Current situation of African trypanosomiasis.

African trypanosomiasis (sleeping sickness) is fatal, if untreated, and occurs in 36 African countries, south of the Sahara, where some 50 million people are at risk of acquiring infection. In the absence of adequate control measures epidemics occur, which are costly and difficult to control. The history of sleeping sickness has been characterized by waves of epidemics, resurgences and outbreaks. Nevertheless, sleeping sickness has been brought practically under control in the early 1950s, in West and Central Africa, through systematic surveillance of the population at risk and in East Africa,

"All of these things interact, that's why it's such a wicked problem": Stakeholders' perspectives of what hinders low back pain care in Australia and how to improve it.

BACKGROUND: Low-quality care for low back pain (LBP) is pervasive in Australia. Drivers of low-quality care have been identified elsewhere and include misconceptions about LBP, vested interests and limited funding for evidence-based interventions. Yet, the literature that identified such drivers is not specific to the Australian context, and therefore, it is likely to represent only part of the local problem. This study aimed to determine where the most influential drivers of LBP care are in the Australian healthcare system and what could be done to address them. METHODS: Clinical leaders fro

Factors that shape the integration of HIV and TB services in Zomba District, Malawi.

BACKGROUND: The co-occurrence of HIV and tuberculosis (TB) presents significant challenges for effective healthcare delivery and patient outcomes. Integrating HIV and TB services has been recognised as a key strategy to optimise care and improve health outcomes. However, the factors that shape the optimal integration of these services remain unclear in many settings. This study aimed to explore the factors that influence the integration of HIV and TB services. METHODS: A purposive sampling technique was used to select study participants from 3 selected health facilities in Zomba, Malawi. This

Making continuity of midwife care implementable across health systems: A global qualitative study.

BACKGROUND: Continuity of midwife care is associated with improved maternal and neonatal outcomes, reduced intervention, and better care experiences, yet implementation across health systems remains uneven. Existing research has largely identified barriers and facilitators, with less attention to how continuity is made implementable across different health system contexts. OBJECTIVE: To identify the recurring forms of contextual and system-level work required to make continuity of midwife care implementable across diverse global settings, from the perspectives of midwives and implementation s

A qualitative study of governance predicament on dengue prevention and control in Malaysia: the elite experience.

BACKGROUND: The challenges faced by healthcare personnel in relation to dengue prevention and control are perennial but noticeably unexplored. It is often difficult to translate policies and decision making by the elite into astute management in consonance with the needs of rank-and-file personnel. In this study, we assess the impact of governance on dengue prevention and control activities in Malaysia as narrated by the elite. METHODS: A qualitative study using a case-study approach was conducted between January 2019 and November 2019 in the districts of Gombak and Klang, where the relevant

Reasons for informal payments from the perspective of health care providers and recipients: a qualitative study in Iran.

Policy action points and approaches to promote fertility care in The Gambia: Findings from a mixed-methods study.

INTRODUCTION: In the Global South, (in)fertility care is scarcely recognized as a priority, yet the government of The Gambia has recently included it as one of the key priorities in its reproductive health strategic plan. This inclusion appears to be the result of years of engagement between policy actors, academic researchers, and activists in the field of reproductive health and specifically of infertility. However, the operationalization of the strategic plan may be hampered by multiple factors. The research aims to identify and analyze challenges that may impede the effective implementatio

Understanding the crisis in harm reduction funding in Central and Eastern Europe.

BACKGROUND: The harm reduction (HR) approach to injecting drug use was rapidly adopted in Central Europe following the fall of the Iron Curtain. The associated social and economic transformation had significant consequences for drug policies in the region. A large number of emerging services have been dependent on funding from a wide range of national and/or local funding programmes, which continue to be unstable, and closely associated with political decisions and insufficient institution building. A sharp distinction is made between health and social services, often without regard to client

METHODS: Researchers who published health systems and policy relevant research in 12 countries in the EMR (Bahrain, Egypt, Ira

INTRODUCTION: In 2017, the Ministry of Health and Public Hygiene (MoH) of Burkina Faso designed and piloted a specimen transport system using the national courier services (La Poste BF) in 4 districts. Based on satisfactory performance indicators, the MoH set a vision aimed at scaling up this system to strengthen disease detection and surveillance of epidemic prone diseases across the country. This work describes the implementation process, performances, and lessons learned. METHODOLOGY: This work describes the implementation process, performances, and lessons learned. Stalkers and harassers of royalty: the role of mental illness and motivation.

BACKGROUND: Public figures are at increased risk of attracting unwanted attention in the form of intrusions, stalking and, occasionally, attack. Whereas the potential threat to the British Royal Family from terrorists and organized groups is clearly defined, there is a dearth of knowledge about that from individual harassers and stalkers. This paper reports findings from the first systematic study of this group. METHOD: A retrospective study was conducted of a randomly selected stratified sample (n=275) of 8001 files compiled by the Metropolitan Police Service's Royalty Protection Unit over 1

Seven decades towards malaria elimination in Yunnan, China.

BACKGROUND: Yunnan Province was considered the most difficult place in China for malaria elimination because of its complex malaria epidemiology, heterogeneous ecological features, relatively modest economic development, and long, porous border with three malaria endemic countries: Lao People's Democratic Republic, Myanmar, and Viet Nam. METHODS: Academic publications and grey literature relevant to malaria elimination in Yunnan covering the period from 1950 until 2020 inclusive were considered. The following academic indexes were searched: China Science Periodical Database, China National Kn

Disaster Psychiatry: An urgent field in psychiatry posing a pertinent question.

Disasters, both natural and man-made, impose a significant burden on the mental health of individuals, communities, and societies. The frequency and intensity of disasters are increasing; 3–4fold compared to the last century, with 400–500 significant disasters/year, affecting >1.5 billion people worldwide and costing 250–400 billion dollars/year. Most natural disasters are directly or indirectly linked to climate change, itself a natural disaster of human origin. Armed conflict is another human self-infliction; 59 state-based conflicts are currently active, the highest since WW2 (Uppsala Confl

Complexities of the charitable and philanthropic landscape within Mexico's children's hospitals.

In today's Mexico, approximately 50% of children experience multidimensional poverty, with Indigenous children particularly affected; half of them live in extreme poverty and nearly all lack access to social security. The current political landscape threatens to exacerbate these issues. Reductions in public spending and the dismantling of federal child health programmes are likely to further hinder children's development, well-being and health by limiting access to vital services like childcare, education and healthcare. Children's hospitals in Mexico may increasingly depend on philanthropy an

Motivations, experiences and perspectives of physicians in Germany regarding monitoring routines in coronary heart disease and post-stroke patients: A qualitative interview study.

Chronic conditions, like coronary heart disease and post stroke status require lifelong care which negatively impacts upon patients' quality of life and increases burden on the healthcare system. Monitoring routines in chronic conditions, including echocardiography and carotid ultrasound, should have evidence of benefit for the patient, detect deterioration and guide treatment adaptations. However, monitoring routines often lack evidence for intervals, specific tests or apparative diagnostic tools. The aim of this qualitative interview study was to identify the motivations, typical procedures
