UNLABELLED: We present 20 patients with established proximal pole scaphoid nonunions treated with curettage and cancellous autograft from the distal radius and screw fixation. Fractures with significant proximal pole fragmentation were excluded. Patients were treated at a mean of 26 weeks after injury (range 12-72). Union occurred in 18 of 20 patients (90%) based on computed tomographic imaging. The two nonunions that did not heal were treated with repeat curettage and debridement and iliac crest bone grafting without revision of fixation. Union was achieved in both at a mean of 11 weeks after

BACKGROUND: Wrist injuries are common, and there is often diagnostic uncertainty following normal initial radiographs when there is ongoing clinical suspicion of a scaphoid fracture. The aims of this study were to define the problem in our hospital, and to identify current practice relating to the management of patients with clinically suspected scaphoid injury across hospitals in England.
METHODS: A retrospective review was undertaken of all patients presenting to our Emergency Department with a wrist injury, over a 12-month period.A cross-sectional survey of all hospitals in NHS England was

The integration of artificial intelligence (AI) in healthcare has sparked interest in its potential to revolutionize medical diagnostics. This systematic review explores the application of AI and machine learning (ML) techniques in diagnosing scaphoid fractures, which account for a significant percentage of carpal bone fractures and have important implications for wrist function. Scaphoid fractures, common in young and active individuals, require an early and accurate diagnosis for effective treatment. AI has the potential to automate and improve the accuracy of scaphoid fracture detection on

In fifteen cadaver wrists and ten foetus wrists palmar ligaments were studied. As a result, the palmar scapho-triquetral ligament was presented by means of gross anatomical study, MR imaging, and histological study. A kinematic study was conducted on three cadaver wrists to analyze patterns of motion on the proximal carpal row bones based on the helical axis concept. Motion analysis showed that the scaphoid has a set of helical axes bundled at the radial side of the wrist. On the contrary, the triquetrum had axes bundled on the ulnar side, approximately in the same position as the lunate. That

Scaphoid fractures are the most common fracture of the carpal bone and frequently occur in athletic populations due to acute trauma or repetitive wrist loading. Management of these injuries in athletes is challenging because of the need to balance reliable fracture healing with timely return to sport. This narrative review summarizes current literature regarding treatment strategies, healing outcomes, and return-to-sport timelines in athletes with scaphoid fractures. We conducted a literature search using PubMed, Scopus, and CINAHL between July 2025 and January 2026. A total of 862 records wer

PURPOSE: This study aimed to explore the feasibility and efficacy of percutaneous fixation of minimally displaced scaphoid waist fractures using a 3-dimensional-printed guide in 10 cases.
METHODS: Fractures were examined using preoperative computed tomography. The skin interface and bone models were reconstructed using computed tomography data. Guidewire insertion was assisted by a guide. Computed tomography was performed 4-6 weeks after surgery until healing of the fracture was confirmed. The mean follow-up period was 7 months (range, 6-9 months). The fracture healing time, grip strength, fl

BACKGROUND: Management of scaphoid nonunions with bone loss varies substantially. Commonly, internal fixation consists of a single headless compression screw. Recently, some authors have reported on the theoretical benefits of dual-screw fixation. We hypothesized that using 2 headless compression screws would impart improved stiffness over a single-screw construct.
METHODS: Using a cadaveric model, we compared biomechanical characteristics of a single tapered 3.5- to 3.6-mm headless compression screw with 2 tapered 2.5- to 2.8-mm headless compression screws in a scaphoid waist nonunion model.

INTRODUCTION: The Australian Health Department provided extended rebatable requesting rights to general practitioners in 2012 for magnetic resonance imaging (MRI) examinations in patients less than 16 years of age for a small set of clinically appropriate indications. Included item numbers 63522 and 63523 'referral by a medical practitioner (excluding a specialist and consultant physician) for a scan of wrist following radiographic examination where scaphoid fracture is suspected'. The aim of this study is to evaluate MRI examinations of suspected scaphoid fractures in the paediatric populatio

BACKGROUND: A detailed understanding of scaphoid anatomy helps anatomic fracture reduction, and optimal screw position. Therefore, we analysed (1) the size and shape variations of the cartilage and osseous surface, (2) the distribution of volumetric bone mineral density (vBMD) and (3) if the vBMD values differ between a peripheral and a central screw pathway?
METHODS: Forty-three fresh frozen hand specimens (17 females, 26 males) were analysed with high-resolution peripheral quantitative computed tomography (HR-pQCT) and dissected to compute a 3D-statistical osseous and cartilage surface mode

The vascularized medial femoral condyle bone graft has many clinical applications. It can be harvested as a thin, pliable, corticoperiosteal graft and wrapped around recalcitrant nonunions in long bones to achieve osseous union. It can also be harvested as a small, structural, corticocancellous graft and used to treat small defects such as scaphoid avascular nonunion. We present a case of thumb metacarpal osteomyelitis resulting in a large segmental osseous defect. Reconstruction was performed using a large, structural, vascularized bone graft from the medial femoral condyle.

Fractures of the scaphoid are the most common carpal bone fracture. With careful management, prognosis is excellent. Imaging is critical, not only for early diagnosis, but also for monitoring patients after initial treatment to detect unstable fractures, the common complications of nonunion and avascular necrosis, and the need for surgical treatment.

We describe a rare anatomical variant, the extensor indicis brevis--a "short" index extensor originating from the ligament over the scaphoid bone. In our patient it presented as a tender mass. The knowledge of such anatomical variations is important in diagnosing dorsal hand masses and in planning tendon transfers.

Background Headless compression screw fixation with bone grafting has been the mainstay of treatment for scaphoid nonunion for the past several decades. Recently, locked volar plate fixation has gained popularity as a technique for scaphoid fixation, especially for recalcitrant or secondary nonunions. Purpose The purpose of this meta-analysis was to compare union rates and clinical outcomes between locked volar plate fixation and headless compression screw fixation for the treatment of scaphoid nonunions. Methods A literature search was performed for studies documenting treatment outcomes f

OBJECTIVES: During a multicentre study on juvenile idiopathic arthritis, wide variations were observed in bone shape, signal intensity and volume of joint fluid as shown by MRI which in part appeared to be unrelated to disease activity. A study was undertaken to examine these features in a cohort of healthy children.
METHODS: 88 children of mean age 9.8 years (range 5-15) underwent MRI imaging (T1-weighted Spin Echo and Spectral Selection Attenuated Inversion Recovery (SPAIR)) of the left wrist. The number of bony depressions, distribution and amount of joint fluid and the presence of bone ma

BACKGROUND: Proximal pole scaphoid fracture is not uncommon and present therapeutic challenges due to impaired perfusion, with risk of secondary non-union, avascular necrosis and wrist degeneration. There is no consensus on surgical techniques for a non-salvageable proximal pole, but proximal hamate autograft to resurface the scaphoid is gaining interest. This systematic review assesses the evidence for proximal hamate autograft in resurfacing non-salvageable scaphoid proximal poles.
METHODS: A systematic review searched the Medline, Embase, PubMed and Scopus databases for the period 2000-202

Avascular necrosis (AVN) of the carpal bones other than Kienböck disease is a rare cause of chronic pain of the wrist. The scaphoid, capitate, and a smaller percent of lunates are at greater risk to develop AVN because of their peculiar intraosseous circulation; however, AVN has been reported in all the carpal bones. On the plain radiograph, AVN of the carpal bones may evolve through a normal radiograph, sclerosis, fragmentation, cyst formation, fracture, bone collapse, change of the carpal height, and alignment and progressive degenerative changes of the wrist joint. In the early stage of AVN

BACKGROUND: The optimal management of pediatric scaphoid fracture nonunions is controversial. We hypothesize that pediatric patients with scaphoid fracture nonunions will have favorable functional outcomes with the utilization of nonvascularized distal radius cancellous autograft with open reduction and internal fixation (ORIF).
METHODS: A review was performed from 2012 to 2017 identifying skeletally immature patients with scaphoid fracture nonunions treated with ORIF and nonvascularized distal radius cancellous autograft, including demographic data, mechanism of injury, length of time from i
