Imatinib mesylate (STI571) for myeloid malignancies other than CML.

Imatinib mesylate, a small molecule tyrosine kinase inhibitor, has had a major impact on the treatment of Philadelphia chromosome positive chronic myelogenous leukemia. This review will explore its potential in the treatment of other myeloid neoplasms, based on its ability to inhibit Kit and PDGFR kinases in addition to Bcr-Abl. Imatinib's potential role in the treatment of Philadelphia chromosome negative chronic myelogenous leukemia, systemic mastocytosis with associated hematologic neoplasms, chronic myelomonocytic leukemia, specific subtypes of acute myelogenous leukemia, myelofibrosis/mye…

Re-Member MOVE: The Anatomy of a Reconciliation.

On May 13, 1985, Philadelphia city officials authorized the dropping of a bomb on a house in a residential neighborhood, killing 11 people of the MOVE organization-including five children-destroying 61 homes and rendering nearly 250 residents homeless. The years leading up to this tragedy involved multiple instances of police brutality, one of which resulted in the more than 40-year incarceration of nine MOVE members (commonly known as the MOVE 9). In this first-person account, we review literature on reconciliation and apologies, explore the value of reconciliation in community psychology, an…

Chronic myelogenous leukemia with the e6a2 BCR-ABL and lacking imatinib response: presentation of two cases.

The BCR-ABL fusion gene represents the hallmark of chronic myelogenous leukemia (CML) and is derived from a translocation between chromosome 9 and 22. The majority of CML patients have a breakpoint in the major BCR region of the BCR gene giving rise to e13a2 or e14a2 BCR-ABL transcripts. Occasionally, other BCR breakpoints occur. The current report describes two e6a2 CML patients with imatinib treatment failure and unusual disease progression. One patient was Philadelphia chromosome positive and one was Philadelphia chromosome negative with an atypical BCR-ABL rearrangement, ins (22;9).

Association of familial leukemia with HLA Cw3: is it real?

A kindred with familial leukemia is described in which three members are affected by leukemia in three consecutive generations. One of these members had Philadelphia positive chronic myeloid leukemia (CML) and other two had precursor B acute lymphoblasticleukemia (ALL)--Philadelphia negative. All three of them shared a common haplotype A2, B55, Cw3. In addition, another member sharing same haplotype developed metastatic breast cancer. Phenomenon of anticipation is demonstrated in the affected family members. Possible causes of genetic susceptibility to leukemia especially the association of Cw…

Xylazine spreads across the US: A growing component of the increasingly synthetic and polysubstance overdose crisis.

BACKGROUND: Sharp exacerbations of the US overdose crisis are linked to polysubstance use of synthetic compounds. Xylazine is a veterinary tranquilizer, long noted in the street opioid supply of Puerto Rico, and more recently Philadelphia. Yet its national trends, geographic distribution, and health risks are poorly characterized. METHODS: In this sequential mixed-methods study, xylazine was increasingly observed by ethnographers in Philadelphia among drug-sellers and people who inject drugs (PWID). Subsequently, we systematically searched for records describing xylazine-present overdose mort…

Rapid Assessment Medical Support (RAMS) for active shooter incidents.

This country has witnessed a steady increase in the number of active shooter incidents in recent years. The traditional emergency medical services (EMS) response to such incidents has been to stage at a safe distance until the scene has been secured by law enforcement. Such an approach may lead to unnecessary delays in medical care and potentially needless loss of life. To address this issue locally, the Philadelphia Fire Department (PFD) and the Philadelphia Police Department (PPD) collaborated to develop the Rapid Assessment Medical Support (RAMS) program. All PFD paramedics have been equipp…

A case of myelodysplastic syndrome developed blastic crisis of chronic myelogenous leukemia with acquisition of major BCR/ABL.

We describe a rare case of myelodysplastic syndrome that developed chronic myelogeneous leukemia with acquisition of Philadelphia chromosome. The major BCR/ABL transcript was confirmed by molecular analysis. Shortly thereafter, the patient showed transformation to blastic crisis. Hematological remission was achieved after 3 months of treatment with imatinib mesylate. The patient relapsed with additional chromosomal abnormalities and the disease became refractory to the treatment. Acquisition of the Philadelphia chromosome is an infrequent event in myelodysplastic syndrome, and the addition of …

Molecular response to imatinib in patient with Ph negative p190 BCR-ABL transcript positive chronic myeloid leukemia with cyclic leukocytosis.

An atypical case of Philadelphia (Ph) negative, e1a2 BCR-ABL transcript positive chronic myeloid leukemia (CML) characterized with cyclic periodic leukocytosis and spontaneous remissions is reported. The patient was treated with imatinib and good hematology response with molecular remission was achieved. So far, only few Ph positive CML patients expressing p190 BCR-ABL protein and different clinical characteristics and treatment have been described in the literature. This is the first report of Philadelphia negative, p190 BCR-ABL positive CML with cyclic spontaneous oscillation of white blood …

Analysis of bcr rearrangements in primary (essential) thrombocythaemia.

A clinical and molecular study was performed on 10 patients with the presumptive diagnosis of primary thrombocythaemia (PTH). In all cases conspicuous megakaryocytic proliferation in bone marrow smears was detected in the absence of the Philadelphia chromosome (Ph1), elevated red blood cell mass and evidence for a reactive cause of the thrombocytosis. We analyzed whether rearrangements of the bcr gene occurred in untreated patients. Bcr rearrangements were detected in the bone marrow cells but not in peripheral blood bells of one patient, indicating that this case might be related at the molec…

Two Cases of Severe Cytomegalovirus Disease in Nontransplant Pediatric Acute Lymphoblastic Leukemia Patients Treated With Tyrosine Kinase Inhibitors.

BACKGROUND: Cytomegalovirus (CMV) disease is a rare but significant complication among nontransplant pediatric leukemia patients. CMV infection has been observed in patients receiving tyrosine kinase inhibitors (TKIs) indicated for the treatment of chronic myeloid leukemia (CML), Philadelphia chromosome-positive (Ph+), and Philadelphia chromosome-like (Ph-like) acute lymphoblastic leukemia (ALL). However, there is limited published data regarding CMV disease in nontransplant leukemia patients treated with TKIs. OBSERVATIONS: We present 2 cases of severe CMV disease in Ph+ and Ph-like ALL pati…

Chronic myeloid leukemia patient with co-occurrence of BCR-ABL junction and JAK2 V617F mutation.

The JAK2 V617F mutation is common in patients with Philadelphia-negative chronic myeloproliferative neoplasms, but few cases of the JAK2 V617F mutation have been described in Philadelphia-positive chronic myeloid leukemia (CML) patients. Here, we report a 21-year-old female who presented with phenotype of CML in whom BCR-ABL transcript and JAK2V617F mutation co-occurred. These findings were determined through cytogenetic analysis, fluorescence in situ hybridization, and allele-specific (AS) PCR. The patient's BCR-ABL transcript disappeared after 6 months of treatment with imatinib, while the J…

Clonal cytogenetic abnormalities after tyrosine kinase inhibitor therapy in Ph+ ALL resolution after decitabine therapy.

Mixed chimerism and presence of minimal residual disease after stem cell transplantation (SCT) usually predict leukemia recurrence. In Philadelphia chromosome positive (Ph+) leukemia tyrosine kinase inhibitors can restore remission. These agents can induce clonal cytogenetic abnormalities in the Philadelphia negative cell population (CCA/Ph-), which may rarely progress to acute myeloid leukemia. A child with Ph+ acute lymphoblastic leukemia showed mixed donor chimerism and persistent bcr-abl transcripts after a matched sibling SCT. There was no response to tyrosine kinase inhibitor (TKI) thera…

Outpatient echocardiography in the evaluation of innocent murmurs in children: utilisation benchmarking.

OBJECTIVES: We sought to benchmark the utilisation of echocardiography in the outpatient evaluation of heart murmurs by evaluating two large paediatric cardiology centres. BACKGROUND: Although criteria exist for appropriate use of echocardiography, there are no benchmarking data demonstrating its utilisation. METHODS: We performed a retrospective cohort study of outpatients aged between 0 and 18 years at the Sibley Heart Center Cardiology and the Children's Hospital of Philadelphia Division of Cardiology, given a sole diagnosis of "innocent murmur" from 1 July, 2007 to 31 October, 2010. JAK2 kinase inhibitors and myeloproliferative disorders.

PURPOSE OF REVIEW: The pathophysiology of Philadelphia-chromosome negative myeloproliferative disorders has significantly advanced with the discovery of JAK2V617F. The prevalence of JAK2V617F mutation has made it a much anticipated target for inhibition; this review will update and assess progress. RECENT FINDINGS: Many agents have been studied in preclinical trials, of which few have entered clinical trials. Data from the clinical trials are limited and mostly in the form of abstracts and reviews. SUMMARY: The prevalence of the JAK2V617F mutation in the classic Philadelphia-chromosome negat…

The Dercum-Muybridge collaboration for sequential photography of neurologic disorders.

OBJECTIVE: To analyze the contributions of American photographer Eadweard Muybridge (1830-1904) and Philadelphia neurologist Francis Dercum (1856-1931) toward creating the first motion-picture sequences of patients with neurologic disorders. BACKGROUND: In the late 1870s and 1880s, prior to the development of movie cameras or projectors, Muybridge photographed sequential images of people and animals in motion, using arrays of sequentially triggered single-image cameras and multilens cameras. METHODS: Examination of published writings and photographic sequences by Muybridge and Dercum, and pr…

Regional differences in maturation of germ cells of cryptorchid testes: role of environment.

OBJECTIVE: To investigate differences in maturation of germ cells in cryptorchid testes in three different regions. PATIENTS AND METHODS: A total of 103 consecutive patients were operated for unilateral undescended testis in Vojvodina, from March 2006 until September 2007, and had a testicular biopsy performed. Germ cells were counted, and the presence of Ad spermatogonia was noted. Biopsies were compared to biopsies of similar patients from two different regions: Philadelphia, USA (130), and Liestal, Switzerland (55 patients). RESULTS: In Vojvodina, 84.5% of patients had Sertoli cells only,…

Do laws bring children in child care centers up to date for immunizations?

BACKGROUND: Pennsylvania state law requires licensed child care centers (CCCs) to document that each enrolled child is up to date (UTD) for routine immunizations within 60 days of enrollment. This study evaluates the law's impact on immunization coverage among children aged <or=59 months who attend CCCs in Philadelphia. METHODS: Out of Philadelphia's 440 commercial CCCs, 75 were randomly selected. Of these, 9 had closed, 3 did not accept children aged <or=59 months, and 3 refused assessment. For the remaining 60 CCCs, vaccination dates were abstracted from CCC records for all enrolled childre…

Ph-negative chronic myeloid leukaemia.

An analysis of five patients with Philadelphia chromosome (Ph) negative chronic myeloid leukaemia (CML) revealed that two were clinically and haematologically identical to Ph-positive CML whereas three should be reclassified as chronic myelomonocytic leukaemia (CMML). At a molecular level the first two patients showed the same juxtaposition of c-abl and bcr genes as is seen in Ph-positive CML. These genomic changes were not seen in the other three patients. Observations on these five patients suggest that the clinical course and prognosis of the rare patient who carries the Ph 'molecular defec…
