Extensor indicis proprius transfer for loss of extensor pollicis longus function.

In the period 1969-1991, 46 extensor indicis proprius tendon transfers were performed for functional loss of extensor pollicis longus. The long-term function of the thumb was reviewed in 22 patients with an average follow-up of 7 years. Based on Geldmacher's evaluation scheme for assessment of the results of extensor tendon reconstruction, we report 5 excellent (23%), 4 good (18%), 12 satisfactory (55%) and 1 poor (4%) result. The mean loss of pinch strength was 8% compared with the contralateral, non-operated thumb. Subjectively, the majority of the patients (86%) described no limitations in …

Medial displacement calcaneal tuberosity osteotomy in the treatment of posterior tibial insufficiency.

The loss of function of the posterior tibial tendon has been associated with a progressive deformity in adults, resulting in a painful flatfoot. Patients who have a painful flatfoot usually develop a valgus deformity of the hindfoot and an abduction deformity of the forefoot. If these deformities are supple, a medial displacement calcaneal tuberosity osteotomy together with a soft tissue repair by talonavicular capsulorraphy and repair of the spring ligament associated with a flexor digitorum tendon transfer to the posterior tibial can result in a satisfactory outcome.

Two-stage extensor tendon reconstruction after composite tissue loss from the dorsum of the hand.

Restoration of digital extension after chronic extensor loss has not been detailed extensively in the literature. The present report details an unusual case of composite tissue loss from the dorsum of the hand after a chronic burn wound. After debridement for chronic carpal osteomyelitis and free-tissue transfer were performed, staged wrist fusion and two-stage extensor tendon reconstruction resulted in a stable, pain-free wrist and functional digital extension. The present case illustrates that two-stage extensor tendon reconstruction, when necessary, is indeed feasible.

Effects of posterior tibial tendon augmented with biografts and calcaneal osteotomy in stage II adult-acquired flatfoot deformity.

Adult-acquired flatfoot deformity (AAFD) is a well-known condition leading to flexible flatfoot deformity. However, only recently have the function and muscle strength for balancing opposing muscles been more appreciated in laboratory studies. With the advancements in collagen science in tendon structure, the rationale and concept of some of the most common procedures in tendon transfer have been challenged. The current availability of biograft technology has provided an alternative in augmentation procedures instead of sacrificing autologous tendons. This novel approach may offer a viable opt…

Intratendinous ganglion cyst of extensor indicis propius tendon: A case report.

INTRODUCTION AND IMPORTANCE: The intratendinous ganglion cyst is an extremely rare entity. Thus, the global incidence has not been reported yet. The literature search showed scarce case reports and none has reported its occurrence in extensor indicis propius (EIP) tendon. The benign nature and the region at the dorsal hand mimic to the more common dorsal wrist ganglion. However, the surgical treatment arises significant risk to the function and may require subsequent tendon graft or tendon transfer. CASE PRESENTATION: A 51-year-old-female presented with 4 years history of a slow-growing lump …

Shoulder biomechanics in normal and selected pathological conditions.

The stability of the glenohumeral joint depends on soft tissue stabilizers, bone morphology and dynamic stabilizers such as the rotator cuff and long head of the biceps tendon. Shoulder stabilization techniques include anatomic procedures such as repair of the labrum or restoration of bone loss, but also non-anatomic options such as remplissage or tendon transfers.Rotator cuff repair should restore the cuff anatomy, reattach the rotator cable and respect the coracoacromial arch whenever possible. Tendon transfer, superior capsular reconstruction or balloon implantation have been proposed for i…

Reconstruction of the Achilles tendon region by free microvascular flaps. 9 cases followed for 1-9 years.

In 1981-89, 9 patients underwent reconstruction for complex injuries in the Achilles tendon region. 10 free microvascular flaps were used: 5 fasciocutaneous and 5 muscle or musculocutaneous flaps. In addition, 4 Achilles tendons and 1 tibial posterior nerve were reconstructed, 1 femoropopliteal bypass was performed, and 6 tibial fractures were treated. The patients were re-examined on an average 3.5 years after the reconstruction. The stability of soft tissues was good in all patients. Good contour was achieved in superficial defects with fasciocutaneous and in deep injuries with latissimus do

Extensor tendon rupture after osteoarthrosis of the wrist associated with nonrheumatoid positive ulnar variance.

Rupture of the extensor tendon after osteoarthrosis of the wrist associated with nonrheumatoid, positive ulnar variance is uncommon. Eight c … tes surrounding it caused these tendons to rupture. It was impossible to use end-to-end sutures, so tendon transfers or tendon grafts were performed. The patients were evaluated one to four years after surgery. Three patients complained of some disability in their daily lives, notably, limited flexion of their fingers. It is important that during surgery the reco …

Excisional Debridement for Chronic Achilles Insertional Tendinosis: A Novel Technique and Case Review.

BACKGROUND: Operative treatment of chronic Achilles insertional tendinosis (AIT) involves tendon debridement, removal of the retrocalcaneal bursitis, and excision of the calcaneal exostosis, often followed by repair of the Achilles tendon and deep tendon transfer. The literature describes a variety of techniques without a single standard of care. METHODS: This is a retrospective review of 57 patients treated with an excisional debridement of the central portion of the Achilles tendon. The novelty of this technique is that instead of complete detachment of the tendon from its insertion, only t…

[Reinterventions after surgery of the extensor tendons].

Tenolysis, tendon grafts or tendon transfer are the main procedures to be considered for secondary repair in zone 6. Tenolysis is also indicated when adhesions prevent gliding in zone 5. Here interposition of the extensor retinaculum may provide protection against deep scar tissue. In zone 4, particularly in the presence of supernumerous tendons of abductor pollicis longus, mismatching of tendon ends may have occurred. In such cases the right connections have to be reestablished. Lesions of the hood which have not been recognized primarily will sometimes, when causing dislocation of the extens…

Minimally invasive reconstruction of a large chronic complete flexor hallucis longus tendon rupture: a case report and literature review.

BACKGROUND: Isolated rupture of the flexor hallucis longus (FHL) tendon is a scarce condition, with limited reported cases documented since 1980. Most reported cases have been managed surgically, using approaches such as direct repair, tendon transfers, or grafting, with variable outcomes. Meanwhile large chronic flexor hallucis longus (FHL) tendon defects exceeding 10 cm are rare and lack a standardized surgical treatment approach, particularly in physically active individuals. We present a distinctive case of a delayed, complete traumatic FHL tendon rupture in an active-demand individual, su…

Tendon injury and repair - A perspective on the basic mechanisms of tendon disease and future clinical therapy.

UNLABELLED: Tendon is an intricately organized connective tissue that efficiently transfers muscle force to the bony skeleton. Its structure, function, and physiology reflect the extreme, repetitive mechanical stresses that tendon tissues bear. These mechanical demands also lie beneath high clinical rates of tendon disorders, and present daunting challenges for clinical treatment of these ailments. This article aims to provide perspective on the most urgent frontiers of tendon research and therapeutic development. We start by broadly introducing essential elements of current understanding abou

Musculoskeletal diseases--tendon.

INTRODUCTION: Tendons establish specific connections between muscles and the skeleton by transferring contraction forces from skeletal muscle to bone thereby allowing body movement. Tendon physiology and pathology are heavily dependent on mechanical stimuli. Tendon injuries clinically represent a serious and still unresolved problem since damaged tendon tissues heal very slowly and no surgical treatment can restore a damaged tendon to its normal structural integrity and mechanical strength. Understanding how mechanical stimuli regulate tendon tissue homeostasis and regeneration will improve th

A Rare Presentation of Bifid Tibialis Anterior Tendon in a Case of Idiopathic Congenital Talipes Equinovarus.

INTRODUCTION: Idiopathic congenital talipes equinovarus (CTEV) is one of the most extensively researched topics for decades. It has been associated with various musculoskeletal anomalies which maybe bony, vascular or involving the ligaments and muscles which may have a direct or indirect impact on its pathoanatomy. This report describes an unusual presentation of a bifid tibialis anterior tendon (TAT) in a case of CTEV. This is the first report of this kind in the literature to the best of our knowledge. CASE REPORT: A 4-year-old female presented with bilateral relapsed CTEV with dynamic supi

Extensor tendon injuries and repairs in the hand.

Due to their superficial course, the extensor tendons are frequently lacerated over the dorsum of the hand and fingers. Excellent functional results are obtained in repairs of simple tendon lacerations. 'Open' mallet lacerations over the distal IP joint or involving the central extensor slip over the proximal IP joint require more precise suturing methods. More proximal extensor tendon divisions near the wrist involve dissection of the retracted finger extensors or long thumb extensor in the distal forearm and more formal tendon repairs, including a possible tendon transfer to the thumb. 'Clos…

[Secondary tendon reconstruction on the thumb].

Closed tendon ruptures of the thumb that require secondary reconstruction can affect the extensor pollicis longus (EPL), extensor pollicis brevis (EPB) and flexor pollicis longus (FPL) tendons. Treatment of rupture of the EPB tendon consists of refixation to the bone and temporary transfixation of the joint. In the case of preexisting or posttraumatic arthrosis, definitive arthrodesis of the thumb is the best procedure. Closed ruptures of the EPL and FPL tendons at the wrist joint cannot be treated by direct tendon suture. Rupture of the EPL tendon occurs after distal radius fractures either d
