Pain Management Services Can Be a Cost-Effective Option over Surgery for the Treatment of Chronic Pain.

PURPOSE OF COMMENTARY: The number of facilities and physicians that offer pain management services has grown significantly since the 1970s. Despite the rise in prevalence of individuals with chronic pain during that time, interventional pain physicians remain underutilized. One reason why this may be the case is a large number of patients are referred directly to surgical specialists prior to being seen by an interventional pain physician. With the rising cost of healthcare, a shift away from surgical specialists and towards pain specialists as part of a multidisciplinary care team may help re

Group pain education is as effective as individual education in patients with chronic temporomandibular disorders.

BACKGROUND: Temporomandibular disorders (TMD) are recognised as the most common chronic orofacial pain condition, with prevalence figures ranging from 3% to 12%. Patients referred to tertiary orofacial pain clinics for the management of TMD often experience delays in receiving treatment. The objective of the present study was to assess a group treatment programme to deliver effective earlier intervention for patients with chronic TMD. METHODS: Forty-two patients with TMD seen at the Oral Medicine Clinic, Royal Dental Hospital of Melbourne, were administered baseline validated pain questionnai

CSF levels of apolipoprotein C1 and autotaxin found to associate with neuropathic pain and fibromyalgia.

OBJECTIVE: Neuropathic pain and fibromyalgia are two common and poorly understood chronic pain conditions that lack satisfactory treatments, cause substantial suffering and societal costs. Today, there are no biological markers on which to base chronic pain diagnoses, treatment choices or to understand the pathophysiology of pain for the individual patient. This study aimed to investigate cerebrospinal fluid (CSF) protein profiles potentially associated with fibromyalgia and neuropathic pain. METHODS: CSF samples were collected from 25 patients with neuropathic pain (two independent sets, n=1

[Neurosurgical treatment of pain].

INTRODUCTION: Surgical treatment of chronic pain includes destructive procedures (neurectomy, rhizotomy, sympathectomy), often referred to as ablative, and accompanied by high morbidity and mortality rates. SURGICAL TREATMENT OF PAIN: During the past three decades, thanks to current knowledge on chronic pain mechanisms and technological developments, such as improved microsurgical and stereotactic techniques, guided by computerized tomography, magnetic resonance imaging and neural tissue impendance monitoring, the majority of ablative procedures have been replaced by new methods. Among them,

The role of surgery in the treatment of typical and atypical facial pain.

Effective management of facial pain syndromes requires a correct clinical diagnosis. The temporal pattern of chronic pain is the most important aspect to be considered. It allows the identification of three groups of patients: (1) those who have paroxysmal pain, (2) those with mixed paroxysmal and constant pain, and (3) those with strictly constant pain. The less is the paroxysmal component, the more likely it seems to be that surgical intervention is useless or even dangerous. In particular, when the diagnosis is atypical facial pain, that is, a diffuse, nonanatomic orofacial pain of unknown

Results of upper cervical palpation test in migraine patients are not explained by pain sensitization.

The exact cause for no pain, local pain and referred pain groups according to the upper cervical palpation test (UPT) and whether neck pain in migraine patients is caused due to pain sensitization or influenced by perceived neck-related disability, is not fully understood. The aim was to determine whether upper cervical spine sensitivity tested by the UPT is associated with neck-related disability or increased pain sensitization in patients with migraine. Forty-two patients with episodic migraine were examined regarding mechanical and pressure pain thresholds, central sensitization (CSI), allo…

The use of radiofrequency ablation for the management of refractory lower extremity pain associated with lumbar spondylosis: illustrative case.

BACKGROUND: The lumbar spine is a common cause of referred pain to the lower extremities. Standard conservative treatments (i.e., physical therapy and pharmacotherapy) can provide only partial or temporary relief, in which case injections might be used. This often consists of epidural steroid injections to treat possible radicular pain. When symptoms still persist, radiofrequency ablation (RFA) to treat facet-mediated pain can offer a viable option before surgical intervention. OBSERVATIONS: In this illustrative case, a patient presenting with a complex array of worsening symptoms in the lowe…

Toothache caused by trigeminal neuralgia of vestibular schwannoma: a case report. [Corrected].

INTRODUCTION: Vestibular schwannoma, also called acoustic neuroma, is a tumor composed of Schwann cells that most commonly involves the vestibular division of the 8th cranial nerve. A thorough medical and dental history and properly conducted diagnostic testing and radiographic evaluation are essential in differentiating odontogenic tooth pain from tooth pain of a nonodontogenic origin. METHODS: This report discusses a case of acoustic neuroma mimicking odontogenic pain. A 61-year-old man presented with pain in the lower left quadrant. Medical history revealed peripheral neuropathy and prior

Atypical odontalgia: an up-to-date view.

Atypical odontalgia (AO) is a little known chronic pain condition. It usually presents as pain in a site where a tooth was endodontically treated or extracted, in the absence of clinical or radiographic evidence of tooth pathology. It is a rare clinical challenge for most clinicians, which leads to the patients being referred to several specialists and sometimes undergoing unnecessary surgical procedures. The pain mechanisms involved in AO are far from clear, and numerous potential mechanisms have been suggested. Currently, the most accredited hypothesis is that AO is a neuropathic pain condit

Low back pain caused by a duodenal ulcer.

The common diagnoses in low back pain are lumbar strain, lumbosacral radiculopathy, osteoarthritis, degenerative disc disease, spinal stenosis, and sacroiliac joint dysfunction. Unusual causes of low back pain that have been previously identified include abdominal aortic aneurysms, pelvic neoplasms, and retroperitoneal hemorrhages. This report describes a case of back pain that was apparently caused by a duodenal ulcer. A 54-year-old man with no significant medical history presented with a complaint of mid to low back pain (T10-L2), which was diagnosed as joint dysfunction. A comprehensive tre

In people with nociplastic dominant chronic pain is multi-disciplinary pain management plus motivational interviewing more effective than multi-disciplinary pain management alone for improving pain self-efficacy and pain interference: A randomised controlled trial protocol.

BACKGROUND: Chronic pain is the leading cause of disability and disease burden globally. Emerging evidence suggests that people with nociplastic dominant chronic pain commonly have heightened pain sensitivity, high levels of pain interference and low self-efficacy. There is low certainty evidence supporting the effectiveness of motivational interviewing in a range of health conditions including chronic pain. However, the impact of motivational interviewing as an adjunct to evidence-based multi-disciplinary pain management has not been adequately evaluated. The aim of this paper is to describe

Mental Pain as a Transdiagnostic Patient-Reported Outcome Measure.

Patient-reported outcomes (PROs) refer to any report coming directly from patients about how they function or feel in relation to a health condition or its therapy. PROs have been applied in medicine for the assessment of the impact of clinical phenomena. Self-report scales and procedures for assessing physical pain in adults have been developed and used in clinical trials. However, insufficient attention has been dedicated to the assessment of mental pain. The aim of this paper is to outline the implications that assessment of mental pain may entail in psychiatry and medicine, with particular

American Burn Association Guidelines on the Management of Acute Pain in the Adult Burn Patient: A Review of the Literature, a Compilation of Expert Opinion, and Next Steps.

The ABA pain guidelines were developed 14 years ago and have not been revised despite evolution in the practice of burn care. A sub-committee of the American Burn Association's Committee on the Organization and Delivery of Burn Care was created to revise the adult pain guidelines. A MEDLINE search of English-language publications from 1968 to 2018 was conducted using the keywords "burn pain," "treatment," and "assessment." Selected references were also used from the greater pain literature. Studies were graded by two members of the committee using Oxford Centre for Evidence-based Medicine-Leve

A systematic review of diagnostic tests to detect pelvic floor myofascial pain.

INTRODUCTION AND HYPOTHESIS: Myofascial pain arising from pelvic floor muscles occurs in women with vaginismus, interstitial cystitis and endometriosis but is often overlooked. The aim is to examine alternative diagnostic tests to detect pelvic floor myofascial pain compared with standardized vaginal palpation of pelvic floor muscles as the reference test. METHODS: A systematic review was prospectively conducted (PROSPERO-CRD42020183092) according to PRISMA guidelines. Databases searched included Ovid Medline 1946-, Embase 1957-, Scopus 1960-, Cochrane Combined, Clinical trials, Google Schola

The Prevalence and Severity of Pain in Patients With Systemic Sclerosis.

We investigated pain prevalence and severity in systemic sclerosis (SSc) and patient-physician perceptions. This was an internet-based survey that compared perceptions of pain type, location, severity, associated factors between patients with SSc and physicians, and pain treatment prescription patterns in Japan in March 2023. Data from 301 patients and 129 physicians revealed that 96.0% of patients experienced pain compared with 43.4% estimated by physicians. The median (interquartile range) Short-form McGill Pain Questionnaire (SF-MPQ-2) pain score was 47.0 (14.0-88.0). Continuous pain had th

Efficacy of Physiotherapy Rehabilitation for Subtrochanteric Femur Fracture: A Case Report.

Proximal femur fractures that occur within 5 cm of the lesser trochanter are commonly referred to as subtrochanteric femur fractures (STF). In this case report, we depicted a 45-year-old who came with a history of road traffic accident (RTA) for which an investigation like an X-ray was performed which revealed STF and the patient was surgically managed. Postoperatively, the patient's main complaints were pain around the hip joint with restriction in performing hip joint full range of motion. For these complaints, she was given physiotherapy. There was a reduction in joint pain, a significant i

Small fiber polyneuropathy as a potential therapeutic target in interstitial cystitis/bladder pain syndrome.

INTRODUCTION AND HYPOTHESIS: Interstitial cystitis/bladder pain syndrome (IC/BPS) and fibromyalgia (FM) are frequently co-occurring medical diagnoses in patients referred to the urology clinic for secondary and tertiary treatment options. METHODS: Abundant literature has shown that many patients with FM have small fiber polyneuropathy (SFPN) that can be confirmed via skin punch biopsy and immunological staining to measure nerve density. RESULTS AND CONCLUSIONS: This finding of SFPN provides a therapeutic target for FM and in this article we hypothesize and provide rationale for the idea that

A strange case of dyspnoea.

Acquired neuralgic amyotrophy, described for the first time by Parsonage and Turner, is a rare idiopathic disease that may occur in otherwise normal healthy individuals. It typically begins with sudden, unilateral shoulder pain that may also involve the neck and/or arm. Less frequently, the disease involves nerves other than those of the brachial plexus, such as phrenic nerves, resulting in dyspnoea. The diagnosis is based on the clinical presentation and is generally supported by electroneurography/electromyography. We report the case of a 45-year-old white man who was referred to our clinic
