CT arthrography and arthroscopy were used to assess tears of the rotator cuff in 259 shoulders. Tear size was determined in the frontal and sagittal planes according to the classification of the French Arthroscopy Society. CT arthrography had a sensitivity of 99% and a specificity of 100% for the diagnosis of tears of supraspinatus. For infraspinatus these figures were 97.44% and 99.52%, respectively and, for subscapularis, 64.71% and 98.17%. For lesions of the long head of the biceps, the sensitivity was 45.76% and the specificity was 99.57%. Our study showed an excellent correlation between

Shoulder sonography is a well-established and standardized method to evaluate the status of the rotator cuff, the long head of the biceps, and the subacromial and subdeltoid bursae. Using high-frequency linear probes, the sensitivity in detecting total tears sized 0.5 cm and greater is > 90% and in detecting partial thickness tears of at least one-third of the cuff substance > 75% when using the dual-criteria standard. Ultrasound achieves comparable or better results than native magnetic resonance tomography at far less cost. False results can be excluded in the majority of cases by strict adh

OBJECTIVE: Sonography and arthrography are techniques used to detect rotator cuff tears (RCT). The diagnostic value of sonography executed by a rheumatologist versus that of arthrography performed by a radiologist for assessment of RCT was investigated among patients with unilateral chronic shoulder complaints without an underlying inflammatory rheumatic disease.
METHODS: Forty-eight patients underwent sonography, arthrography, and surgical inspection, the results of surgical inspection being the gold standard.
RESULTS: Sensitivity for assessment of a full thickness RCT was 0.86 for sonograp

OBJECTIVES: Degenerative rotator cuff tears do not heal spontaneously, necessitating surgical intervention. This makes prevention crucial, but effective prophylactic measures are currently lacking. Oxidative stress has recently been implicated as a cause of degenerative rotator cuff tears, while mitochondrial injury has been reported in the development of age-related rotator cuff degeneration. Taurine, which has antioxidant properties, has been found to be effective in the treatment of various mitochondrial abnormalities. This prompted us to investigate the inhibitory effect of taurine and som

The shoulder and elbow are prone to many overuse injuries in baseball. Injury to the muscles or ligaments leads to pain and loss of effectiveness in competition. Although many of these disorders respond to conservative management, surgery is sometimes necessary to provide pain relief and restore function. Unfortunately, overuse injuries that require surgery still are career-threatening injuries. Correct diagnosis and treatment increase the probability of a throwing athlete returning to competition.

BACKGROUND: Rotator cuff tears are common. A previous systematic review reported on factors associated with rotator cuff tears; however, it included relatively few studies and few variables, and in addition, it had considerable heterogeneity. To identify the factors associated with symptomatic rotator cuff tears and to help guide clinicians to potentially modifiable factors, we felt a broader and more inclusive meta-analysis would be useful.
QUESTIONS/PURPOSES: In this systematic review and meta-analysis, we asked what (1) demographic, (2) disease, and (3) imaging factors are associated with

Inferior subluxation of the humeral head can occur after shoulder trauma or surgery. One hundred consecutive patients were evaluated prospectively after shoulder surgery or injury. The radiographic incidence of inferior subluxation of the humeral head 2 weeks after rotator cuff repair was 10%. The radiographic incidence of inferior subluxation after fracture of the proximal humerus was 42%, and the incidence 2 weeks after prosthetic humeral head replacement was 60%. The immediate postoperative radiograph showed an inferior subluxation of the humeral head in 4% of patients after prosthesis inse

Thirty-seven patients had surgical repair within three months after significant ruptures of the shoulder rotator cuff. Twelve were repaired within three weeks (Group 1), six within three to six weeks (Group 2), and 19 within six to 12 weeks after injury (Group 3). Follow-up periods averaged 7.0 years (range, 1.25-21.00 years). Pain relief was generally satisfactory; however, active postoperative abduction averaged 168 degrees for Group 1, 126 degrees for Group 2, and only 129 degrees for Group 3. At open operation, the tear size was graded as small, medium, or large. The correlation of average

BACKGROUND: To show that magnetic resonance imaging (MRI) can be useful to diagnose radiographically invisible greater tuberosity fractures in patients with clinical presentations suspicious for rotator cuff injury.
METHODS: Six patients with subtle greater tuberosity fractures who were sent for MRI because of possible rotator cuff tear are reviewed (age range, 27-51 years; mean, 40.5 +/- 8.4 years). Five of the patients recount direct trauma to their shoulders, whereas one noticed shoulder pain after a fall in which she fractured her ankle.
RESULTS: MRI showed an area of edema confined to t

BACKGROUND: Massive rotator cuff surgical management can be challenging. Controversy exists in the literature regarding the potential benefit of full head coverage for arthroscopic rotator cuff repair. The purpose of this study was to compare re-tear rate and clinical outcome of arthroscopic repair of massive rotator cuff tears in relation to the achievement of full humeral head footprint coverage or not. Hypothesis We hypothesized that there will be no difference in re-tear rate and clinical outcomes between full and partial coverage rotator cuff repairs.

PURPOSE: To investigate the relationship between aponeurotic expansion of supraspinatus tendon and disorders of biceps brachii long head (LHB), supraspinatus and subscapularis tendons.
METHODS: Images of 3.0 T shoulder magnetic resonance images (MRI) of a total of 154 patients and 157 shoulders (3 cases with bilateral imaging) between the ages of 18-45 were retrospectively evaluated for the presence of aponeurotic expansion. When identified it was further categorized according to its thickness. Tendinopathy and tears of LHB, supraspinatus and subscapularis tendons and LHB subluxations were ev

BACKGROUND: The aim of this study was to describe the mini-open suture bridge technique with porcine dermal patch augmentation for massive rotator cuff tear and to assess preliminary clinical and radiological results.
METHODS: Five patients with massive rotator cuff tear for which it was not possible to restore the anatomical footprint underwent mini-open suture bridge repair using a porcine dermal patch. The patients' average age was 53.4 years (range, 45 to 57 years), and the average duration of follow-up was 20.6 months (range, 14 to 26 months). Patients were evaluated with preoperative an

BACKGROUND: Transosseous-equivalent (TOE) rotator cuff repair has been theorized to be "self-reinforcing" against potentially destructive and increasing tendon loads. The goal of this study was to biomechanically verify and characterize the effect of increasing tendon load on frictional resistance over a repaired footprint for single-row (SR) and TOE repair techniques.
METHODS: In 10 fresh frozen human shoulders, TOE and SR supraspinatus tendon repairs were performed in each specimen. For all repairs, a pressure sensor was secured at the tendon-footprint interface. The supraspinatus tendon wa

OBJECTIVE: To evaluate the effectiveness of exercise in the treatment of people with subacromial impingement syndrome (SAIS).
METHODS: A systematic review and meta-analysis were conducted. Ten electronic databases were searched from the dates of their inception until August 2010. Included studies were randomized controlled trials investigating exercise in the management of SAIS. Outcomes were pain, strength, function, and quality of life. Data were summarized qualitatively using a best evidence synthesis. Treatment effect size and variance of individual studies were used to give an overall su

BACKGROUND: Surgical repairs of torn rotator cuff tendons frequently fail. Immobilization has been shown to improve tissue mechanical properties in an animal model of rotator cuff repair, and passive motion has been shown to improve joint mechanics in animal models of flexor tendon repair. Our objective was to determine if daily passive motion would improve joint mechanics in comparison with continuous immobilization in a rat rotator cuff repair model. We hypothesized that daily passive motion would result in improved passive shoulder joint mechanics in comparison with continuous immobilizatio

INTRODUCTION: The superior labral anterior and posterior (SLAP) lesion is a cause of chronic shoulder pain and significant disability. This study aims to review the distribution of types of SLAP lesions, associated lesions and the outcome of arthroscopic management.
MATERIALS AND METHODS: This series involved all 33 patients who underwent arthroscopic management of SLAP and associated lesions by a single surgeon. The pre- and postoperative disability scores were assessed using the disabilities of the arm, shoulder and hand (DASH) outcome measure.
RESULTS: Type 2 SLAP lesion (64%) was the mos

Sheep (and goats) are a convenient large-animal model for rotator cuff repair because of availability, ease of handling and housing, animal cost, and acceptance to society as a research animal. Tenotomy of the infraspinatus tendon and subsequent reattachment to the proximal humerus is useful to address the biomechanical, histologic, and biochemical processes of rotator cuff repair. Detaching this tendon and immediately reattaching it does not represent the clinical picture but serves as a relatively rapid way to screen different suture anchors, suture patterns, scaffolds, growth factors, and o

BACKGROUND: Rotator cuff tendinopathy is a common entity. We hypothesized that obesity, because of biomechanical and systemic risk factors, increases the risks of rotator cuff tendinitis, tears, and related surgical procedures.
METHODS: A frequency-matched case-control study was conducted. Three hundred and eleven patients who were fifty-three to seventy-seven years old and who underwent rotator cuff repair, arthroscopy, and/or other repair of the shoulder in a large hospital from 1992 to 2000 were included in the study. These surgical procedures were used as proxies for the risk of rotator c

The diagnosis of chronic lesions of the rotator cuff is challenging. We have developed a new index to improve the sonographic diagnosis of chronic tears of the cuff. In a pilot study, we examined 50 asymptomatic healthy volunteers by ultrasound to establish the diameter of the rotator cuff in relation to the tendon of the long head of biceps. Subsequently, the index was calculated in 64 patients who had had shoulder pain for more than three months caused by clinically diagnosed lesions of the rotator cuff. The compensatory hypertrophy of the biceps tendon was quantified sonographically in rela
