Safety profile of superior petrosal vein (the vein of Dandy) sacrifice in neurosurgical procedures: a systematic review.

OBJECTIVE Walter E. Dandy described for the first time the anatomical course of the superior petrosal vein (SPV) and its significance during surgery for trigeminal neuralgia. The patient's safety after sacrifice of this vein is a challenging question, with conflicting views in current literature. The aim of this systematic review was to analyze the current surgical considerations regarding Dandy's vein, as well as provide a concise review of the complications after its obliteration. METHODS A systematic review was performed according to Preferred Reporting Items for Systematic Reviews and Meta

[Intracranial chondroma arising from the skull base: two case reports featuring the image findings for differential diagnosis].

We reported two cases of intracranial skull base chondroma and discussed the differential diagnosis and the treatment strategies. The first case was a 39-year-old male who presented with left exophtalmos, visual loss and oculomotor disturbance. MRI showed a huge tumor occupying the bilateral cavernous sinus. Partial removal of the tumor was performed through the left orbitozygomatic subtemporal approach. The second case was a 54-year-old male who presented with left hemiparesis. MRI showed a brain stem infarction with a huge tumor located at the right middle fossa. Partial removal was performe

Mature teratoma arising from the middle cranial fossa.

A 25-year-old man presented with a sudden spurt in size of a bony swelling around the external ear, which was present since birth. This was associated with a painful swelling of the right half of the face and pain, redness, and decreased vision in the right eye. Neurologic examination showed decreased visual acuity in the right eye and paresis of the fifth, seventh, and eighth cranial nerves. Neuroimaging showed an extra-axial, partly cystic tumor of mixed density, located entirely within the middle cranial fossa scalloping the temporal bone and extending into the right infratemporal fossa and

Simultaneous combined endonasal and transcranial surgery for large skull base tumors invading multiple regions: combination variations, pros and cons, and surgical results.

Simultaneous combined endonasal and transcranial surgery (SCENaTCS) is an accepted treatment option for giant pituitary tumors to prevent postoperative intratumoral hemorrhage from residual tumors. For various skull base pathologies, especially tumors invading multiple regions, SCENaTCS may be applicable, with advantages equal to or greater than those of staged surgeries using either the same or a different surgical approach. Twenty-six consecutive SCENaTCS procedures for 24 patients with highly complex skull base tumors were classified into following four types on anatomical basis: Type 1 (n

Two Cases of Isolated Intracranial Aspergilloma Mimicking Convexity and Skull Base Meningioma: Diagnostic and Radiological Dilemmas.

BACKGROUND: Intracranial aspergilloma is a rare fungal infection of the central nervous system, predominantly affecting immunocompromised individuals. However, cases in immunocompetent hosts are increasingly reported, often mimicking meningiomas due to similar radiological features, leading to diagnostic delays and poor outcomes. CASE PRESENTATION: We present two cases of isolated intracranial aspergilloma in immunocompetent patients. The first case involved a 55-year-old female with a right parietal scalp swelling mimicking a convexity meningioma, confirmed as aspergilloma on histopathology

Comprehensive analysis of the distribution of dural artery-derived tumor-feeding arteries in intracranial meningiomas.

OBJECTIVE: Knowledge of the location of tumor-feeding arteries is necessary for the safe surgery of intracranial meningiomas. Hence, this retrospective study aimed to comprehensively analyze the distribution of tumor-feeding arteries. METHODS: Patients who underwent intracranial meningioma surgery at our institution between 2015 and 2023 were included in this study. The tumor attachment sites and tumor-feeding arteries were evaluated based on the results of preoperative examinations. The tumor attachment sites were classified as non-skull bases (convexity, parasagittal, and falx) or skull bas

Applications of Endoscopic Endonasal Surgery in Early Childhood: A Case Series.

INTRODUCTION: Endoscopic endonasal surgery (EES) has been slower to gain popularity in early childhood due to anatomical challenges. We sought to describe the safety and efficacy of EES in early childhood. METHODS: All patients younger than 7 years who underwent EES at a large Cranial Base Center from 2002 to 2019 were reviewed as a retrospective cohort study. RESULTS: Thirty-six patients underwent EES before the age of 7 years. Four patients had two-stage EES. Two patients required combined transcranial and endonasal approaches. The mean age at the time of initial surgery was 4 years (range

TRANS-FRONTAL SINUS APPROACH FOR OLFACTORY GROOVE MENINGIOMAS: A 19 YEAR EXPERIENCE.

INTRODUCTION: Olfactory groove meningiomas (OGMs) account for 8-13 % of all intracranial meningiomas. The gold standard of treatment is generally surgery, however various approaches have been used and the literature is still uncertain about the superiority of one treatment over the others. The most debated techniques are traditional microscopic open approaches versus the endoscopic endonasal ones. The aim of this paper is to prove a valid surgical route that gathers the benefits of both endoscopic and transcranial routes. METHODS: Fifty consecutive patients underwent trans-frontal sinus surgi

Papillary meningioma with vital abnormal vessels.

Papillary meningioma is an uncommon meningioma subtype of World Health Organization grade III. It could show some radiologic profiles pointing to malignant behavior, such as a cystic change, a heterogeneous enhancement, and an ill- defined border. However, to date, the radiologic profile described in this article has not been reported in previous literatures, and it is just the characteristic being considered as the major cause for patients' death. A 16-year-old adolescent boy with a 6-month history of headache was admitted to our department on June 28, 2012. Magnetic resonance imaging showed
