Comparison of ictal SPECT and interictal PET in the presurgical evaluation of temporal lobe epilepsy.

We retrospectively compared ictal technetium 99m hexamethylpropyleneamineoxime single-photon emission computed tomography (SPECT) and interictal 18F-fluorodeoxyglucose positron emission tomography (PET) in 35 patients with well-lateralized temporal lobe epilepsy (TLE). Based on SPECT scans the two observers correctly lateralized seizure foci with certainty in 89% of patients; interobserver agreement was excellent. Both observers incorrectly lateralized the seizure focus on two SPECT scans; one error was explained by rapid electroencephalographic spread to the contralateral side and for the oth…

Amyloidosis secondary to rheumatoid arthritis associated with plexiform change in bilateral temporal lobes.

A 70-year-old woman with rheumatoid arthritis (RA) and secondary amyloidosis presented repeated consciousness loss. The pathological findings at autopsy revealed multi-organic deposits of amyloid A-protein and so-called 'plexiform change' of blood vessels in bilateral temporal lobes. The arterial plexiform change, which is found in the lung specimen of primary pulmonary hypertension, might be a new pathological cerebrovascular change associated with RA.

Reduction of herniated temporal lobe in patients with severe head injury and uncal herniation.

Thirty-two patients with severe head injury (Glasgow coma scale 5-7/15) and uncal herniation were treated surgically from January 1988 to June 1990. Reduction of the herniated temporal lobe in addition to classical surgical procedures (craniotomy with evacuation of hematoma and resection of the contused brain) was performed in 10 patients (group A). The remaining 22 patients (group B) were treated similarly but without reduction of the herniated temporal lobe. In group A, there was one operative mortality. Eight patients made a rapid and complete recovery of pupil size and light reflex within …

Memory loss due to transient hypoperfusion in the medial temporal lobes including hippocampus.

A typical case of transient global amnesia (TGA) was investigated with single photon emission computed tomography (SPECT) and magnetic resonance imaging (MRI) with 1.5-tesler scans. During the amnesic episode, a marked decrease of cerebral blood flow was observed in the areas confined to the territory of the bilateral posterior cerebral arteries including the hippocampus. After the episode, cerebral blood flow returned to normal and a circumscribed lesion was detected in the middle portion of CA 1 field of the left hippocampus. The SPECT findings prove direct evidence that the medial temporal

[A case of duplication of the middle cerebral artery associated with arteriovenous malformation of the temporal lobe].

A rare case of duplication of the middle cerebral artery associated with an arteriovenous malformation of the left temporal lobe is reported. A 45-year-old man was admitted to our clinic on January 7, 1985 with a history of sudden onset of headache and nausea, followed by loss of consciousness for 20 minutes. Neurological signs were negative on admission. An arteriovenous malformation of the left temporal lobe fed by a duplicated middle cerebral artery and the original MCA were demonstrated on left carotid angiography. No other vascular lesion was observed on the angiograms. The arteriovenous …

[Organization of sleep in recent temporal lobe epilepsy before and after treatment with carbamazepine].

Sleep in epileptic patients with complex partial seizures has been considered as being less well organized than in normal subjects. This study attempted to precise whether this disorganization could be related to the length of the disease and could be improved by carbamazepine treatment. The study was performed in 15 patients with recently diagnosed (less than 3 months) and untreated temporal lobe epilepsy. Neurologic examination and CT scan were normal. Patients did not present a generalized seizure in the preceding 48 hours and during the study. Nocturnal sleep polygraphic recordings were pe…

Dissociative states with abnormal temporal lobe EEG. Multiple personality and the illusion of possession.

Twelve patients with clinical and EEG manifestations reminiscent of temporal lobe epilepsy reported symptoms of dissociative states. In seven of these patients, the clinical picture was consistent with multiple personality, whereas the other five had the illusion of supernatural possession. These cases suggest that in selected instances dissociative states may constitute complex behavioral manifestations of chronic limbic epilepsy.

Emotion recognition deficits in temporal lobe epilepsy are non-lateralized and independent of surgery.

UNLABELLED: To examine whether patients with unilateral temporal lobe epilepsy (TLE) show deficits in recognizing emotions from facial expressions and vocal prosody, and whether these impairments differ by seizure focus or surgical status. METHODS: Thirty-four patients with TLE were evaluated before or after temporal lobe resection for drug-resistant epilepsy. Thirty right-handed healthy controls without neurological or psychiatric history were included. Emotion recognition was assessed using the Emotion Recognition Test (ERT), consisting of a Facial ERT (adapted from Ekman and Friesen's faci…

The role of SCN1A mutations in temporal lobe epilepsy: Genetic insights and clinical implications.

PURPOSE: Temporal lobe epilepsy (TLE) is the most common focal epilepsy, yet the genetic mechanisms underlying its development remain incompletely understood. The sodium channel gene SCN1A, known to be involved in several epilepsy syndromes including Dravet syndrome and genetic epilepsy with febrile seizures plus (GEFS+), has recently been investigated for its potential role in TLE. This systematic review aimed to synthesize evidence on the role of SCN1A variants in the pathogenesis, clinical features, and treatment outcomes of TLE. METHODS: A comprehensive literature search of PubMed, Scopus…

Cognitive reserve and memory in temporal lobe epilepsy and caregivers: a comparative pilot study.

This pilot study analysed differences in cognitive reserve and memory between patients with temporal lobe epilepsy (TLE) and another chronically stressed population - caregivers of patients with epilepsy - and explored the relationships between these variables. In this cross-sectional study, 40 adults (20 patients with left TLE and 20 caregivers; mean age: 48.43 ± 8.86 years) underwent a neuropsychological assessment that examined cognitive reserve proxies (i.e. years of education, total intelligence quotient, and scores in the information subtest of the Wechsler Adult Intelligence Scale) and …

Mechanisms contributing to the age at onset of temporal lobe epilepsy due to hippocampal sclerosis.

Temporal lobe epilepsy (TLE) secondary to hippocampal sclerosis (HS) is a common cause of drug-resistant epilepsy. HS is characterized by neuronal loss in selected hippocampal subfields and gliosis. Age at epilepsy onset (AEO) critically influences disease severity and treatment response. We compared early AEO (<10 years) and late AEO (>11 years) in hippocampi from patients undergoing surgery for HS (n = 30). HS hippocampi showed altered mitochondrial enzyme activities and membrane potential (vs controls). Early AEO showed reduced membrane potential and ATP and increased proton leak, whereas o…

Lack of context modulation in human single neuron responses in the medial temporal lobe.

In subjects implanted with intracranial electrodes, we use two different stories involving the same person (or place) to evaluate whether and to what extent context modulates human single-neuron responses. Nearly all neurons (97% during encoding and 100% during recall) initially responding to a person/place do not modulate their response with context. Likewise, nearly none (<1%) of the initially non-responsive neurons show conjunctive coding, responding to particular persons/places in a particular context during the tasks. In line with these findings, taking all neurons together it is possible

Altered Effective Connectivity of the Attentional Network in Temporal Lobe Epilepsy with EEG Data.

Existing studies have shown that the attentional function of epilepsy is prone to be impaired. However, the characterization of brain connectivity behind this impairment remains uncertain. This study investigates attention-related brain connectivity in 92 patients with temporal lobe epilepsy and 78 healthy controls using a 32-channel EEG monitor during an attention network test. Compared to controls, patients showed reduced temporal-occipital connectivity in the alerting and orienting networks, but increased frontal-occipital connectivity in the executive network. Additionally, this study show…

Fear perception as a function of hemisphere- and time-specific dynamics in the medial temporal lobes.

The medial temporal lobes (mTL) are thought to enhance visual processing of fearful faces, yet the underlying mechanisms remain underspecified. To fill this gap, we recorded and compared event-related potentials (ERPs) and stimulus-induced gamma-band activity (GBA) from 36 patients with left- or right-hemispheric antero-medial temporal lobe resections including the amygdala (lTLR/rTLR) and 18 healthy controls. Only rTLR patients were found to lack fear-neutral differentiation in early P1 amplitudes (~100 ms) and exhibited heightened GBA for neutral faces over ipsi-resectional occipito-temporal…

Early impairment in dentate pattern separation in a rodent model of temporal lobe epilepsy.

The dentate gyrus of the hippocampal formation utilizes sparse neural coding to support memory formation. In temporal lobe epilepsy (TLE), the dentate undergoes extensive restructuring, including loss of local inhibition. It is unknown how circuit changes impact dentate network computations in awake behaving animals. Here we perform high-density tetrode recordings from the dentate in male rats treated with systemic kainic acid (KA) to induce TLE. In KA-treated rats, dentate spatial representations were less precise at the single neuron level, however the stability of the spatial code in repres…

Unveiling the Hidden Culprit: A Case of a Temporal Lobe Abscess Presenting With Nonspecific Symptoms.

A brain abscess is a potentially life-threatening condition in which a collection of pus forms in the brain tissue due to an infection. Mastoiditis and chronic otitis media are among the leading causes of brain abscesses; however, with modern advancements in imaging technology and antibiotic treatments, the incidence of such cases has significantly declined. This report describes a 72-year-old patient with no co-morbidities and a recent travel history presenting with respiratory symptoms, a short duration of ear symptoms, and confusion, who was subsequently diagnosed to have a temporal lobe ab…

Neuroprotective Effects of Anodal tDCS on Oxidative Stress and Neuroinflammation in Temporal Lobe Epilepsy.

Background: Epilepsy affects over 50 million people worldwide, and about 30% remain drug-resistant-underscoring the urgent need for new therapies. This study evaluated the neuroprotective effects of anodal transcranial direct current stimulation (tDCS) in PTZ-induced epilepsy at acute and chronic stages in rats. Methods: Sixty male Wistar Albino rats (12 per group) were randomly assigned to five groups: control, acute epilepsy, acute epilepsy+ tDCS, chronic epilepsy, and chronic epilepsy+ tDCS. Behavioral tests-including the open-field, novel-object recognition, and Y-maze-assessed locomotion,

Effects of MeCP2 on chronic seizures and cognitive function in mice with temporal lobe epilepsy.

Mutations in methyl CpG binding protein 2 (MeCP2) are linked to Rett syndrome, in which epilepsy is one of the most well-described disorders. However, little is known about the specific role of MeCP2 during epileptogenesis. Our previous study has demonstrated that MeCP2 has a unique control on the development of mossy fiber sprouting (MFS) in the epileptic hippocampus. This study aimed to (1) examine whether MeCP2 affects spontaneous recurrent seizures (SRSs) and cognitive deficits in mice with pilocarpine-induced epilepsy, and (2) profile MeCP2's downstream molecular events. In the dentate gy
