Extensor tendon lacerations of the hand are commonly seen in the emergency department. These injuries can often be definitively managed by the emergency physician who has a working knowledge of the complex extensor mechanism anatomy plus basic surgical skills. A thorough initial assessment including a tourniquet examination for adequate exposure is the key to making the complete diagnosis. Surgical indications, materials, techniques, complications, and postoperative management involved in extensor tendon repair are reviewed. The emergency physician's decision to treat or refer these injuries w

Experience will show that proper management during the first 3 postoperative weeks will be the major determinant for the final result in extensor tendon injuries. In the simple injury, meticulous care in properly immobilizing each joint, combined with edema control, allows the patient to begin the mobilization phase with few problems, and a simple course of therapy can be anticipated. In the complex injury, early passive motion techniques will reduce the devastating complications of an increased fibroblastic response common to crushing injuries or combined lesions of the extensor tendons.

A case of trigger finger in a fourteen-year-old child, presenting as locking in extension is reported. At operation a tumour arising from the superficialis tendon was found and removed.

In both new and old tendon injuries, errors and hidden dangers exist that may lead to later complications. These arise at all four phases of primary repair or secondary reconstruction, namely preoperative assessment, exposure and evaluation, repair or reconstruction, and, finally, postoperative care. Often, the resulting complications are the same following tendon suture or tendon graft, the most common being adhesion with joint contracture. The list of complications also includes rupture, bowstringing, skin necrosis, infection, recurvatum, the lumbrical plus finger, and the quadriga syndrome.

Five cases of rupture of the quadriceps tendon and two cases of rupture of the patellar ligament form the authors' own patient material (1975-1984) are reviewed. It seems we must differentiate between rupture of the quadriceps tendon (namely, rupture in an elderly patient with degeneratively changed tendon in trivial traumas) on the one hand, and rupture of the patellar ligament (rupture in the young patient with considerable trauma, as in sports) and hence a more or less purely traumatically caused rupture, on the other. Except for one case of spontaneous rupture associated with cortison trea

The clinical characteristics of clubfoot suggest a relative hypoplasia of the posterior and medial foot and leg. To test this hypothesis we compared quantitative cell and organelle characteristics between anterior and posterior tibial tendon sheaths from four clubfeet and two normal feet. Clubfoot posterior tibial tendon sheath had significantly less cell and cytoplasm volume than did clubfoot anterior sheath. Normal posterior tibial tendon sheath had significantly greater cell and cytoplasm volume than did normal anterior sheath. A similar tendency was found for nuclear volume and cell number

The keys to the treatment of extensor tendon injuries are knowledge of the anatomic characteristics of the area, the correct position of immobilization of the wrist and fingers, and the timing for institution of active and resistive exercises.

Due to their superficial course, the extensor tendons are frequently lacerated over the dorsum of the hand and fingers. Excellent functional results are obtained in repairs of simple tendon lacerations. 'Open' mallet lacerations over the distal IP joint or involving the central extensor slip over the proximal IP joint require more precise suturing methods. More proximal extensor tendon divisions near the wrist involve dissection of the retracted finger extensors or long thumb extensor in the distal forearm and more formal tendon repairs, including a possible tendon transfer to the thumb. 'Clos…

The factors that normally determine the location and insertion of a muscle were explored in human experiments of nature with early problems … The dissection findings from all three types imply a general hierachy of muscle tendon attachments. Tendons appear to attach preferentially to bone. In the absence of the bone to which they would normally attach, they will attach to the next closest bone. If no such bone is available, tendons may attach to other tendons; and if no tendons is available, oc …

The calcareous peritendinitis of the long peroneal tendon is an occupational disease. It results from one sided overstraining with consecutive micro-or macroruptures in the tendon, lokalised degeneration, and deposition of calcium salts. The disease can be asymptomatic or produce only slight pain. In other cases with acute injury the calcified deposits cause acute discomfort. Three cases with varying course are reported and differential diagnosis and therapy are discussed.
