Digital Motherhood in the Postpartum Period: A Descriptive Study.

BACKGROUND: During the first 6 weeks after birth, women often encounter problems and seek answers to their questions. This period is also crucial in terms of technology use. AIM: The aim of this study was to examine the digital motherhood approach to self-care and infant care in the 6-week postpartum period, and its association with different variables. METHODS: This descriptive study recruited 278 women in the postpartum period. Data were collected from the Digital Motherhood Survey. Descriptive statistics and chi-squared analysis were performed. RESULTS: The mean age of the women was 28.9

Recommendations for the use of brain MRI in the neonatal period.

Magnetic resonance imaging (MRI) of the brain is a key tool in the diagnosis and prognosis of neurological conditions in newborn infants, especially when used along cranial ultrasound (cUS). Although its use has increased over the last decade, significant variations in protocols between centers persist. This document, developed by the Spanish Neonatal Brain Group in collaboration with pediatric neuroradiologists, establishes recommendations based on scientific evidence and clinical experience to standardize its use. The document describes the logistics of neonatal MRI, including patient prepar

Anxiety and Related Disorders During the Perinatal Period.

Anxiety and anxiety-related disorders are, as a group, the most common mental health conditions and are more common among women compared with among men. It is now evident that these disorders affect one in five pregnant and postpartum people and are more common than depression. For some disorders (e.g., obsessive-compulsive disorder), there is also evidence of an elevated risk for their development and exacerbation during perinatal periods. In this article, we review the literature pertaining to anxiety and anxiety-related disorders during the perinatal period. We also provide information rela

Rebound Hyperkalemia in Hypokalemic Thyrotoxic Periodic Paralysis.

Background: Thyrotoxic periodic paralysis (TPP) is a rare, life-threatening, reversible condition, in which episodic muscle weakness occurs due to low serum potassium in the setting of thyrotoxicosis. In addition to treating thyrotoxicosis, prompt restoration of potassium levels is essential while monitoring for rebound hyperkalemia. Purpose: To emphasize the importance of recognizing and appropriately managing TPP, with a focus on the potential dangers of potassium overcorrection. Case Report: We present a patient with TPP whose potassium replacement therapy caused dangerous hyperkalemia, req

Anxiety Sensitivity in the Perinatal Period: A Scoping Review.

BACKGROUND: Anxiety Sensitivity (AS), the trait-like tendency to interpret anxiety-related sensations as harmful, is a well-established risk factor for anxiety disorders and other mental health conditions. Less is known about the role of AS in perinatal mental health-encompassing pregnancy and the postpartum period-despite the heightened risk for anxiety and anxiety-related disorders such as Obsessive Compulsive Disorder and Posttraumatic Stress Disorder (PTSD). OBJECTIVE: The goal of this scoping review was to examine 1) the state of research on AS in perinatal populations, 2) its relationsh

The Incubation Periods of Mpox Virus Clade Ib.

BACKGROUND: Mpox virus (MPXV) clade Ib, first detected in the Democratic Republic of the Congo (DRC) in September 2023, spread internationally within months, prompting an emergency declaration from the World Health Organization. Data on its incubation period, which both shapes outbreak dynamics and informs epidemic response strategies, remain limited. OBJECTIVE: To estimate the incubation periods of mpox clade Ib, examining evidence for differences by route of exposure and demographic factors. DESIGN: Bayesian analysis of clinical surveillance data collected between June and October 2024. S

Application of the age-period-cohort model in tuberculosis.

Up to now, tuberculosis (TB) remains a global public health problem, posing a serious threat to human health. Traditional methods for analyzing time-varying trends, such as age and period, tend to ignore the poor impact of birth cohorts, which is an important factor in the development of TB. The age-period-cohort (APC) model, a statistical method widely used in recent decades in economics, sociology, and epidemiology, can quantitatively estimate the efficacy of different age, period, and birth cohort groups for TB by separating the effects of these three dimensions and controlling for confound

Dysautonomia and activity in the early stroke recovery period.

Maintaining cerebral perfusion in the early stages of recovery after stroke is paramount. Autoregulatory function may be impaired during this period leaving cerebral perfusion directly reliant on intravascular volume and blood pressure (BP) with increased risk for expanding cerebral infarction during periods of low BP and hemorrhagic transformation during BP elevations. We suspected that dysautonomia is common during the acute period related to both pre-existing vascular risk factors and potentially independent of such conditions. Thus, we sought to understand the state of the science specific

Striking the Balance: Bipolar Disorder in the Perinatal Period.

The authors reviewed the literature, published between 2018 and 2023, on treating bipolar disorder in the perinatal period in order to summarize current treatment perspectives. Mood episodes occur during pregnancy and there are high rates of both initial onset and recurrence in the postpartum period. Bipolar disorder itself is associated with higher risks of adverse pregnancy outcomes, including gestational hypertension, hemorrhage, cesarean delivery, and small for gestational age infants. A general principle of perinatal treatment includes maintaining psychiatric stability of the pregnant per

Coincident Thyrotoxic Hypokalemic Periodic Paralysis and Cardiomyopathy.

Thyrotoxic periodic paralysis (TPP) and thyrotoxic cardiomyopathy (TCMP) are potentially lethal complications of thyrotoxicosis that require emergent recognition and management to attenuate significant morbidity and mortality. We present the case of a 23-year-old Asian male with no prior medical history who developed TPP with coincident TCMP, which was successfully managed with antithyroid and heart failure therapies. The clinician should be aware of the diagnosis and treatment of these 2 life-threatening conditions in a hyperthyroid state.

Periodic Paralysis: A Case Series with a Literature Review.

INTRODUCTION: Periodic paralysis is a condition that causes recurrent episodes of flaccid paralysis, and it can be primary or secondary. Hypokalemic periodic paralysis is the most common type of primary periodic paralysis, and it is inherited through autosomal dominant gene transmission. Males are affected three times more often than females, and the paralysis attacks usually occur at night after a period of vigorous exercise. It is crucial to exclude other diagnostic entities based on the nature of presentation, physical examination, and paraclinical studies. Thyrotoxic periodic paralysis is

Hypokalemic periodic paralysis presenting as cardiac arrest.

Hypokalemic periodic paralysis (HPP) is a channelopathy with a genetic predisposition which may occur in the presence or absence of hyperthyroidism. We present a unique case of severe HPP associated with both Grave's thyrotoxicosis and pemphigus vulgaris. The patient developed proximal limb weakness after treatment with rituximab. He then subsequently presented with witnessed ventricular fibrillation arrest. Cardiopulmonary resuscitation was initiated in the field and return of spontaneous circulation was achieved. His initial potassium was 1.9 mmol/L with a marked metabolic acidosis.

Generalised linear model of periodic limb movements.

During sleep, the lower extremities exhibit periodic repetitive movements which are referred to as Period Limb Movement (PLM). Polysomnography (PSG) is the gold standard for diagnosing periodic limb movement disorder. The frequency of PLM episodes per hour of sleep (PLMI) determines the severity of the condition. PLM are generated by a dynamic process, however PLMI measures only the average PLM rate and does not capture the dynamic properties of PLM. Here, we characterise PLM dynamics using a generalised dynamic model as a function of sleep stage, timing of past PLM events and adjacent sleep d

Hyperkalemic periodic paralysis with paramyotonia and the anaesthetic implications.

Hyperkalemic periodic paralysis (HyperKPP) is a rare disease with significant anaesthetic implications. We compare two perioperative courses in the same patient. The first surgery resulted in paralysis and a protracted hospitalisation, while the second surgery resulted in a same-day discharge. Various anaesthetic techniques may be used; however, clear communication surrounding optimisation both for home medications (eg, continuing potassium wasting diuretics) and avoidance of triggering medications (primarily: depolarising neuromuscular blockers), along with thermoregulation and glucose manage

Thyrotoxic Periodic Paralysis With Hypokalemia: A Case Study.

Thyrotoxic periodic paralysis (TPP) is a rare life-threatening condition most commonly seen in individuals between the ages of 20-40 years. It is most prevalent in Hispanic and Asian populations. Here we present a case report of a young male patient admitted to our facility with an acute onset of paralysis. He was found to have new-onset hyperthyroidism and severe hypokalemia. TPP was exacerbated by the intake of a high-carbohydrate meal as well as a steroid injection within 24 hours of symptom onset.

Thyrotoxic periodic paralysis presenting with quadriparesis and hyperreflexia.

Thyrotoxic periodic paralysis (TPP) is a rare complication of hyperthyroidism that manifests as painless flaccid paralysis. An East Asian man in his late 20s presented to the emergency department with an acute onset of quadriparesis associated with hypertonia and hyperreflexia. His initial symptoms and signs suggested involvement of the brain and spinal cord; however, MRI of the neuroaxis was normal. His serum potassium concentration was low, and thyroid test results were consistent with hyperthyroidism. The patient was diagnosed with TPP associated with Graves' disease and was treated with po

Ethics of a Mandatory Waiting Period for Female Sterilization.

Due to a history of coerced sterilization, a federal Medicaid sterilization policy mandates that a specific consent form be signed by a patient at least thirty days prior to when the patient undergoes sterilization. However, in contemporary obstetrical practice, the Medicaid sterilization policy serves as a policy-level barrier to autonomously desired care. We review the clinical and ethical implications of the current Medicaid sterilization policy. After discussing the utility and impact of waiting periods for other surgical procedures, we explore the psychology of time required for decision-
