Neutrophil-Driven Inflammation in Pediatric Chronic Recurrent Multifocal Osteomyelitis.

OBJECTIVE: Chronic recurrent multifocal osteomyelitis (CRMO) is a pediatric autoinflammatory bone disease with poorly defined pathophysiology and limited biomarkers. Murine models implicate neutrophils, neutrophil-derived IL-1β, and the adaptor protein PSTPIP2, but direct evidence in human CRMO is lacking. This study aims to translate findings from mouse models to human disease by investigating the role of neutrophils and PSTPIP2 in CRMO. METHODS: Blood samples from 34 patients with CRMO and 20 controls were analyzed by flow cytometry, biochemical assays, and transcriptomic profiling of purif…

A Case Report of Vitamin C Deficiency Mimicking Osteomyelitis.

Vitamin C, also known as ascorbic acid, plays a pivotal role in forming blood vessels, cartilage, muscles, and collagen in bones. We report a 6-year-old non-verbal female with global developmental delay who presented with complaints of lower limb pain and inability to bear weight. Symptoms started five weeks prior to presentation and had progressed from decreased activity to complete loss of weight-bearing. Physical examination showed gingival hyperplasia, perifollicular petechiae, lower limb edema, and corkscrew hair. Initial radiologic findings raised concerns of osteomyelitis, showing bone …

Bilateral femur osteomyelitis: A rare case with successful outcome.

UNLABELLED: Chronic femoral osteomyelitis with segmental bone loss remains a complex reconstructive challenge, particularly in the setting of failed prior fixation and multidrug-resistant infection. Bilateral femoral involvement is extremely rare and lacks a standardized management algorithm. This report aims to describe a staged, limb-salvage strategy employing different reconstruction techniques for each femur based on defect size, biological potential, and infection status. CASE PRESENTATION: A 28 years old male presented with bilateral thigh pain and inability to ambulate three years afte…

Severe Necrotizing Locally Advanced Basal Cell Carcinoma With Osteomyelitis.

Locally advanced basal cell carcinoma (BCC) is an uncommon and complex presentation of BCC. These tumors can present as extensive nonhealing ulcers and require skin biopsy to rule out other causes, including malignant, infectious, and inflammatory etiologies. In this case report, a 57-year-old female without significant past medical history presented with a 5-year history of a chronic progressive ulcer involving the neck and back. Histopathological study of the tissue identified nodular and infiltrative BCC, prompting further multidisciplinary treatment and management with vismodegib, radiatio

Idiopathic primary sternal osteomyelitis in an otherwise healthy patient.

Primary sternal osteomyelitis (PSO) is an exceptionally rare infection of the sternum, typically occurring in the context of trauma, surgery or immunocompromise. We report the case of a previously healthy adolescent boy in his second decade of life presenting with fever, chest pain and localised tenderness without any identifiable risk factors. Initial workup, including a chest X-ray and echocardiogram, was unremarkable, but persistent bacteraemia with methicillin-sensitive Staphylococcus aureus prompted further imaging. MRI revealed sternal osteomyelitis with a small parasternal abscess. The …

Multiplex quantification of osteomyelitis pathogens based on bead array.

Accurate diagnosis of osteomyelitis is hindered by the limitations of conventional culture‑based methods, which lack the capacity for rapid simultaneous identification and quantification of multiple pathogens. Mixed infections involving multiple pathogens often exacerbate clinical outcomes, necessitating rapid and multiplex diagnostic approaches. We developed a multiplex detection assay integrating asymmetric solid-liquid biphasic PCR with a bead array platform, designated ASL-PCR, for the simultaneous identification of five common osteomyelitis‑associated pathogens: Staphylococcus aureus (S. …

Surgical management for distal phalanx osteomyelitis: A narrative review.

Distal phalanx osteomyelitis, although rare, represents a challenging infection that can have devastating functional and cosmetic consequences if inadequately managed. In patients with comorbidities-most notably diabetes mellitus-the infection may progress rapidly, causing bone destruction, soft tissue involvement, and ultimately, joint destruction. This review discusses the pathophysiology, diagnosis, and surgical management of distal phalanx osteomyelitis. It also highlights key gaps in the current literature, particularly the lack of standardized treatment protocols and limited long-term ou…

Novel Treatments for Diabetic Foot Osteomyelitis: A Narrative Review.

Diabetic foot osteomyelitis (DFO) is a severe complication of diabetes mellitus and a leading cause of non-traumatic lower extremity amputation. Treatment remains clinically challenging with high recurrence rates despite standard antibiotic therapy and surgical debridement. This narrative review synthesizes current evidence on novel operative and nonoperative therapies for DFO, focusing on emerging biomaterials, local antibiotic delivery systems, innovative surgical techniques, and adjunctive topical agents. Studies examining bioabsorbable and nonabsorbable antibiotic carriers, such as calcium…

Biportal Endoscopic Intramedullary Debridement for Management of Tibial Osteomyelitis.

Chronic osteomyelitis occurs if the bone infection persists for more than 6 weeks. The presence of dead bone (sequestrum) and intramedullary abscess with chronic infected tissue may protect bacteria from the endogenous host immune response and limit the effectiveness of many antibiotics. Classically, removal of infected tissue inside the medullary cavity requires extensive osteotomy to create a bone window of adequate size. Bone endoscopy (medulloscopy) provides a minimally invasive approach to the medullary cavity of long bones. The purpose of this Technical Note is to describe the details of…

Intracellular survival of Staphylococcus aureus in macrophages during osteomyelitis.

Staphylococcus aureus, traditionally viewed as an extracellular pathogen, is increasingly recognized for its ability to persist intracellularly, particularly within macrophages. This intracellular lifestyle is central to osteomyelitis, a chronic bone infection characterized by persistent inflammation, bone destruction, and impaired repair. Within bone, S. aureus exploits macrophage plasticity by driving a shift from pro-inflammatory, bactericidal M1-like states to anti-inflammatory, tissue-reparative M2-like phenotypes. This polarization suppresses immune clearance and promotes an environment …

Case Report: Osteomyelitis in a giant panda (Ailuropoda melanoleuca).

Osteomyelitis is an inflammatory disease of bone tissue induced by microbial infection, and poses a significant health burden in humans and animals. The global annual incidence in humans is estimated ~1-10 cases per 100,000 individuals, with notably higher rates observed in high-risk populations. In domestic cubs and companion animals, trauma-related osteomyelitis can occur at an incidence rate ranging from 0.1 to 5%. Although osteomyelitis is well-documented in both human and veterinary medicine, with diverse and complex diagnostic and therapeutic approaches available for human cases, it has …

Immunometabolic reprogramming in diabetic osteomyelitis: from mechanisms to therapeutics.

Diabetes mellitus (DM) is a globally prevalent metabolic disorder characterized by impaired immune function due to poor glycaemia control, significantly increasing the risk of osteomyelitis. The occurrence of bone infection not only compromises patients' quality of life but also poses substantial challenges in clinical management. Recent studies have identified immunometabolic reprogramming as a pivotal player in the pathogenesis and progression of diabetic osteomyelitis. This reprogramming not only disrupts immune cell functionality but also modulates the local microenvironment, thereby impai…

Unusual presentation of retropharyngeal abscess complicated by vertebral osteomyelitis.

Retropharyngeal abscesses are rare in adults and often associated with immunocompromised states or complications from foreign body ingestion. This case report highlights an unusual presentation of acute retropharyngeal abscess that subsequently led to cervical vertebral osteomyelitis and focal neurological deficit. We describe a middle-aged man who presented with a 10-day history of progressive dysphagia, right upper limb weakness and respiratory distress, culminating in stridor. Examination revealed a bulge in the posterior pharyngeal wall and motor weakness in the right upper limb (3/5 power…

[Osteomyelitis in diabetic foot: reconstruction with autologous bone graft].

Moderate to severe infections in the diabetic foot (DF), such as osteomyelitis, frequently require surgical intervention and prolonged antibiotic therapy. Amputation is often unavoidable, with high morbidity and mortality rates in the initial years. Bone resection can lead to deformities and loss of foot function, introducing further complications. This report presents a case of a 45-year-old patient with type 2 Diabetes Mellitus (DM) and osteomyelitis in the first ray, managed through surgical debridement, an antibiotic-loaded cement spacer, and vacuum-assisted closure (VAC) to control infect…
