Vision and vertigo: some visual aspects of vestibular disorders.

This review deals with two syndromes, oscillopsia and visual vertigo. Oscillopsia is the illusion of oscillation of the visual surroundings. For diagnosis purposes one should ask, when does the oscillopsia occur? If oscillopsia is only present during head (or whole body) movements, the likely underlying cause is a bilateral defect in the vestibulo-ocular reflex (VOR). The more common causes are post meningitic vestibular damage, gentamicin ototoxicity or bilateral idiopathic vestibular failure. When oscillopsia develops after specific head positions, it is usually due to a positional nystagmus

Cellular basis of vestibular compensation: analysis and modelling of the role of the commissural inhibitory system.

In this study we used a cellular network model of the brainstem vestibulo-ocular reflex (VOR) pathways to investigate the role of the vestibular commissural system in "vestibular compensation", the behavioural recovery that takes place after unilateral labyrinthectomy (UL). The network was initialized on the basis of mathematical analysis and trial simulations to generate a VOR response with a physiologically realistic gain and time constant. The effects of a selective decrease in the strength of commissural inhibitory input to the ipsi-lesional medial vestibular nucleus (MVN) neurones, withou

A Proposed Mechanism for Visual Vertigo: Post-Concussion Patients Have Higher Gain From Visual Input Into Subcortical Gaze Stabilization.

PURPOSE: Post-concussion syndrome (PCS) is commonly associated with dizziness and visual motion sensitivity. This case-control study set out to explore altered motion processing in PCS by measuring gaze stabilization as a reflection of the capacity of the brain to integrate motion, and it aimed to uncover mechanisms of injury where invasive subcortical recordings are not feasible. METHODS: A total of 554 eye movements were analyzed in 10 PCS patients and nine healthy controls across 171 trials. Optokinetic and vestibulo-ocular reflexes were recorded using a head-mounted eye tracker while part

Vestibular Rehabilitation as an Early Intervention in Athletes Who are Post-concussion: A Systematic Review.

BACKGROUND: Sports-related concussions (SRC) are a common injury sustained by many athletes of all different age groups and sports. The current standard treatment is rest followed by aerobic activity. Minimal research has been done on the effects of vestibular rehabilitation for concussion treatment, especially in physical therapy practice. OBJECTIVE: The purpose of this study was to examine the effects of early intervention of vestibular rehabilitation (VRT) on an athlete's time to return to play compared to rest alone. STUDY DESIGN: Systematic Review. METHODS: Two searches were conducted

Progress in Balance Function and Vestibular Symptoms Following Vestibular Rehabilitation in a Patient With Left Cerebellar Infarction.

Vertigo and balance impairment are the primary symptoms of vestibular impairment. These symptoms affect an individual's activities of daily living (ADL) and quality of life (QOL). In patients with stroke, vestibular symptoms vary depending on the area of damage, making it crucial to assess the appropriateness and effectiveness of vestibular rehabilitation (VR) according to each symptom and affected region. Here, we aimed to report the progress of balance function and vestibular symptoms after VR in a patient with left cerebellar infarction. A 50-year-old male patient with left cerebellar infar

Bilaterally positive head-impulse tests can differentiate AICA infarction from labyrinthitis.

INTRODUCTION: Video head-impulse tests (video-HITs) often fail to detect anterior inferior cerebellar artery (AICA) infarction due to peripheral and central vestibular system involvement. Anecdotal studies suggest that video-HITs may reveal bilateral impairment in AICA infarction. However, the diagnostic utility of video-HITs has not been established, particularly when compared to labyrinthitis, which accounts for the majority of acute audiovestibular syndrome (AAVS) cases. METHODS: We reviewed the medical records of consecutive patients presenting with new-onset acute hearing loss and sponta

Evidence of Vestibular and Balance Dysfunction in Patients With Mild Cognitive Impairment and Alzheimer's Disease.

OBJECTIVES: Given the expected rise in dementia prevalence, early diagnosis is vital. As a growing body of literature has identified a potential association between vestibular function and cognition, vestibular assessment may aid in early screening. The aim of the study was to better comprehend the proposed association between vestibular function and Alzheimer's disease (AD) by comparing vestibular parameters (vestibular function testing and clinical balance measures) between a group with mild cognitive impairment (MCI), AD, and healthy controls with age-normal cognition. DESIGN: Cross-sectio

Rebalancing the Vestibular System by Unidirectional Rotations in Patients With Chronic Vestibular Dysfunction.

Introduction: Vestibular dysfunction is a common disorder that results in debilitating symptoms. Even after full compensation, the vestibulo-ocular reflex (VOR) could be further improved by using rehabilitation exercises and visual-vestibular adaptation. We hypothesized that in patients with asymmetric vestibular function, the system could be rebalanced by unidirectional rotations toward the weaker side (i.e., a pure vestibular stimulation). Methods: Sixteen subjects (5 female and 11 male, 43.2 ± 17.0 years old) with chronic vestibular dysfunction that was non-responsive to other types of medi

Preferential Impairment of the Contralesional Posterior Semicircular Canal in Internuclear Ophthalmoplegia.

BACKGROUND: The vertical vestibulo-ocular reflex (VOR) may be impaired in internuclear ophthalmoplegia (INO) as the medial longitudinal fasciculus (MLF) conveys VOR-signals from the vertical semicircular canals. It has been proposed that signals from the contralesional posterior semicircular canal (PSC) are exclusively transmitted through the MLF, while for the contralesional anterior canal other pathways exist. OBJECTIVE: Here, we aimed to characterize dysfunction in individual canals in INO-patients using the video-head-impulse test (vHIT) and to test the hypothesis of dissociated vertical

Compensatory saccades in head impulse testing influence the dynamic visual acuity of patients with unilateral peripheral vestibulopathy1.

BACKGROUND: Both the dynamic visual acuity (DVA) test and the video head-impulse test (vHIT) are fast and simple ways to assess peripheral vestibulopathy. After losing peripheral vestibular function, some patients show better DVA performance than others, suggesting good compensatory mechanisms. It seems possible that compensatory covert saccades could be responsible for improved DVA. OBJECTIVE: To investigate VOR gain and compensatory saccades with vHIT and compare them to the DVA of patients with unilateral peripheral vestibulopathy. METHODS: VOR gain deficit and compensatory saccades were

Audiological disturbances caused by long-term exposure to industrial solvents. Relation to the diagnosis of toxic encephalopathy.

Sixty workers, consecutively admitted due to suspicion of solvent-induced chronic toxic encephalopathy (CTE), were investigated with pure-tone audiometry, determination of speech recognition of monosyllabic words and distorted speech and cortical response audiometry (CRA). Eighteen workers not exposed to occupational solvents and noise were also investigated. The scores in the distorted speech test were significantly lower and the CRA latencies were significantly longer in the solvent group than in the control group. There was no difference between the groups in the pure-tone and monosyllabic

[Objective assessment of disorders of visual perception following unilateral vestibular loss. Studies of the so-called Dandy symptom].

Visual ability and compensatory eye movements during defined vertical oscillation were investigated in 20 patients with unilateral lesions of labyrinthine function and in 20 normal subjects. Oscillation frequencies were performed at the rate of 1 to 1.5 Hz with an amplitude of 5 cm, comparative to head locomotions of a running person. In synchronism with this, the visual function was tested with Landolt rings. Patients complaining of subjective visual disturbance during walking and running, also presented a measurable blur of vision under test conditions. In addition, eye movements were record

Pathological eye-head coordination in roll: tonic ocular tilt reaction in mesencephalic and medullary lesions.

The fundamental pattern of coordinated eye-head roll motion is based upon utricular/saccular as well as vertical canal input, and mediated by the graviceptive pathways from the labyrinths via pontomedullary vestibular nuclei to the rostral midbrain tegmentum. The tonic bilateral graviceptive input stabilizes the eyes and head in the normal upright position. A unilateral lesion causes imbalance in vestibular tone in the roll plane which results in a tonic ipsiversive ocular tilt reaction (OTR). OTR, the triad of ipsilateral head tilt, skew deviation and ocular torsion, occurred as a tonic respo
