A 47-year-old man presented with severe hypokalemic paralysis and respiratory failure. A large amount of potassium was administered along with providing intensive care, and his condition improved. Hypokalemia was attributed to increased urinary potassium excretion. A kidney biopsy was performed to make a definitive histological diagnosis. It revealed acute tubulointerstitial nephritis (TIN). After the diagnosis, prednisolone was administered, and the TIN gradually improved. From the clinical course and laboratory findings, the TIN was presumed to be an autoimmune disorder. Further specific aut

Since the first identification of neonatal microcephaly cases associated with congenital Zika virus infection in Brazil in 2015, a distinctive constellation of clinical features of congenital Zika syndrome has been described. Fetal brain disruption sequence is hypothesized to underlie the devastating effects of the virus on the central nervous system. However, little is known about the effects of congenital Zika virus infection on the peripheral nervous system. We describe a series of 4 cases of right unilateral diaphragmatic paralysis in infants with congenital Zika syndrome suggesting periph

large subtorcular occipital encephalocele in neonate carries higher risk of associated hindbrain anomalies and secondary process for neurological deterioration which predict the surgical outcomes and long-term prognosis. The dysfunction of the lower cranial nerves often leads to worsening of neurological status from poor respiratory function and repeated aspiration pneumonia. The aims of repairing encephalocele include a good closure of the defect, preservation or restoration of neurological functions and better cosmetic results. The author presented a successful surgical strategy for manageme

The Dutch 'euthanasia and assisted suicide' practice guideline advises using 2000 mg thiopental to induce coma, followed by a muscle relaxant to cause death by respiratory paralysis. However, when a doctor administers such a high dose of thiopental as a bolus injection to a cachectic patient, there is a high likelihood of immediate death and other side effects, which can be distressing for the family. Doctors who administered less than 2000 mg have been reprimanded for not working according to current standards. Arguments are given concerning in which circumstances it is reasonable to use the …

Paralysis of the diaphragm is a severe consequence of cervical spinal cord injury. This condition can be experimentally modeled by lateralized, high cervical lesions that interrupt descending inspiratory drive to the corresponding phrenic nucleus. Although partial recovery of ipsilateral diaphragm function occurs over time, recent findings show persisting chronic deficits in ventilation and phrenic motoneuron activity. Some evidence suggests, however, that spontaneous recovery can be enhanced by modulating neural pathways to phrenic motoneurons via synaptic circuitries which appear more comple

Spinal muscular atrophy (SMA) is a relatively common, profoundly disabling and incurable disease that presents in early childhood with hypotonia, weakness and decreased movement. Without ventilatory support, premature death from respiratory insufficiency is universal in children with spinal muscular atrophy type 1 (SMA1). With mechanical ventilation, however, long-term survival in SMA1 has been reported from numerous international centres. Children kept alive by this means experience progressive paralysis and eventually become effectively 'locked in' on the ventilator, with no useful movements

A 22-year-old male was admitted to hospital with diarrhea and vomiting, cardiac arrhythmias, severe hypokalemia and gradual onset of muscular weakness. A potassium infusion was started, but for several hours serum potassium remained low. Evidence of toxic ingestion was initially lacking. When it became clear -- after a considerable delay -- that the patient had ingested barium nitrate, hemodialysis was started. This resulted in rapid clinical improvement with correction of hypokalemia and restored muscular function. Intoxication with barium causes hypokalemia, arrhythmias, muscular weakness an

We present a 2.5-year-old girl in severe asthma crisis who clinically deteriorated on conventional mechanical ventilation, but was successfully ventilated with high-frequency oscillatory ventilation (HFOV). Although HFOV is accepted as a technique for managing pediatric respiratory failure, its use in obstructive airway disease is generally thought to be contraindicated because of the risk of dynamic air-trapping. However, we suggest that obstructive airway disease can safely be managed with HFOV, provided certain conditions are met. These include the application of sufficiently high mean airw

This is a case report of atypical Guillain-Barré syndrome (GBS). A 42-year-old woman displayed pharyngeal paralysis after a mild upper respiratory infection, subsequently having developed ophthalmoplegia, facial diplegia and accessory nerve palsy within about ten days. She had neither weakness nor abnormal tendon reflexes in the extremities. During the acute phase of the disease we found significant elevation of anti-GQ1b and anti-GT1a IgG antibodies in the serum, and immunoadsorption therapy had a remarkable effect on the symptoms. Although our case was extremely atypical of GBS in terms of e

In this report we describe the clinical history and symptoms in a 36-year-old male with Hajdu-Cheney syndrome, an autosomal dominant condition with dissolution of the terminal phalanges (acro-osteolysis), characteristic craniofacial dysmorphism, and musculoskeletal alterations. He was admitted at that age because of progressive respiratory problems, with Cheyne-Stokes respiration and bilateral vocal cord paralysis. Terminal renal failure with cystic renal disease was diagnosed at the age of 14 years. The findings in the present patient illustrate the risk of progressive neurologic degeneration

Diaphragmatic plication is a safe, well tolerated procedure for congenital diaphragmatic eventration. It is the treatment of choice in neonates with respiratory distress following phrenic nerve injury. The abdominal approach is as good as the thoracic and may be safer in cases with central eventration and when the diagnosis of congenital diaphragmatic hernia cannot be excluded with confidence. However, in phrenic nerve paralysis the thoracic approach is superior in showing the anatomical distribution of the phrenic nerve. Experience with eight paediatric patients treated for diaphragmatic even

An awake patient presented with central neurogenic hyperventilation induced by a cerebral tumor. Corticosteroid therapy and brain irradiation while the patient was anesthetized and respiration controlled under pancuronium-induced respiratory paralysis were followed by tumor regression and resolution of hyperventilation. Recurrence of tumor 6 weeks later was not accompanied by recurrence of hyperventilation. Cytologic study of cerebrospinal fluid revealed B-cell lymphoma. This patient brings to 10 the number of cases recorded with tumor-induced central neurogenic hyperventilation. Five of the e…

An acute polyneuropathy developed during intensive-care treatment of three young men who had sustained severe multiple traumas and of one woman with bacterial meningoencephalitis. In the first case there was a predominantly distal paraparesis of the legs; in the second, flaccid neurological deficits occurred particularly in the areas of the left ulnar, peroneal and tibial nerves combined with paralysis of cranial nerves IX, X and XII. A symmetrical sensorimotor polyneuropathy with tetraplegia and cranial nerve deficits occurred in the third case, and an asymmetrical sensorimotor polyneuropathy

The paper reports on clinical and pathologic findings in a 51-year-old man who presented with rapidly progressive spinal muscular atrophy associated with relatively early respiratory paralysis. Clinical diagnosis was based on typical EMG, and bioptic and enzyme findings. In the CSF protein and cell content were normal, the IgG concentration was high, the IgG/Alb quotient increased, intrathecal synthesis of IgG was evident, and oligoclonal gammopathy was found. All of these findings generally appear in neuroallergic diseases. Total loss of anterior-horn cells, demyelination of fasciculus cuneat…

The authors report a new case of caecal perforation complicating acute dilatation of the colon without organic obstruction (Ogilvie's syndrome). They recall the two characteristics of this syndrome : abdominal distension due to colonic ileus, without any organic cause, and the constant coexistence of an associated pathological condition (traumatic, post-operative, infective, cardio-vascular, respiratory or neurological). The major complication is caecal perforation, announced by a clinical and radiological preperforative syndrome. The pathogenesis of Ogilvie's syndrome remains mysterious. Two

Yemen's prolonged conflict and fragile healthcare infrastructure have created a setting where infectious diseases like cholera frequently overlap with under-recognized post-infectious autoimmune conditions. In this context, we report a pediatric case of Guillain-Barré syndrome (GBS) following recent Campylobacter jejuni infection during a laboratory-confirmed cholera outbreak in rural Southern Yemen. The 12-year-old patient presented with acute flaccid paralysis and respiratory failure after a diarrheal illness consistent with cholera. Nerve conduction studies supported the GBS diagnosis, and

Guillain-Barré syndrome (GBS) is an acute immune-mediated inflammatory demyelinating polyneuropathy characterized by symmetrical limb weakness and areflexia. GBS is typically monophasic but may recur in 1%-7% of patients. Many etiologies exist for GBS; one rare associated etiology is surgery, most notably orthopaedic, abdominal, and cardiac surgery. Here, we present a 76-year-old male who developed postsurgical GBS after a right total knee arthroplasty (TKA), 11 years after developing GBS following a left TKA. The patient developed worsening ascending paralysis and, ultimately, respiratory fai

Neurological manifestations of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection have been reported in adults and in children, varying from mild to more debilitant symptoms, including fatigue, headache and dizziness. A series of studies have revealed a possible association between Guillain-Barré syndrome (GBS), the most common cause of acute flaccid paralysis at all ages, and SARS-CoV-2 infection. Case reports of novel coronavirus disease 2019 (COVID-19)-associated GBS mainly include adult patients, while only a few pediatric cases have been reported. The present study des

The Australian Paediatric Surveillance Unit (APSU) has been prospectively collecting national data on rare childhood conditions since 1993, with monthly reporting of cases by paediatricians. In this report we describe annual results from studies for ten communicable diseases and complications of communicable diseases that were conducted using APSU surveillance in 2019 and place these in an historic context. Results are reported on acute flaccid paralysis, congenital cytomegalovirus infection, neonatal herpes simplex virus infection, perinatal exposure to HIV, paediatric HIV infection, severe c

Sixteen trauma victims with adult respiratory distress syndrome were retrospectively examined. High injury severity score (mean: 44), massive transfusion requirements, and prolonged ventilator days characterize this group. Persistent intra-abdominal infection accounted for two of three deaths in this series. Most of these patients were managed without paralysis using intermittent mandatory ventilation and positive-end expiratory pressure (PEEP). High frequency jet ventilation was necessary in one subject. Two patients exhibited early ARDS reversal, ie, clinical improvement, better chest x-ray
