Atrial septal defect closure via mini-thoracotomy in pediatric patients: Postoperative analgesic effect of intercostal nerve block.

BACKGROUND: In this study, we evaluated the efficacy of intercostal nerve block for postoperative pain management in pediatric patients undergoing atrial septal defect closure through a right lateral mini-thoracotomy. METHODS: Between January 2016 and January 2019, a total of 63 pediatric patients (37 males, 26 females; mean age 34.8±26.8 months; range, 2 to 96 months) who underwent corrective congenital heart surgery for atrial septal defect closure through a right lateral mini-thoracotomy were retrospectively reviewed. The patients were divided into two groups as those (Group 1, n=33) recei

Treatment of Unstable Pediatric Tibia Shaft Fractures in Finland.

BACKGROUND: Surgical treatment of pediatric tibia shaft fractures has gained popularity despite closed reduction and cast-immobilization providing good long-term results. There is no consensus about optimal methods and satisfactory quality of treatment. MATERIALS AND METHODS: During 2010 to 2014, 226 pediatric patients were treated under anesthesia for tibia shaft fractures in Finland's five university hospitals. A total of 164 (73%) patients had closed fractures of the tibia or both tibia and fibula without other injuries (62 tibia only and 102 both tibia and fibula). Forty-one (18%) had ope

Perioperative analgesia with combined Pericapsular Nerve Group (PENG) and lateral femoral cutaneous nerve blocks in pediatric femoral neck surgery: a two-case report.

BACKGROUND: The importance of regional anesthesia techniques is increasing as part of multimodal analgesia strategies for managing postoperative pain in pediatric hip and femur surgeries. CASE PRESENTATION: We aim to present two cases, aged 10 years and 18 months, scheduled for surgery due to femoral neck fractures. As part of multimodal pain management, ultrasound-guided PENG + LFCN blocks were performed in both cases. Postoperative pain was assessed using VAS and FLACC scales. CONCLUSIONS: Our observations in these two cases suggest that the combined administration of PENG and LFCN blocks

Biodegradable Plates for Mandibular Fracture Fixation in Young Children.

Mandibular fractures are rare in children and normally result from high-impact trauma. Clinical protocols have evolved in recent years, especially after the introduction of rigid internal fixation methods and the development of novel resorbable materials. The purpose of this report is to present the case of a three-year-old toddler who fell from a bunk bed at home, resulting in a unilateral fracture of the mandibular parasymphysis and a joint fracture on the left side. The patient was treated under general anesthesia with rigid internal fixation with biodegradable plates and screws. Pediatric

Update on pediatric cataract surgery and intraocular lens implantation.

PURPOSE OF REVIEW: Cataract surgery is the most commonly performed intraocular surgery in the pediatric population worldwide. Although the basic surgical techniques have not significantly changed over the last several years, there are many related issues under continual debate and research. These include use of local perioperative anesthesia, capsular management, type and power of intraocular lenses (IOLs), sutured IOLs, and risk of subsequent glaucoma. This review will primarily focus on postsurgical outcomes and IOL-related controversies. RECENT FINDINGS: The just completed Infant Aphakia T

Pediatric wrist fractures: variations in management across countries. An evidence-based summary of evidence.

INTRODUCTION: Fractures of the distal radius are common in pediatric population, with considerable variation in the management of pediatric wrist fractures across different countries. It is crucial to consider the different approaches to conservative management and surgical intervention. The decision on the appropriate treatment method often depends on the type and severity of the fracture, as well as the available healthcare resources and expertise in each country. This article tries to identify these variations, so the various healthcare systems can work toward implementing best practices in

Use of anesthetic agents in neonates and young children.

BACKGROUND: Some drugs used for sedation and anesthesia produce histopathologic central nervous system changes in juvenile animal models. These observations have raised concerns regarding the use of these drugs in pediatric patients. We summarized the findings in developing animals and describe the steps that the Food and Drug Administration (FDA) and others are taking to assess potential risks in pediatric patients. The FDA views this communication as opening a dialog with the anesthesia community to address this issue. METHODS: We reviewed the available animal studies literature examining t

A Study of the Differences in Central Venous Pressure Between the Distal Lumen and Proximal Lumen of Central Venous Catheters in Pediatric Patients.

BACKGROUND: The choice of a central venous catheter (CVC) lumen to connect the central venous pressure (CVP) measurement line varies by facility. However, if the CVP values differ based on the connected CVC lumen, this variation could significantly affect the interpretation of the CVP measurements, raising major concerns regarding circulatory management of the patient. AIMS: This study aimed to determine whether a difference exists in the CVP values between the proximal and distal lumens of the CVC in small pediatric patients undergoing cardiac surgery. METHODS: Seventeen pediatric patients

Endoscopic ultrasound-guided drainage of pancreatic fluid collections is safe to perform in children: A comparison of two endoscopic approaches.

OBJECTIVES: Pancreatic fluid collections (PFCs), either pseudocysts or walled-off necrosis, are a significant cause of morbidity in pediatric patients with acute pancreatitis, leading to pain, obstruction, or infection. Endoscopic ultrasound (EUS)-guided drainage of symptomatic PFCs is the recommended therapy in adult patients and is accomplished using either lumen-apposed metal stents (LAMS) or double pigtail plastic stents (DPPS). Their comparative outcomes in children remain unstudied. METHODS: We performed a retrospective case series of all EUS-guided drainage of symptomatic PFCs among pe

Hybrid periventricular surgery for ventricular septal defects in children: a single-center large-cohort study in Kazakhstan.

BACKGROUND: The hybrid technique for ventricular septal defect (VSD) closure combines elements of conventional open-heart surgery with cardiopulmonary bypass (CPB) and minimally invasive approaches, including beating-heart occluder implantation through right ventricular free-wall puncture. Hybrid periventricular VSD closure offers several advantages over CPB-based surgery. Its less invasive nature, facilitated by catheter-based access to the defect, may reduce anesthesia duration and hospital stay in pediatric patients. Despite these potential benefits, the adoption of hybrid VSD correction re

A Single-Center Descriptive Analysis of Interventions for Hypotension in Children: A Retrospective Cohort Study.

BACKGROUND: Intraoperative hemodynamic management goals in pediatric patients vary significantly with age. Physiologic variability and the absence of a widely accepted definition of intraoperative hypotension contribute to inconsistent treatment thresholds and practice variation. AIMS: The primary aim was to observe which blood pressure thresholds in various age groups led to clinical intervention in noncardiac surgery. The secondary aim was to determine the most commonly employed treatment interventions. METHODS: A single-center, retrospective, observational cohort study analyzed anesthesia
