Results of the monitor observation of the recto-skin temperature gradient in 263 patients are described. Among them 199 patients were in critical condition (traumatic shock--in 51; burn--in 99; cardiogenic shock--in 33; septic collapse--in 11). The elevation of this parameter more than 9 degrees C shows the presence of shock. It can be used for control of the efficiency of the antishock therapy and represents a simple, reliable and highly informative method not requiring an expensive equipment.

It was established that the agonists of opiates (enkephalin) relieve the course of traumatic shock while the antagonists (diprenophrine) aggravate it. The favorable effect of the agonists of opiates is due to optimization of the relation between the requirements of the organism in oxygen and its supply. The antagonists of opiates aggravate shock due to increased requirements in oxygen in decrease of its transport.

We present a case of a patient with thyroid storm exhibiting symptoms similar to those of traumatic injury. This patient presented multiple traumatic wounds on his limbs and face, loss of consciousness, and tachycardia. Diagnosis was initially confounded by the similarity between the symptoms of traumatic hypovolemic shock and trauma-induced thyroid storm. This case report discusses the factors leading to the diagnostic delay and highlights the dangerous manifestations of thyroid storm.

Thirteen confirmed cases of toxic shock syndrome temporally related to use of the vaginal contraceptive sponge have been reported. The observed risk of toxic shock syndrome in sponge users may be elevated above estimated background rates, but this risk remains very low. Traumatic manipulation of the sponge, use during menstruation or the puerperium, and prolonged retention of the sponge may additionally increase toxic shock syndrome risk. As with all contraceptives, risks must be balanced against benefits.

In contrast to the inability to demonstrate endotoxin detoxifying activity in either control rat blood, blood cells, plasma, or serum, the induction of mild endotoxemia or trauma resulted in a prompt and marked detoxifying activity in blood plasma or serum, but not in the cellular elements. The functional significance of the blood anti-endotoxin system was demonstrated by a capability for passive transfer and by its loss during severe traumatic shock. A role for the RES in elaboration of the blood anti-endotoxin system is postulated.
