The utility of immunohistochemistry and in situ hybridization in placental pathology.

It has long been recognized that routine histologic examination of the placenta has limitations, especially with regard to the diagnosis of infectious diseases and the concomitant cytokine response that may cause severe in utero fetal damage. Immunohistochemical testing of the placenta in such situations can be very useful in terms of identifying the infectious agent as well as in demonstrating a marked increase in cytokines such as tumor necrosis factor alpha and interleukin 8, produced primarily by cells native to the villi and fetal membranes. One hundred placentas (20 normal childbirths, 2

Placental transfer of antibiotics administered to the mother: a review.

BACKGROUND: The purpose of antibiotic treatment in pregnant women is to treat the mother and/or the fetus since it is known that antibiotics administered to the mother cross the placenta and reach the fetus. A comparison of the drug concentration in maternal and fetal plasma gives an indication of the exposure of the fetus to the maternally administered antibiotics. AIM: The aim of this study was to review the literature pertaining to the placental transfer of antibiotics in man and to classify the antibiotics according to the type of transfer involved. A table has been developed for use by p

Placental Doppler velocimetry in gestational diabetes mellitus.

OBJECTIVE: To evaluate if maternal glucose level and growth of the fetus were related to placental vascular impedance in pregnancy complicated by gestational diabetes mellitus. MATERIAL AND METHODS: A retrospective study of 146 gestational diabetic women of which 117 needed insulin therapy. Glycosylated hemoglobin (HbA1c) was evaluated as well as umbilical and uterine artery Doppler velocimetry. The results were related to adverse outcome of pregnancy including newborn birthweight. RESULTS: Abnormal umbilical artery blood flow velocity was seen in 5% of the cases and abnormal uterine artery

[Cystic degeneration of placental subchorionic intervillous thrombi].

This is a case study of a 31 year-old woman who presented for a routine follow-up examination at 33 weeks of gestation. A 9-cm intrauterine membranous cystic structure, floating in the amniotic fluid, with 3 internal round structures attached on a stalk in close proximity to the placental insertion of the umbilical cord was demonstrated in sonography. A color Doppler study of the cyst was normal and also revealed a normal systolic/diastolic flow ratio in the umbilical arteries. The cyst was not reported on extended sonographic scans at 15 weeks and 24 weeks gestation. The patient delivered spo

Prenatal diagnosis of placental chorioangioma: our experience.

Placental chorioangioma is the most common benign tumor of placenta. The relationship of vascularized chorioangiomas to adverse pregnancy outcome is well recognized. We report 3 cases of placental chorioangioma. Hypervascularization of the lesions in all patients and the immune hydrops with adverse fetal outcome in 2 cases are the complications of our mini-series. Ultrasonography and Doppler ultrasonography findings were useful in establishing the prenatal diagnosis and the prognosis.

Serum and placental interleukin-18 are elevated in preeclampsia.

OBJECTIVES: Interleukin-18 (IL-18) is a proinflammatory cytokine with pleiotrophic qualities and its roles in pregnancy, labor onset and pregnant complications have been proposed. However, the alterations of serum and placental IL-18 have been less investigated. The objective of the current investigation was to detect IL-18 concentrations in serum and placentas from patients with preeclampsia and women with normal pregnancy. METHODS: 27 patients with preeclampsia and 28 women with normal pregnancy were recruited. Blood and placental samples were taken and IL-18 concentrations in serum and pla

Placental teratocarcinoma in a mare with possible metastasis to the foal.

A teratocarcinoma was diagnosed in the amnion of a 5-year-old Arabian mare that delivered a healthy, full-term foal. The foal died at 2.5 months of age as a result of metastasis of an undifferentiated component of the mass. This case is unique because it is the first reported case of placental teratocarcinoma in animals and the malignant component apparently metastasized to the foal resulting in its death.

Bromodichloromethane inhibits human placental trophoblast differentiation.

Epidemiological data suggest an association between exposures to bromodichloromethane (BDCM), a trihalomethane found in drinking water as a result of drinking water disinfection, and an increased risk of spontaneous abortion. We previously hypothesized that BDCM targets the placenta and showed that the secretion of chorionic gonadotrophin (CG) was reduced in primary cultures of human term syncytiotrophoblasts exposed to BDCM. In the present study we extend this observation by evaluating the effects of BDCM on the morphological differentiation of mononucleated cytotrophoblast cells to multinucl

Placental calcium pump: clinical-based evidence.

Placenta can be considered as a pump of calcium necessary for the normal development of the fetus. We believe that the location of this pump is in the placental basement membrane. The calcification of this membrane has been described only in cases of in utero fetal death. In this study we describe for the first time a case of placental calcification in a living fetus. The fetus of a normal 21-year-old pregnant woman showed heart abnormalities but the genetic analysis showed a normal male karyotype. The histology of the placenta demonstrated multiple intravillous linear and granular calcific in

Three-dimensional sonographic features of placental abnormalities.

OBJECTIVE: To describe three-dimensional (3D) sonographic features of placental abnormalities in utero. METHODS: A total of 6 cases with placental abnormalities (1 chorioangioma, 1 subchorionic hematoma, 2 placental cysts, and 2 subplacental fibromas) from 22 to 34 weeks of gestation were studied with transabdominal 3D sonography. RESULTS: Placental abnormalities could be easily imaged with both two-dimensional (2D) and 3D sonography; however, visualization of the continuity and curvature of structures was more easily accomplished with 3D sonography. In the case with chorioangioma, the thick

[Is thrombophilia a risk factor for placental vascular disorders?].

First trimester recurrent fetal loss and severe second or third trimester placental vascular disorders are still frequently unexplained. Acquired biological thrombophilia (antiphospholipid syndrome, essential thrombocythemia) are known risk factors. Genetic thrombophilia may represent a new risk factor for placental vascular diseases, but reported data are conflicting. Combined hereditary thrombophilias are indeed a moderate risk factor, but even in these cases most pregnancies are normal giving healthy live birth children. Whether hereditary thrombophilia is associated with recurrent severe p

[Confined placental mosaicism: definition, consequences and outcome].

Confined placental mosaicism (CPM) occurs in approximately 2 per cent of viable pregnancies studies by chorionic villus sampling. The great majority of pregnancies with confined placental mosaicism proceed uneventfully, resulting in normal liveborn infants. However, a few specific chromosomal abnormalities have been associated with fetal growth retardation. Fetal karyotype should be checked by amniocentesis and such pregnancies should have careful ultrasound follow-up.

Placental development in normal and compromised pregnancies-- a review.

Intrauterine growth restriction (IUGR) is a significant cause of infant mortality and morbidity. It is now clear that IUGR infants exhibit higher rates of coronary heart disease, type 2-diabetes, hypertension and stroke as adults. Therefore, fetal growth not only impacts the outcome of the perinatal period, but also impacts adult well-being. The etiologies of IUGR are numerous, but are often associated with abnormalities in placental structure and function. The process of implantation and placentation requires the production of a plethora of growth factors, cell-adhesion molecules, extracellul…

Infantile hemangiomas: speculation on placental trophoblastic origin.

BACKGROUND: The unique immunobiology of the placental trophoblast and the increased incidence of hemangiomas in infants born after chorionic villus sampling suggest that an immunologically regulated ectopic focus of trophoblasts could be the cell of origin for proliferative infantile hemangiomas. OBJECTIVE: To compare tissue from infantile hemangiomas with that of other vascular lesions for the presence of selected placental trophoblast-specific cellular markers. DESIGN AND PATIENTS: Twelve tissue specimens taken from infantile hemangiomas on patients aged 5 days to 2 years were retrospectiv

Placental lesions associated with abnormal growth in twins.

Abnormal growth in one or both twins may contribute to the increased morbidity and mortality observed in twin gestation. Our objective in this retrospective study of all twin pregnancies delivered at our hospital (n = 240) over a 2-year period was to study the relationship between placental lesions and abnormal growth. Standardized placental examinations were performed in 192 cases (80%), which constituted the study population. Two growth abnormalities were studied: discordant growth as defined by > 15% difference in birth weight and small-for-gestational-age (SGA) birth as defined by birth we

[Placental teratoma: differential diagnosis with fetal acardia].

The acardiac fetus is a rare congenital malformation observed only in monozygotic twin pregnancies (1/35.000 pregnancies). Placental teratoma is extremely rare (less than 20 cases have been reported in the literature since 1925). A case of placental teratoma, misdiagnosed as fetus acardius amorphus, is presented with ultra-sound follow-up, delivery and outcome of the normal fetus. Pathological findings, histogenesis, differential diagnosis with the acardiac fetus and finally review of the literature will be discussed.
