Gene of the month: APOL1.

Apolipoprotein L1 (APOL1) is a protein encoded by the APOL1 gene, found only in humans and several primates. Two variants encoding two different isoforms exist for APOL1, namely G1 and G2. These variants confer increased protection against trypanosome infection, and subsequent African sleeping sickness, and also increase the likelihood of renal disease in individuals of African ancestry. APOL1 mutations are associated with increased risk of chronic kidney disease, inflammation, and exacerbation of systemic lupus erythematosus-associated renal dysfunction. This review serves to outline the stru

The severity of psychiatric disorder: a replication.

A prior report documented the importance of diagnosis and psychotic symptoms as components of a global rating of severity of psychiatric disorder. The availability of 2-year followup data on 174 (80%) of the original 217 patients permits examination of the stability of components of a global rating. Menninger Health-Sickness Rating Scale (MHRS) scores again correlated with diagnosis and with psychotic symptoms; unlike the initial report, followup MHRS ratings were closely associated with a depression-neurosis factor and social role performance. Age, race, and sex were independent of severity,

Tachy-brady syndrome: Electrophysiology and evolving principles of management.

Sudden alterations in the heart rate may be associated with diverse symptoms. Sinus node dysfunction (SND), also known as sick sinus syndrome, is a sinoatrial (SA) node disorder. SND is primarily caused by the dysfunction of the pacemaker, as well as impaired impulse transmission resulting in a multitude of abnormalities in the heart rhythms, such as bradycardia-tachycardia, atrial bradyarrhythmias, and atrial tachyarrhythmias. The transition from bradycardia to tachycardia is generally referred to as "tachy-brady syndrome" (TBS). Although TBS is etiologically variable, the manifestations rema

Pacing for the prevention of atrial fibrillation.

Nonpharmacological methods are being evaluated for the prevention of atrial fibrillation. The role of atrial pacing has been extensively examined in a large number of retrospective and prospective studies, and recently in randomized multicenter trials. The populations examined have varied from patients with sick sinus syndrome with incidental AF, to patients with drug refractory AF. In addition to standard atrial pacing from the high right atrium, pacing from novel sites like the interatrial septum, Bachman bundle, and dual-site or biatrial pacing have been employed for the prevention of AF. Plasma atrial natriuretic peptide and spontaneous diuresis in sick neonates.

Plasma concentrations of immunoreactive human atrial natriuretic peptide (human ANP) were sequentially determined in 12 infants with respiratory distress syndrome (RDS) or meconium aspiration syndrome (MAS) during various phases of diuresis to elucidate the role of human ANP in the occurrence of spontaneous diuresis in the newborn. Plasma immunoreactive ANP concentrations during the diuretic as well as the maximum diuretic phase were significantly (p less than 0.001) higher than during the prediuretic phase. A gradual decrease occurred during the post diuretic phase, returning to prediuretic v

Meta-cognitions in tourette syndrome, tic disorders, and body-focused repetitive disorder.

OBJECTIVE: To explore if self-reported presence of thinking about tics or body-focused repetitive behaviours (BFRBs; gests) are direct triggers of tic or gest onset in 3 groups: Tourette syndrome (TS; n =18), persistent chronic tic disorders (TDs; n = 42), and a comparison group with BFRB (n = 36). METHOD: The 3 groups completed a thinking about tics inventory, listing 22 items derived from clinician consensus that asked whether thoughts always, sometimes, or never exclusively triggered tic onset. Other questionnaires measured mood, perfectionism, impulsivity, premonitory urge, and self-rated

The lived experience of alopecia areata: a qualitative study.

This study explored experiences of individuals living with alopecia areata (AA) and alopecia universalis (AU) and investigated their accounts of adjusting to, and coping with, such conditions. Whilst previous research has primarily focused on the adverse psychosocial impact of alopecia, this investigation used Interpretive Phenomenological Analysis to provide a more holistic perspective. Biographical interviews were undertaken with 12 participants (7 women; 5 men) diagnosed with alopecia for periods ranging between 2 and 49 years. Results revealed that strategies used by participants evolved o

[A rare differential diagnosis of a somatoform autonomous disorder of the gastro-intestinal tract: the hepatocellular liver carcinoma in childhood].

OBJECTIVES: A severely ill 11-year-old boy came to the child psychiatric outpatient department of the Fachkrankenhaus Neckargemünd with the diagnosis of a somatoform disorder. Main symptoms included nausea, vomiting, abdominal pain and diarrhoea. He wished to be examined in order to enter the Stephen-Hawking-School for physically handicapped children in the town of Neckargemünd. RESULTS: Manual examination revealed a palpable mass in the right upper quadrant of the abdomen, which was imaged as a tumour of the liver. During the subsequent operation, a 500 ml hepatocellular carcinoma was found.

When tinnitus loudness and annoyance are discrepant: audiological characteristics and psychological profile.

This study evaluates sociodemographic and clinical characteristics of patients reporting discrepant levels of tinnitus loudness and annoyance. 4958 subjects recruited from a national tinnitus association completed a comprehensive screening questionnaire including Klockhoff and Lindblom's loudness grading system and the psychometric Mini-TQ (Tinnitus Questionnaire). There was a moderate correlation of 0.45 between loudness and annoyance. Of the subjects reporting very loud tinnitus, about one third had only mild or moderate annoyance scores. They were not different from those with high annoyanc

Does distress tolerance moderate the impact of major life events on psychosocial variables and behaviors important in the management of HIV?

Living with HIV involves management of multiple stressful disease-related and other life events. Distress tolerance may provide a functional, individual-based context for qualifying the established relationships between major life events and psychosocial variables important in the management of HIV. The present study provided a preliminary test of the hypothesis that distress tolerance moderates the impact of major life events on these predictors of disease progression. HIV-positive patients (n=116) completed psychosocial and medical questionnaires. Results indicated that major life events int

A case study of telephone interpersonal counseling for women with breast cancer and their partners.

PURPOSE/OBJECTIVES: To present a case study of one woman with breast cancer and her partner to provide a firsthand account of an innovative telephone interpersonal counseling intervention. DATA SOURCES: Journal articles, book chapters, research data, and transcriptions of telephone counseling sessions. DATA SYNTHESIS: Substantial evidence exists that face-to-face psychosocial interventions improve psychological adjustment and health-related quality of life for patients with cancer. Yet psychosocial interventions are not offered routinely, and many patients with cancer do not use face-to-face

[The initial dysphoric reaction (IDR) to the first dose of neuroleptics].

Initial dysphoric response (IDR) is characterized by a negative affective reaction within 48 h after a neuroleptic test dose. Currently, there are three scales for measuring the IDR (Neuroleptic Dysphoria Scale, Dysphoric Response Index, Drug Attitude Inventory). The debate continues about the definition, measurement, and underlying aetiology of IDR. Nevertheless, with a prevalence of 30%, the response is a clinically relevant phenomenon. Empirical data give some evidence of an association between IDR and short-term outcome. No correlation of IDR with pretreatment variables has been found yet

New-onset psychogenic seizures after intracranial neurosurgery.

BACKGROUND: Patients with physical brain abnormalities have an increased risk of developing psychogenic nonepileptic seizures (PNES). Here we describe patients who developed PNES after intracranial neurosurgery for indications other than the control of refractory epileptic seizures and explore whether neurosurgical intervention is at risk factor for PNES. METHOD: We searched the database of 372 patients diagnosed with PNES at our department over the last 10 years and identified 17 patients (4.6%) in whom PNES first started after intracranial neurosurgery. Surgical procedures included the comp

Parental health attitudes, illnesses and supports and the referral of children to medical specialists.

Parental illness and maternal health beliefs in the parents of a group of 40 children referred to a paediatric neurology service because of headaches were compared with illness histories and health beliefs in the parents of a matched group of 40 children presenting to the same service with recognized structural pathology, mainly epilepsy. The study focused on the mother's perceptions of her own health (current and past), on life events, social supports and on her beliefs about her own health and the validity of medical reassurance. Significant differences between the two groups were found in r

Sickness behavior in mice deficient in interleukin-6 during turpentine abscess and influenza pneumonitis.

Interleukin-6 (IL-6), among other cytokines, is thought to be involved in the regulation of sickness behavior (e.g., anorexia, cachexia, fever, and lethargy) induced by infections bacterial and viral origin) and sterile tissue necrosis (burns and surgical traumas). Mice deficient in IL-6 (IL-6 KO) were generated by gene targeting. Homozygous IL-6 KO male and female mice and their appropriate controls were implanted with biotelemeters to monitor body temperature (Tb) and motor activity (Act). Normal circadian rhythms in Tb and Act as well as rates of food intake and weight gain did not differ s

Psychosocial and cognitive factors associated with adherence to dietary and fluid restriction regimens by people on chronic haemodialysis.

BACKGROUND: Failure by people on chronic haemodialysis to adhere adequately to dietary and fluid restrictions can have serious medical consequences. Numerous psychosocial factors possibly associated with adherence have been investigated in previous research. However, most previous studies have examined one or a few variables in isolation, and have tended to focus on sociodemographic variables not easily amenable to intervention. Much previous work has tended to ignore potential differences in adherence between male and female dialysands. METHODS: Sociodemographic and psychosocial factors asso
