Periradicular repair after single- and two-visit root canal treatments using ultrasonic irrigant activation and calcium hydroxide dressing of teeth with apical periodontitis: study protocol for randomized controlled trials.

BACKGROUND: In certain clinical situations, root canal treatment in teeth with apical periodontitis is performed in multiple visits, with the use of intracanal dressing between visits, aiming to reduce microorganisms and their by-products of the root canal system prior to filling. However, in recent years, discussions have been growing about the real need for the use of intracanal dressing in these cases. The use of ultrasonic activation of the auxiliary chemical substance has increased the potential for decontamination promoted during the chemomechanical preparation of the root canal. Thus, t

The Relationship Between Severity of Apical Periodontitis and Next-Generation Systemic Inflammatory Biomarkers.

Apical periodontitis (AP) is an endodontic infection that may persist asymptomatically and be reflected in systemic inflammatory status. This study aimed to evaluate the association between radiographic severity of AP and next-generation systemic inflammatory biomarkers in systemically healthy adults. Archived clinical, radiographic and haematological data of 206 adult patients treated under general anaesthesia between 2021 and 2024 were retrospectively analysed. Patients were grouped according to Periapical Index (PAI) scores. Systemic immune-inflammation index (SII), neutrophil-to-lymphocyte

Intraosseous Adenoid Cystic Carcinoma Mimicking Endodontic Periapical Lesion Along With Orofacial Pain: A Case Report.

A 57-year-old patient with temporomandibular disorder and toothache underwent root canal treatments on teeth 42-31 by an endodontist. Despite these treatments, the patient experienced persistent pain radiating to his ears and sought evaluation by another endodontist. No notable facial or intraoral swelling nor paresthesia was observed; however, he reported difficulty with swallowing and trismus. A large periapical radiolucent lesion was identified spanning from tooth 42 to 32, and root resorptions were observed at the apices of teeth 42, 41 and 31. The initial diagnoses for teeth 42, 41, and 3

Dental and temporomandibular joint pathology of the North American river otter (Lontra canadensis).

Museum skull specimens from 165 North American river otters (Lontra canadensis) from Alaska, USA, were analysed systematically using an established protocol designed for examination of mammalian dentition. The study population was collected from 1956 to 2012 and comprised a similar proportion of males (n = 82, 49.7%) and females (n = 72, 43.6%). Eleven animals were of unknown sex. The study group incorporated a higher proportion of skeletally mature adults (n = 128, 78.5%) compared to young adults. As with many other mammalian species, the most common types of pathology in this species were pe

Ectopic sulcular pain: detection and treatment.

Patients who present with acute or chronic posterior dental pain but cannot identify the tooth from which the pain originates may suffer from a common but often unrecognized condition. The present article introduces a new term for this disorder, ectopic sulcular pain (ESP), derived from its unusual presentation, location, and defining symptom. It is tempting to call ESP an infection, but this has not been confirmed. In ESP, oral examination reveals no visual abnormalities, and there are no evident fractures, caries, periodontitis, attachment loss, traumatic occlusion, or periapical abscesses.

Upper anterior tooth mobility. Selected associations in untreated periodontitis.

The relationship between buccolingual tooth mobility (TM) and other clinical parameters was studied in 124 upper anterior teeth in 21 patients with untreated periodontitis. Periapical long-cone radiographs were available for 74 teeth in the same population. TM was measured with Mühlemann's macroperiodontometer. Probing depths, attachment levels and bleeding were measured with a constant force probe. Bone levels were estimated from radiographs. Presence of occlusal contact was noted in the intercuspal position and excursive movements of the mandible. Attachment levels showed the highest correla

Digitally Guided Modified Intentional Replantation for a Tooth with Hopeless Periodontal Prognosis: A Case Report.

Background and Clinical Significance: Advanced periodontitis with severe vertical bone loss and grade III mobility is usually managed by extraction and implant placement. Digital workflows and modern regenerative techniques have opened the possibility of preserving teeth that would traditionally be considered for extraction. This report describes a digitally guided modified intentional replantation (MIR) protocol applied to a maxillary tooth with severe periodontal involvement and unfavourable prognosis. Case Presentation: A 68-year-old male, non-smoker, with a history of heart transplantation

Histologic Findings of a Human Immature Revascularized/Regenerated Tooth with Symptomatic Irreversible Pulpitis.

INTRODUCTION: Pulp revascularization/regeneration in immature permanent teeth with necrotic pulp and/or apical periodontitis is an effective approach for inducing root maturation. Previous histologic studies showed cementoid/osteoid tissue and/or periodontal ligament-like tissue formed within the root canals. This case report describes the histologic findings of a human symptomatic irreversible pulpitis immature permanent tooth with most of the pulp removed after a revascularization/regeneration procedure. METHODS: A human immature permanent mandibular premolar (tooth #29) was diagnosed as sy

Pulpotec® Beyond Pulpotomy: A Case Series of Versatile Applications in Vital Pulp Therapy Procedures in Permanent Teeth.

Vital pulp therapy is a conservative approach aimed at preserving pulp vitality in teeth affected by deep caries or pulp exposure. Pulpotec® has been widely used as a pulpotomy medicament in primary teeth; however, its application in permanent teeth remains limited. This case series illustrates the use of Pulpotec® in five permanent molars that underwent indirect pulp capping, direct pulp capping, and pulpotomy. Clinical and radiographic evaluations were performed over a follow-up period of up to six months. All treated teeth remained asymptomatic with no tenderness, swelling, or signs of peri

Maxillary sinusitis of odontogenic origin.

Odontogenic etiology accounts for 10% to 12% of cases of maxillary sinusitis. Although uncommon, direct spread of dental infections into the maxillary sinus is possible due to the close relationship of the maxillary posterior teeth to the maxillary sinus. If a periapical dental infection or dental/oral surgery procedure violates the schneiderian membrane integrity, infection will likely spread into the sinus, leading to sinusitis. An odontogenic source should be considered in individuals with symptoms of maxillary sinusitis and a history of dental or jaw pain; dental infection; oral, periodont

Radiographic features of vertically fractured, endodontically treated maxillary premolars.

OBJECTIVE: The purpose of this study was to evaluate the most frequent radiographic appearance of bony lesions associated with vertically fractured roots of endodontically treated maxillary premolars. STUDY DESIGN: The radiographic features of 102 endodontically treated teeth and their periradicular areas (51 with and 51 without vertically fractured roots) were evaluated and compared. RESULTS: The predominant appearance of the periradicular area in the teeth with vertically fractured roots was the "halo" lesion (57%); by contrast, in the non-vertically fractured roots group, a "periapical" r

Histological response to titanium endodontic endosseous implants in dogs.

Endodontic endosseous implants stabilize teeth that have crown-root ratios compromised by periodontal disease, trauma, or apical resorption. By increasing the crown-root ratio, the implant improves the prognosis of the tooth, thus increasing its longevity. The purpose of this study was to evaluate, in vivo, the healing response to a newly introduced titanium endodontic implant. Eight implants were placed in the maxillary incisors and mandibular premolars of two adult beagle dogs after completion of root canal and osseous preparation. Peri-implant tissues were examined radiographically and hist

Partial pulpotomy in young permanent teeth with deep carious lesions.

The material comprised 37 young posterior teeth with deep carious lesions and exposed pulps, treated with partial pulpotomy and dressed with calcium hydroxide. The teeth were divided into two groups. Group 1 consisted of 31 teeth with no clinical or radiographic symptoms before treatment, Group 2 of 6 teeth with temporary pain, widened periodontal space periapically and/or productive osteitis, i.e. increased density of the surrounding alveolar bone. After an observation time of 24 to 140 months (mean = 56 months), healing had occurred in 29 of 31 teeth in Group 1 (93.5%) and in 4 of 6 teeth in

Periodontal lesions associated with deep traumatic overbite.

Certain aspects of malocclusion, particularly deep overbite, can be related to periodontal pathology, especially in the presence of poor oral hygiene. The authors have noted an association between deep overbite and unusual periodontal lesions. These lesions often appear on radiographs as circumscribed radiolucencies, remote from the alveolar crest and sometimes close to the apex. Gingival surface injury and food impaction may be important aetiological factors. This is illustrated in this article by a number of case studies.

Teeth loss after teriflunomide treatment: Casual or causal? A short case series.

BACKGROUND: Teriflunomide is a once-daily oral immunomodulator approved for the treatment of relapsing-remitting multiple sclerosis (RRMS) with a consistent safety profile in clinical trials. We report three cases of multiple teeth loss during teriflunomide treatment. CASE REPORTS: Case 1: a 39 year-old woman started teriflunomide for RRMS, switching from interferon beta. Four months later she complained about mandibular pain followed by the sudden loss of 4 teeth, in the absence of bleeding or trauma. Suspecting a causal role, we discontinued teriflunomide and started the accelerated elimina
