Antibiotic prophylaxis and prosthetic valve endocarditis.

Early onset prosthetic valve endocarditis is one of the most lethal complications after valve replacement. During the first year of operation of our new cardiac surgical program, we observed 10 cases of prosthetic valve endocarditis, the majority being caused by staphylococci, making an incidence of 10.6%. Subsequent investigations uncovered a very high prevalence of methicillin-resistant strains which led to a radical change in the antibiotic prophylaxis, from a cephalosporin-based protocol to a two drug regime of vancomycin and netilmicin. There were no cases of prosthetic infection among th

Bioaccessibility, composition, and anti-inflammtory activity of bound polyphenols from burdock (Arctium lappa L.) root insoluble dietary fiber.

INTRODUCTION: Reconstruction of mandibular disarticulation defects is a challenging area of head and neck surgery, with a variety of options available for replacement of the condylar head. The gold standard is autogenous reconstruction of the condyle-ramus unit. The use of a prosthetic condylar head is controversial, but in challenging cases, and those with a likely poor prognosis it may be considered and can achieve a good functional result. The objective of this study is to evaluate the outcomes of its use in a high volume head and neck reconstructive unit. MATERIALS AND METHODS: A retrospe

The Lillehei-Kaster aortic valve prosthesis. Long-term results in 273 patients with 1253 patient-years of follow-up.

The Lillehei-Kaster aortic valve prosthesis was inserted in 313 patients between August 1975 and December 1984. Aortic valve replacement alone was done in 193 patients. Additional procedures included coronary artery bypass grafting in 62 patients, mitral valve replacement in 22, mitral valvuloplasty in 8, and miscellaneous procedures in 28. The 273 patients discharged from the hospital are the basis of this report. They have been followed up for 1253 patient-years. The range of follow-up is 1 to 11 years, and the mean is 4.6 years. The 5-year survival rate for all patients was 68%, whereas for

In-Host Adaptation of Salmonella enterica Serotype Dublin during Prosthetic Hip Joint Infection.

Genome degradation has been central to the adaptation of Salmonella enterica serotypes to their hosts throughout evolution. We witnessed the patho-adaptation of a strain of Salmonella Dublin (a cattle-adapted serotype) to a human host during the course of a recurrent prosthetic hip joint infection evolving over several years.

Outcomes of Transvenous Lead Extraction for Cardiovascular Implantable Electronic Device Infections in Patients With Prosthetic Heart Valves.

BACKGROUND: Lead-related or valve-related endocarditis can complicate cardiovascular implantable electronic device (CIED) infection in patients with both CIED and prosthetic valves. The objective of this study was to determine the outcomes of transvenous lead extraction for CIED infection in patients with prosthetic valves. METHODS AND RESULTS: We retrospectively screened 794 transvenous lead extraction procedures, between September 1, 2001 and August 31, 2012, at Mayo Clinic to identify patients with prosthetic valves who underwent lead extraction for infection. Demographic, clinical, and fo

Epidural collection following a late infected total knee replacement--a case report.

Prosthetic joint infection is a well-described complication following primary joint replacement. The association between an epidural abscess and joint replacement surgery following the use of epidural anaesthetics has been previously documented. There is however, no report in the literature of an epidural abscess forming as a result of haematogenous spread from an infected prosthetic joint. This case report presents such a scenario and highlights the importance of early diagnosis and treatment of infected prosthetic joints to avoid further systemic complications.

[Infection of a total knee prosthesis revealed by an abscess of the posterior leg compartment. A case report].

We report a case of infection of a non-cemented total knee prosthesis that led to abscess formation in the posterior compartment of the leg. This case illustrates the deleterious effect of screws used to fix the non-cemented articular surface of the tibia. Infection, like osteolysis due to polyethylene granuloma, can develop along the screw tract and reach the posterior compartment, especially when the screw protrudes through the posterior cortical of the tibia. Diffusion of an intra-articular event into the posterior compartment should be suspected in patients with a total knee prosthesis who

Vancomycin-resistant Enterococcus infected total knee arthroplasty.

Use of vancomycin to treat infections and hospital colonization with methicillin-resistant Staphylococcus aureus has contributed to the development of vancomycin resistance in Enterococcus. Postoperative infection with vancomycin-resistant Enterococcus developed in 2 patients after total knee arthroplasty, indicating that the infections were nosocomial. Both patients required multiple procedures. The infections were controlled with serial open débridements. One knee was fused successfully, and the other was managed with resection arthroplasty.

Infection of PTFE grafts used to create arteriovenous fistulas for hemodialysis access.

Between January 1993 and December 1994, we treated 19 patients for infection involving expanded polytetrafluoroethylene (ePTFE) grafts used to create arteriovenous fistulas (AVF) for hemodialysis. Manifestations included false aneurysm in nine cases, periprosthetic abscess in seven cases, and tunnel contamination in three cases. The port of entry was the puncture site or catheter in 63% of cases. The delay to onset of infection was significantly longer in patients with secondary graft infection than in those with postoperative graft infection (7.8 months vs. 1.8 months; p < 0.05). Primary trea

Local treatment of prosthetic vascular graft infection with multivesicular liposome-encapsulated amikacin.

BACKGROUND: Contaminated surgical fields limit the use of prosthetic vascular grafts. We studied the efficacy of sustained-release amikacin applied locally to contaminated grafts in the prevention of infectious complications. MATERIALS AND METHODS: Thirty-one New Zealand white rabbits underwent placement of a polytetrafluoroethylene (PTFE) interposition graft in a 1-cm segment of the descending aorta. The surgical field was infected with application of 10(5) to 10(8) Staphylococcus aureus organisms suspended in normal saline solution. Nineteen rabbits underwent contaminated aortic graft place

Clinical experience with metal-on-metal total joint replacements: indications and results.

Survivorship of total joint arthroplasty depends on the durability of fixation and durability of articulation. The metal-on-polyethylene articular couple has been the most widely used. Polyethylene wear (and the associated cytochemical events that culminate in osteolysis) has been identified as a major factor adversely influencing the durability of joint replacement. This stimulated the orthopaedic community to explore the possibility of using alternative bearings with lower wear rates. Hard-on-hard bearings have been shown to be associated with reduced wear. Metal-on-metal bearings have wear

Transesophageal color Doppler three-dimensional echocardiographic detection of prosthetic aortic valve dehiscence: correlation with surgical findings.

Three-dimensional echocardiography is being used with increasing frequency to evaluate cardiac function and structure. We present two cases of prosthetic aortic valve endocarditis where three-dimensional transesophageal echocardiography correlated with the intraoperative surgical findings of prosthetic dehiscence and communication with an abscess cavity. When compared with two-dimensional transesophageal echocardiography, three-dimensional echocardiography was more accurate in defining the exact site of dehiscence and communication with an abscess cavity. The echocardiographic images were orie

Bacterial orbital infection leading to orbital exenteration: a case series.

Orbital exenteration (OE) secondary to infection is rare. This case series reports 3 cases and aims to elucidate the possible underlying etiopathogenesis. A retrospective data analysis was performed of patients who underwent an OE and subsequently received an orbital prosthesis from the Dutch Institute for Facial Prosthetics between 2017 and 2024. Only cases secondary to infection were included. Of the 44 cases treated, only 3 were infection related. An odontogenic infection was the underlying cause in 2 cases, a periorbital/orbital necrotizing fasciitis due to a skin infection in the third ca

Laser Hair Reduction to the Residuum Improves Quality of Life in People with Amputations.

People with amputations using limb prosthetics commonly develop dermatologic complications, such as impaired skin integrity and folliculitis, at the residuum-prosthesis interface (RPI). Dermatologic complications are the primary cause of dissatisfaction with prosthetics, hindering rehabilitation and leading to prosthesis abandonment. This has a deleterious impact on health-related quality of life (HRQOL). Positive outcomes have been associated with laser hair reduction (LHR) for those with traumatic lower limb amputations; however, this has not been studied in those with upper limb amputations

Patient and implant survival following joint replacement because of metastatic bone disease.

BACKGROUND: Patients suffering from a pathological fracture or painful bony lesion because of metastatic bone disease often benefit from a total joint replacement. However, these are large operations in patients who are often weak. We examined the patient survival and complication rates after total joint replacement as the treatment for bone metastasis or hematological diseases of the extremities. PATIENTS AND METHODS: 130 patients (mean age 64 (30-85) years, 76 females) received 140 joint replacements due to skeletal metastases (n = 114) or hematological disease (n = 16) during the period 20

Aortic Graft Infection Secondary to Iatrogenic Transcolonic Graft Malposition.

UNLABELLED: Aortic graft infections are a rare but devastating complication of aortic revascularization. Often infections occur due to contamination at the time of surgery. Iatrogenic misplacement of the limbs of an aortobifemoral graft is exceedingly rare, and principles of evaluation and treatment are not well defined. We report 2 cases of aortobifemoral bypass graft malposition through the colon. CASE REPORT: Case 1 is a 54-year-old male who underwent aortobifemoral bypass grafting for acute limb ischemia. He had previously undergone a partial sigmoid colectomy for diverticulitis. Approxim

Predictors of failure for prosthetic joint infections treated with debridement.

OBJECTIVE: Prosthetic joint infections (PJI) may be cured in selected patients with debridement and prosthesis retention. We aimed to identify predictors of failure to better target patients most likely to benefit from this conservative strategy. METHODS: Observational study of patients presenting with PJI initially treated at our hospital with debridement between 2008 and 2011, with>6 months of post-treatment follow-up. RESULTS: Sixty consecutive patients presenting with PJI (hip, n=34; knee, n=26) fulfilled the inclusion criteria. Failures (n=20, 33%), predefined as persistence of PJI sign
