Pathology versus statistical infrequency: potential sources of gender bias in personality disorder criteria.

The antisocial, narcissistic, dependent, histrionic, and borderline personality disorders often obtain differential sex prevalence rates. One explanation has been that the diagnostic criteria for these personality disorders have different gender implications for maladaptivity (e.g., perhaps the dependent personality disorder diagnostic criteria are considered by clinicians to be more pathological for women than for men). This hypothesis was explored in two studies that obtained judgments by professional clinicians of the maladaptivity and statistical infrequency of personality disorder diagnos

A psychiatric appraisal of "glossodynia".

"Glossodynia" is a diagnostic rubric that includes patients enduring pain or a burning sensation in the tongue without any oral pathology or systemic disease. Psychiatric profiles of 50 women patients with "glossodynia" were compared with those of control subjects. The patients scored significantly lower on the Extraversion Scale of the Eysenck Personality Questionnaire and higher on the Toronto Alexithymia Scale than the control subjects, whereas there was no significant difference in the mean General Health Questionnaire score. The psychopathology of "glossodynia" may be more markedly associ

Variables affecting the decision making of a review panel.

This study was designed to identify the variables that influence a review panel's decision to discharge or detain an involuntary patient. The variable set included information on the patient's psychiatric history, current hospitalization and treatment as well as ratings of dangerousness, insight and psychopathology, as reflected in the attending physician's case summary p

Somatized psychiatric disorder presenting as palpitations.

BACKGROUND: Psychiatric disorder is underdiagnosed in primary care practice, often because it is somatized and the patient reports only physical symptoms. Palpitations are among the symptoms that often are somatized. METHODS: We studied prospectively 125 consecutive medical outpatients referred for ambulatory electrocardiographic monitoring to evaluate a chief complaint of palpitations. They completed an in-person research interview at the time of monitoring and a telephone follow-up interview 3 months later. The referring physicians completed questionnaires about their patients before receiv

The subjects were 22 Japanese female patients with both bulimia nervosa and alcoholism; the comparison group comprised 22 age-matched female patients with bulimia nervosa but without alcoholism. Patients with both bulimia and alcoholism had more borderline personality disorders and pathological symptoms such as stealing, suicide attempts, and liver injuries than the nonalcoholic comparison

Evidence for developmentally based diagnoses of oppositional defiant disorder and conduct disorder.

This paper compares the validity of DSM-III-R diagnoses of oppositional defiant disorder (ODD) and conduct disorder (CD) and an alternative option which is subdivided into three levels according to developmental sequence and severity: modified oppositional disorder (MODD), intermediate CD (ICD), and advanced CD (ACD). Using a sample of 177 boys followed over 3 years, both the DSM-III-R and the alternative diagnostic constructs are evaluated on three criteria: symptom discriminative validity, and diagnostic external and predictive validity. Most DSM-III-R ODD and CD symptoms discriminated betwe

[Catatonic syndrome].

Catatonia has been more studied in the past 10 years after a period of declining interest. This is due, on the one hand, to its inclusion in major psychiatric nosographic systems, and on the other hand, to the possibility to treat efficiently this condition, which was previously a fatal one. Catatonia is characterized by concurrent motor, emotional, and behavioural symptoms. This syndrome is commonly observed in persons with mood disorders, and less frequently in schizophrenia as well as in numerous neurological affections. A central dopaminergic dysfunction could explain the its apparition Ca

Neuroendocrine investigation of depression in mentally retarded patients. A pilot study.

As part of a pilot study, eight adults with severe mental retardation were screened for the presence of endogenous depression by the standard 1-mg overnight dexamethasone suppression test (DST). The eight had been rigorously screened to rule out potential for false-positive DST. Two of the eight patients demonstrated nonsuppression, and a third had borderline results. Of these three, two manifested behavioral disturbances that could be attributed to major depressive disorder. The authors discuss the results, the difficulty diagnosing psychiatric disorders in severely retarded persons, and the

Bipolar disorder presenting as stalking--a case report.

Psychiatric disorders may underlie some stalking cases, but no reports exist of the relationship of actual psychiatric status with the expression of stalking behaviour. CASE REPORT: A 22-year-old adoptive woman perpetrated stalking towards her gynaecologist, who took legal action to protect herself. She was admitted to a general hospital psychiatric department and diagnosed with bipolar disorder-I, manic phase, and personality disorder, not otherwise s

This study sought to understand the experiences of peer specialists with criminal justice histories and how they incorporate these experiences into their work. METHODS: Purposiv
