Actinomycosis of the middle turbinate: an unusual cause of nasal obstruction.

Actinomycosis is a rare chronic infectious disease caused by Actinomyces israelii, which is an anaerobic filamentous, gram-positive saprophyte organism of the oral cavity. Historically, these bacteria were known as fungi because of their light microscopic appearance. Actinomycosis consists of three different forms: cervico-facial (the most common form), abdominal and pulmonothoracic. It commonly involves the head and neck region including the mandible, paranasal sinuses, lacrimal gland, parotid gland and orbit. Poor oral hygiene and dental diseases have been known to be the source of actinomyc

Oral diseases possibly associated with hepatitis C virus.

Morbidity associated with hepatitis C virus (HCV) infection can involve a variety of extrahepatic conditions, including lichen planus (LP) and sialadenitis, predominantly or exclusively involving the oral region, conditions which have been largely neglected in reviews. The literature suggests that HCV-infected patients may frequently have Sjögren-like sialadenitis with mild clinical symptoms, whereas oral LP may be significantly associated with HCV infections in Southern Europe and Japan but not in Northern Europe. These geographical differences could be related to immunogenetic factors such a

Parotid preservation or immune protection? The dual dilemma in head and neck cancer radiotherapy.

Head and neck cancer (HNC) represent a common category of malignant tumors. Radiotherapy, as the primary treatment modality for these tumors, while effectively controlling tumor progression, is often accompanied by a series of treatment-related side effects. As a major salivary gland in the head-and-neck region, the parotid gland (PG) is particularly susceptible to radiation damage during radiotherapy, given its anatomical proximity to the target irradiation area. The radiation dose and irradiated volume of the PG not only disrupt its physiological secretory function, leading to debilitating s

Adamantinoma‑like Ewing Sarcoma of Head and Neck: A Clinicopathologic Study of Three Cases Including One EWSR1-Negative Case with NF1 Mutation.

Adamantinoma-like Ewing sarcoma (ALES) is a rare variant of Ewing sarcoma that predominantly occurs in the head and neck region, characterized by epithelial differentiation and overlapping morphology with other small round cell tumors. We present three cases of head and neck ALES (two in the parotid gland and one in the nasal cavity) in patients aged 23-42 years. Histologically, the tumors exhibited a spectrum of features, including small round blue cells and basaloid squamous morphologies. Immunohistochemically, the tumor cells variably expressed CKpan, p63, CD99, NKX2.2, FLI1, CD56, and Syn.

Hydatid Cyst of the Oromaxillofacial Region - A Rare Location: A Case Report and Review of the Literature.

Oromaxillofacial hydatid cysts are rare, even in countries where echinococcosis is endemic. This study aimed to describe an isolated oromaxillofacial hydatid cyst and to discuss the epidemiology, clinical features, and treatment thereof. The authors report the case of a 12 year-old boy who presented with a 2-week history of an enlarging hydatid cyst in the parapharyngeal space extending to the neck, with no pulmonary or hepatic involvement. We present our clinical findings, diagnostic techniques, and treatment and review the relevant literature. Forty-one cases, with a male:female ratio of 1.2

Frey's syndrome following a facial burn treated with botulinum toxin.

Frey's syndrome occurs as a result of damage to the auriculotemporal nerve, which causes inappropriate regeneration of damaged parasympathetic fibres to salivary glands to innervate the sympathetic receptors of sweat glands in the face. The symptoms are pathological flushing and sweating with gustatory stimuli. It most commonly occurs following parotid surgery and has not previously been reported following burn injury. We present a 50-year-old man who sustained 1% TBSA full thickness burn to the right side of his face as a child. This was excised and reconstructed with skin grafts as well as f

Peripheral primitive neuroectodermal tumor of the parotid gland in a child: A case report.

Primitive neuroectodermal tumor (PNET) is a term used to describe a group of highly malignant neoplasms of soft-tissue origin, with varying degrees of divergent differentiation. The occurrence of peripheral PNET in the head and neck region has been reported infrequently in the medical literature. This disease generally occurs in adolescents and young adults, and rarely occurs in children <3 years old. The current study presents an extremely rare case of pPNET of the parotid gland in a 2-year-old male, which had been previously misdiagnosed and treated as a mumps. The lesion showed the characte

[Effective treatment by both anti-androgen therapy and chemotherapy for a patient with advanced salivary duct carcinoma].

Salivary ductcarcinoma (SDC)is a high-grade malignant tumor arising predominantly in the parotid gland. Androgen receptor(AR)expression was mainly restricted to SDC in salivary cancer. We report successful treatment of a patient with advanced SDC using an endocrine chemotherapy. A 76-year-old man was hospitalized with lumbalgia and swelling of left submandibular region. Radiological examination indicated a tumor in left submandibular gland and metastatic tumors in lumbar vertebra, accompanied by swollen lymph nodes of the neck, mediastinum. Needle biopsies of both vertebral tumor and cervical

Epithelioid sarcoma of the parotid gland of a child.

Epithelioid sarcoma is a rare soft-tissue tumor that usually occurs in young adults, with a median age of 26 years. This malignancy has been divided into distal and proximal types. The latter is found in proximal body sites including the head and neck region. We present a rare case of parotid proximal-type epithelioid sarcoma in a 1-year-old male child; this is the 4th reported case in the literature and the youngest in a pediatric patient to date. The tumor showed prominent rhabdoid features by light microscopy. Immunohistochemical studies revealed positive staining to cytokeratin (AE1/AE3),

Clonality analysis of lymphoproliferative disorders in patients with Sjögren's syndrome.

The aim of this study was to clarify the nature of the clonal lymphocyte infiltration in Sjögren's syndrome (SS) patients associated with lymphoproliferative disorders. We examined B cell clonality in lymphoproliferative tissues from six primary SS patients associated with lymphoproliferative disorders or lymphoma by cloning and sequencing of the gene rearrangement of the immunoglobulin heavy chain complementarity determining region 3 (IgVH-CDR3). Three patients with sequential observation showed progressional clonal expansion with the presence of the same subclone in different tissues during

[Isolated paralysis of the ramus marginalis mandibulae nervi facialis: clinical aspects, etiology, diagnosis and therapy. An overview].

BACKGROUND: Disorders of lower lip in facial expression are mostly caused by functional impairment of the marginal mandibular branch of the facial nerve. In a typical case, active exposure of the incisors of the mandible is not possible and the lip appears distorted towards the healthy side. The most frequent cause of a lesion of the marginal mandibular branch is iatrogenic injury during operations in the mandibular or parotid region. DIFFERENTIAL DIAGNOSIS: Functional disorders of the platysma, congenital hypoplasia or aplasia of the lower lip muscles, and defective healing following facial …

The temporoparietal fascial flap in reconstruction of the cranio-maxillofacial area.

An improved understanding of the vascular supply of the layers of the temporal fossa has increased the potentiality of reconstructive techniques involving this area. Each separate vascularized layer of this region can be used, yet the temporoparietal fascial flap is the more useful for treating a wide range of composite defects. It represents an ultra thin coverage which is useful in resurfacing exposed bone, tendons and vital structures such as nerves and vessels, in providing neovascularity as a recipient graft bed and in providing coverage or lining in major facial reconstructions. We descr

[Neonatal carcinoma. Review of the literature apropos of a case].

A cortical adrenal carcinoma arising during the neonatal period and associated with virilization recurred after an initial excision and is in complete remission after a follow-up period of 10 years. The authors have performed a review of the medical literature which has yielded 56 neonatal carcinomas including this one. More than half of these cases concerned the thyroid gland (15 cases) and the adrenal cortex (14 cases). These were followed by malignant melanomas (12 cases), carcinomas of the parotid gland (6 cases), oropharyngeal region (3 cases), adrenal medulla (2 cases) and stomach, liver

Postherpetic gustatory flushing and sweating.

An 11-year-old girl who had suffered right facial herpes zoster at the age of 6 years was left with anesthetic scars in the distribution of the third division of the trigeminal nerve. Since then, certain tastes provoked flushing and sweating localized to the scarred areas, lasting for 10 to 15 minutes after a latency of a few seconds. The response was evoked most readily from the ipsilateral posterior section of the tongue and was virtually abolished by local administration of anesthesia to the tongue. It remained unaltered after blockade of the sphenopalatine and stellate ganglia but was dimi

Diagnostic and Therapeutic Approach to Rare Tongue Lipoma.

Lipomas are benign soft tissue tumors composed of mature adipocytes, commonly found in subcutaneous tissues. Despite their prevalence in various body regions, they are relatively rare in the oral and maxillofacial regions. This study retrospectively analyzed the clinical and imaging characteristics, as well as the treatment outcomes of 57 patients diagnosed with lipoma. Among these, 42 cases (73.7%) were located in the oral-maxillofacial and head-neck regions, predominantly in areas with relatively abundant adipose tissue, including the facial/cheek area, parotid region, submental/submandibula…

Efficacy of elective nodal irradiation in skin squamous cell carcinoma of the face, ears, and scalp.

BACKGROUND: In patients at high risk for regional node metastasis from squamous cell carcinoma (SCC) of the skin of the face, ear, or scalp, radiotherapy to the regional nodes is an alternative to parotid or neck surgery. Data on the efficacy of elective nodal radiotherapy in this setting are scarce such that there is no publication specifically addressing the subject. The purpose of our study is to fill this void in the skin cancer literature. METHODS: This is a single-institution study of outcomes following elective nodal radiotherapy in 71 consecutively treated adults with SCC of the face,

Volume rejuvenation of the lower third, perioral, and jawline.

This is the third and final article discussing volumetric rejuvenation of the face. The previous two articles, Rejuvenation of the Upper Third and Management of the Middle Third, focused on the upper two-thirds of the face while this article focuses on the lower face, including the marionette area, jawline, and neck. Again, the authors of the previous two articles have provided a summary of rejuvenation utilizing a product of which they are considered an expert. Robert Glasgold has provided volumetric analysis of the region as an introduction.
