and joint loading at the knee during gait. A lateral shoe wedge was simulated by moving the center of pressur

Neck and back muscle loading in pilots flying high G(z) sorties with and without lumbar support.

INTRODUCTION: Fighter pilots frequently encounter neck and back pain and injuries due to high G(z) loading. A lumbar support could diminish muscle strain in the lower back under G(z) loading and might also have a positive effect on cervical muscle strain, due to a more ergonomic sitting posture. The aim of this study was to determine the effects of individually adjusted lumbar supports on fighter pilots' neck and back muscle loadings. METHODS: There were 11 Finnish Air Force pilots who acted as test subjects. They flew two basic air combat maneuvering sorties with and without the LS. The EMG

Evaluating the relationships between the postural adaptation of patients with profound cerebral palsy and the configuration of the Seating Buggy's seating support surface.

We are currently investigating the physiological polymorphism of wheelchair users with profound cerebral palsy and the properties of the Seating Buggy (developed by S. Nishimura, 1998) to clarify important and general elements of wheelchairs for widespread use. Cerebral palsy is a diagnostic term used to describe a group of motor syndromes resulting from disorders in early brain development. Recently, it has been shown that the Seating Buggy produces functional head-neck alignments and active control of sitting balance for people with profound cerebral palsy. The Seating Buggy is a wheelchair

Carbon fibre reinforced plastic knee-ankle-foot orthosis with a partially flexible thigh cuff: a modification for comfort while sitting on a toilet seat.

At the request of a polio survivor, a partially flexible thigh cuff made of leather and canvas for a carbon KAFO was devised to allow the wearer to feel more comfortable while sitting on a toilet seat. The original, acrylic resin, thigh cuff was partially excised to make an opening (15x10 cm), which was stuffed with rubber sponge, and was sealed with leather and canvas. The opening's surround was vertically and horizontally reinforced with carbon fibres. This modification provided relief to the polio survivor from the discomfort previously experienced while sitting on a toilet seat, and satisf

Correlations between subjective treatment responses and plantar pressure parameters of metatarsal pad treatment in metatarsalgia patients: a prospective study.

BACKGROUND: Metatarsalgia is related to repetitive high-pressure loading under the metatarsal head (MH) that causes pain. The high pressure under the MH can be reduced by adequately applying metatarsal pads (MPs). Plantar pressure measurements may provide a method to objectively evaluate pressure loading under the MH. However, it is still unclear if the decrease in plantar pressure under the MH after MP treatment is associated with subjective improvement. This study aims to explore the correlations between subjective pain improvement and outcome rating, and the plantar pressure parameters in m

Hindfoot containment orthosis for management of bone and soft-tissue defects of the heel.

BACKGROUND: Bone, soft-tissue, and nerve deficits of the weightbearing surface of the foot are frequent sequelae from foot trauma or diabetes mellitus and present challenging treatment issues. Injury to the specialized, shock-absorbing, heel-pad tissue containing spirally arranged fat chambers is particularly difficult to manage. Appropriate footwear modifications and shoe inserts for protection of this skin are essential to the long-term management of bone and soft-tissue defects of the heel. This study evaluated the performance of a new custom total contact foot orthosis (Hindfoot Containmen

The natural history of idiopathic toe-walking: a long-term follow-up of fourteen conservatively treated children.

AIM: To determine the long-term results after conservative treatment (physiotherapy, casting, orthoses, or a combination of these) of idiopathic toe-walking (ITW). METHODS: Tiptoe-walking is diagnosed as idiopathic (habitual) if no signs of neurological, orthopaedic, or psychiatric disease are detected. The diagnosis is one of exclusion. Sixteen former patients with ITW, all now at least 13 y old, were asked to participate in a follow-up investigation 7-21 y after being first diagnosed. Two cases were excluded because heel-cord lengthening had been performed later on in other hospitals. The r

A comparison of the effects of solid, articulated, and posterior leaf-spring ankle-foot orthoses and shoes alone on gait and energy expenditure in children with spastic diplegic cerebral palsy.

Fourteen children with spastic diplegic cerebral palsy were evaluated wearing threedifferent ankle-foot orthoses and shoes alone. The ankle-foot orthoses included solid, articulated ankle, and posterior leaf-spring types. Evaluation measures included computerized gait analysis, Energy Efficiency Index data, and individual preference. Highly significant kinematic differences were found at the ankle with shoes alone approaching normative data and braces showing abnormal dorsiflexion. No significant differences, were found in velocity, cadence, stride length, or in the Energy Efficiency Index. Ei

A body-powered functional upper limb orthosis.

This paper describes the development and preliminary testing of a functional upper-limb orthosis for people that have limited strength in their arms. This is symptomatic of conditions such as muscular dystrophy (MD), spinal muscular atrophy (SMA), and partial spinal cord injury. The exoskeletal orthosis is wheelchair mounted, has two links and four degrees of freedom. It uses linear elastic elements to balance out the effects of gravity in three dimensions. Preliminary results on testing with ten subjects will be presented.
