Autonomic regulation during sleep and wakefulness: a review with implications for defining the pathophysiology of neurological disorders.

Cardiovascular and respiratory parameters change during sleep and wakefulness. This observation underscores an important, albeit incompletely understood, role for the central nervous system in the differential regulation of autonomic functions. Understanding sleep/wake-dependent sympathetic modulations provides insights into diseases involving autonomic dysfunction. The purpose of this review was to define the central nervous system nuclei regulating sleep and cardiovascular function and to identify reciprocal networks that may underlie autonomic symptoms of disorders such as insomnia, sleep a

A 9-year-old female with iron deficiency has severe periodic limb movements while taking mirtazapine for insomnia.

UNLABELLED: Mirtazapine is a Food and Drug Administration-approved atypical antidepressant used off-label for insomnia. Mirtazapine has been associated with movement disorders in adults. A 9-year-old female was seen in the sleep clinic for symptoms of insomnia, nocturnal awakenings, restless sleep, and growing pains. Mirtazapine was started prior to presentation for severe insomnia. A sleep study showed frequent repetitive leg movements prior to sleep onset as well as significant periodic limb movement disorder with a periodic limb movement index of 25.1/hour. The child was found to have a fer

Primary Sleep Disorders.

Primary sleep disorders include those not attributable to another medical or psychiatric condition: insomnia disorder, hypersomnolence disorder, narcolepsy, obstructive sleep apnea hypopnea syndrome, central sleep apnea syndrome, and the parasomnias. They are commonly encountered and are comorbid with many psychiatric disorders. It is important to recognize these disorders and be comfortable treating them or to know when to refer to a sleep disorders center and sleep specialist. Treatment of a comorbid sleep disorder can improve the overall quality of life, symptoms in mood disorders, and symp

Chronic Abdominal Myoclonus Controlled with Prednisone Therapy: A Case Report.

Abdominal myoclonus may manifest as involuntary, repetitive contractions of the abdominal wall due to a variety of neurologic pathologies. There are, however, limited cases reporting abdominal wall myoclonus without any clear neurologic etiologies. Here we present a case of a 72-year-old male with a history of rheumatoid arthritis, asthma, obstructive sleep apnea (OSA), and restless leg syndrome who presented with chronic, involuntary, repetitive contractions of his abdomen without any spinal or extremity involvement. His extensive neurologic and secondary systematic workup was negative, and h

Sleep Dysfunction in Parkinson's Disease.

INTRODUCTION: Sleep disorders are common in Parkinson's Disease (PD). It can antedate the motor manifestations of PD. It is related primarily to the involvement of sleep regulating structures, secondary involvement through motor, depressive and dysautonomic symptoms and the tertiary involvement through anti-parkinsonian medications. AIM: The aim of our study is to evaluate the frequency and nature of the sleep abnormalities in Idiopathic Parkinson's Disease, analysing the sleep architecture using polysomnography and to correlate the results with the disease parameters. MATERIALS AND METHODS:

However, our understanding of the pathophysiological mechanisms involved is preliminar … regulate circadian function and striatal expression of circadian genes. The association of TD with restless legs syndrome and periodic limb movements indicates shared pathophysiology, including iron deficiency, and variants in the BTDB9 gene. Management of Sleep Disturbances in Parkinson's Disease.

Parkinson's disease (PD) is defined by its motor symptoms rigidity, tremor, and akinesia. However, non-motor symptoms, particularly autonomic disorders and sleep disturbances, occur frequently in PD causing equivalent or even greater discomfort than motor symptoms effectively decreasing quality of life in patients and caregivers. Most common sleep disturbances in PD are insomnia, sleep disordered breathing, excessive daytime sleepiness, REM sleep behavior disorder, and sleep-related movement disorders such as restless legs syndrome. Despite their high prevalence, therapeutic options in the in-…

Effects of Cannabinoids on Sleep and their Therapeutic Potential for Sleep Disorders.

The recent trend for legalization of medicinal cannabis and cannabinoid-containing products, together with their soporific effects, has led to a surge of interest of their potential therapeutic role in the management of some common sleep disorders, such as insomnia, sleep disordered breathing, and restless legs syndrome, and less common disorders such as narcolepsy and parasomnias. Although much of the pre-clinical and clinical data were derived from studies with relatively small sample sizes and limited by biases in assessment, and in clinical trials lack of allocation concealment, as a whole…

Sleep in Parkinson's Disease with Impulse Control Disorder.

PURPOSE OF REVIEW: This paper aims to explore the relationship between impulse-control disorders (ICDs) and sleep problems in patients with … t a risk factor for developing ICDs in PD patients. Moreover, REM sleep behavior disorder (RBD) and restless legs syndrome (RLS) have been indicated as independent risk factors for ICDs in PD patients. On the other hand, also ICDs may lead to sleep restriction and fragmentation, suggesting a bidirectional relationship. The association between sleep problems and ICDs in …

Mitochondrial ferritin in the regulation of brain iron homeostasis and neurodegenerative diseases.

Mitochondrial ferritin (FtMt) is a novel iron-storage protein in mitochondria. Evidences have shown that FtMt is structurally and functional … f neurodegenerative diseases, as its increased expression has been observed in Alzheimer's disease, restless legs syndrome and Friedreich's ataxia. Studies from our laboratory showed that in Alzheimer's disease, FtMt overexpression attenuated the β-amyloid induced neurotoxicity, which on the other hand increased significantly when FtMt expression was knocked down. It is …

Sleep problems in midlife and beyond.

Good sleep quality is important for good health, both physical and mental, and indeed for quality of life, performance and productivity. Sle … ition to reductions in sleep quality, specific sleep disorders, like sleep-disordered breathing and restless legs syndrome, become more prevalent in midlife and especially after menopause. Because sleep problems are commonly present in association with other conditions, rather than as isolated, independent disorders, treatment is often complex and patients generally nee …

Impact of dopamine agonists on compulsive behaviors: a case series of pramipexole-induced pathological gambling.

BACKGROUND: Dopamine agonists (DAs), long used in treating Parkinson's disease and effective in relieving symptoms of restless legs syndrome, have frequently been reported to induce problematic compulsive behaviors (e.g., obsessive gambling, hypersexuality) in individuals who had never had difficulties with such behaviors before. OBJECTIVE: The authors report two cases that add to a small-but-growing literature suggesting that these drugs be dispensed with appropriate caution. METHOD: The authors describe two patients seen in a psychiatric setting-one, after a suicide attempt, and one with d…

Treatment of hyperkinetic movement disorders.

Parkinson's disease, the most common hypokinetic movement disorder, has received much attention from the clinical and scientific community, but there has been a relative paucity of comprehensive reviews of hyperkinetic disorders, even though they are equally or even more disabling. Hyperkinetic movement disorders include tremors, dystonia, chorea, tics, myoclonus, stereotypies, restless legs syndrome, and various other disorders with abnormal involuntary movements. Substantial progress has been made in the understanding of the role of the basal ganglia in the pathophysiology of these hyperkine…

Periodic leg movements during sleep: phenotype, neurophysiology, and clinical significance.

Periodic leg movements during sleep (PLMS) are the most important objective finding in restless legs syndrome (RLS). During the last decade, PLMS have been very important for the assessment and comprehension of their pathophysiological correlates, which have been paralleled by the emergence of new computer-assisted and data-driven rules for their identification, scoring, and analysis. The present article focused on the most relevant PLMS-related findings of the last decade, and sought to provide a coherent and comprehensive overview on this enigmatic motor phenomenon. First, a clear descriptio…

The genetics of sleep disorders.

Most sleep disorders result from complex interactions between genes and the environment. Modern molecular techniques are increasingly applied to determine the contribution of genes to sleep and its disorders. The genetic basis of circadian rhythms has been explored using Drosophila and rodent models. This culminated in the identification of the molecular basis of one autosomal dominant form of familial advanced sleep phase syndrome: mutations in the human period 2 gene. Genetic studies in an autosomal recessive canine model of narcolepsy and in gene-targeted mice have identified the hypothalam
