The challenge of post-operative peritonitis after gastrointestinal surgery.

Post-operative peritonitis (PP) is a life-threatening hospital-acquired intra-abdominal infection with high rates of mortality. Diffuse PP remains a challenge for surgeons. Prognosis and outcome of patients are directly related to early diagnosis and prompt intervention. The diagnosis of PP may be difficult because there are no specific clinical signs and laboratory tests to reject or confirm the diagnosis. Atypical clinical features may be responsible for a delay in reoperation. An early diagnosis and prompt treatment is crucial to prevent the development of organ failure and improve the outc…

[Biliary peritonitis after traumatic rupture of a choledochal cyst].

Choledochal cysts are rare congenital malformations of the biliary tract. Traumatic rupture of a choledochal cyst can be misleading. An 11-year-old boy was admitted for peritonitis and intestinal occlusion after blunt abdominal trauma, evolving over 48 h. Laparotomy revealed bile ascites and a suspected duodenal perforation. After referral to our center, a CT scan showed a perforated choledochal cyst. Six months later, a complete excision of the cyst was successfully performed. This treatment is mandatory because of the risk of further complications such as lithiasis, pancreatitis, cholangitis…

Jejunal diverticulum enterolith causing perforation and upper abdominal peritonitis.

A patient presented with a 4 h history of acute onset, progressive upper abdominal pain. There was localised peritonitis, with raised inflammatory markers and lactate. CT scan showed a large calcified mass, with evidence of mesenteric twist/volvulus causing some degree of small bowel obstruction. At laparotomy, there were multiple jejunal diverticula, one of which had perforated due to a large enterolith. Resection of the affected jejunum and washout was performed and the patient recovered well. Complications of jejunal diverticula and enteroliths are reported and should be considered in patie…

[The nature of postoperative complications in patients with peritonitis].

We studied the postoperative period in patients with peritonitis. The structure of the most important factors that slow down the healing process and lead to mortality. Among the factors that affect the healing process is the most important character of fluid, and the prevalence of peritonitis (causative factor), which causes complications on the part of the internal organs and wounds.

An unusual cause of peritonitis in a deployed environment.

Acute abdominal pain is a common presenting complaint to both primary and secondary care, and is a frequent cause of hospital admission among deployed personnel. Identification of generalised peritonism on abdominal examination is a classical indicator of intra-abdominal pathology that may warrant exploratory laparotomy. Negative findings at laparotomy should serve as a diagnostic prompt to consider other non-surgical mimics of an acute abdomen.

Perforated Meckel's Diverticulum Lithiasis: An Unusual Cause of Peritonitis.

Meckel's diverticulum is the commonest congenital malformation of gastrointestinal tract and represents a persistent remnant of the omphalomesenteric duct. Although it mostly remains silent, it can present as bleeding, perforation, intestinal obstruction, intussusception, and tumours. These complications, especially bleeding, tend to be more common in the paediatric group and intestinal obstruction in adults. Stone formation (lithiasis) in Meckel's diverticulum is rare. We report a case of Meckel's diverticulum lithiasis which presented as an acute abdomen in an otherwise healthy individual. T

Disseminated congenital tuberculosis presenting as peritonitis in an infant.

Congenital tuberculosis is a rare disease which is curable after early diagnosis. Making an early diagnosis of tuberculosis in an infant is difficult, however, because of its nonspecific clinical findings. It therefore requires a high index of suspicion. In this paper we report the case of an infant with disseminated congenital tuberculosis involving the chest, liver, spleen, abdominal lymph nodes, mesentery, and peritoneum, and which presented as chylous ascites with peritonitis.

Spontaneous bacterial peritonitis by Brucella in a cirrhotic patient.

Spontaneous bacterial peritonitis (SBP) is a frequent form of decompensation of end-stage liver disease, with an incidence of 15-20% and a short-term mortality of 10-33%. The usual causative agents (90% of SBP) are enteric Gram-negative bacteria-Escherichia coli and Klebsiella pneumoniae. Brucella is known to be a possible, but exceedingly rare, causative agent of SBP. We present the case of a 47-year-old Egyptian man, with hepatitis C cirrhosis, and a 2 week history of ascites, jaundice, encephalopathy, fever and pain on his right shoulder that started while travelling in the Middle East. Lab…

[Pericardial effusion in a cat with feline infectious peritonitis].

This case report describes the disease progression of a male cat with pericardial effusion. Clinical signs (dyspnea, lethargy, and weakness) started very acutely. The initial laboratory profile showed only an increase in alanine aminotransferase enzyme activity. Diagnostic imaging revealed pericardial effusion. Effusion analysis showed a Rivalta-positive, modified transudate. Detection of feline coronavirus antigen in macrophages was negative. General condition and laboratory parameters dramatically worsened within seven days. Therefore, the owners decided to euthanize the cat. Even if effusio

Post-coital posterior fornix perforation with peritonitis and haemoperitoneum.

INTRODUCTION: Vaginal injuries are common outside of obstetric practice. Post coital posterior fornix perforation and intra-abdominal bleeding is however an uncommon cause for laparotomy. PRESENTATION OF CASE: We present two cases of posterior fornix perforation with hypovolemic shock after sexual intercourse in two young women. In both cases there was a delay in the diagnosis because there was illicit sex. Both women however eventually had laparotomy and uneventful post-operative outcomes. DISCUSSION: This is an uncommon condition but it is important to suspect it in sexually active women.

[Fallopian tube prolapse and Chlamydia peritonitis after vaginal hysterectomy].

Fallopian tube prolapse and tuboovarian abscess are both uncommon complications to hysterectomy. A 42 year-old woman who had undergone vaginal hysterectomy two years prior, presented with pain, fever, and tender polyp-like mass in the vaginal vault and pelvic mass. Surgery revealed a tuboovarian abscess. A histological examination of the mass excised from the vaginal vault confirmed presence of fallopian tube epithelium. A polymerase chain reaction analysis of the peritoneal fluid revealed the presence of Chlamydia.

Listeria monocytogenes peritonitis: presentation, clinical features, treatment, and outcome.

BACKGROUND: Spontaneous bacterial peritonitis (SBP) is a serious complication in cirrhotic patients. Gram (-) (E. coli, Klebsiella pneumoniae), and Gram (+) (Streptococci, Staphylococci) bacteria are most frequently cultured from patients'ascites. Listeria monocytogenes (Lm) is scarcely reported as a causative agent. OBJECTIVE: Our objective is to describe Lm peritonitis as a clinical entity, including its presentation, clinical features, treatment, and the potential factors that might affect survival outcome. DATA SOURCES: MEDLINE, Scholar.Google, Scopus databases, including English, Spanis…

[Optimization of ascitic fluid culture in spontaneous bacterial peritonitis].

INTRODUCTION AND AIMS: Spontaneous bacterial peritonitis (SBP) is ascitic fluid (AF) infection in the absence of an intraperitoneal source of infection in patients with liver disease and portal hypertension. The aim of this study was to evaluate the yield of microbiological cultures to optimize their collection and systematic implementation in routine clinical practice. METHODS: We analyzed two cohorts: the first consisted of retrospective data from the clinical records of 156 consecutive episodes of SBP from January 2003 to December 2005 (group R), and the second was composed of data collect…

Radiology of vernix caseosa peritonitis: case report and discussion.

Vernix caseosa peritonitis is a rare complication of pregnancy that can occur following Caesarean section. If fetal vernix enters the peritoneal cavity, it may incite an acute inflammatory response leading to peritonitis. This diagnosis should be considered when patients present with an acute abdomen pain following Caesarean section. Radiological imaging may identify lesions in the abdominal wall wound or peritoneal cavity, and the diagnosis can be made by image-guided biopsy.
