Evaluation and Management of Children with Thyrotoxicosis.

BACKGROUND: Uncontrolled thyrotoxicosis, especially in early infancy, may cause irreversible damage to the central nervous system as well as profound effects on the function of many organs. Thyrotoxicosis has multiple etiologies and treatment depends on the underlying etiology. An accurate diagnosis is essential so that appropriate treatment can be initiated without undue delay. OBJECTIVE: To review in depth the evaluation, diagnosis, and treatment of children with thyrotoxicosis. METHODS: A PubMed search was completed in Clinical Queries using the key terms "thyrotoxicosis" and "hyperthyroi

A novel bioreporter assay for thyrotropin receptor antibodies using a chimeric thyrotropin receptor (mc4) is more useful in differentiation of Graves' disease from painless thyroiditis than conventional thyrotropin-stimulating antibody assay using porcine thyroid cells.

BACKGROUND: Graves' disease (GD) is caused by thyrotropin (TSH) receptor antibodies (TSHRAbs) that bind to TSHR and activate thyrocytes. The measurement of TSHRAbs therefore has been used to assist in the diagnosis and management of GD. METHODS: In this study, we evaluated the clinical significance of a newly developed bioreporter assay for the detection of TSHRAbs (Thyretain). The Thyretain bioreporter assay utilizes a chimeric receptor (Mc4), in which residues 262-335 of TSHR are replaced with a rat lutropin-choriogonadtropin receptor segment. This bioreporter is designed to specifically de

Sorely Mistaken-Soft Palatal Myxedema in Decompensated Hypothyroidism Presenting as a Sore Throat: Case Report.

INTRODUCTION: Oropharyngeal myxedema is a rare presenting symptom of decompensated hypothyroidism that can mimic more common causes of sore throat. CASE REPORT: We describe a case of an older woman who presented with throat pain and dysphagia, found to have soft palate edema on exam and imaging. Laboratory testing confirmed severe hypothyroidism, and her symptoms eventually resolved with thyroid hormone replacement therapy. CONCLUSION: This case highlights a rare and under-recognized presentation of a common endocrine disorder. Consider myxedema from severe hypothyroidism in patients with su

Effectiveness of Different Treatment Modalities in Initial and Chronic Phases of Thyroid Eye Disease: A Systematic Review With Meta-analysis.

BACKGROUND: Thyroid eye disease (TED), a common extrathyroidal manifestation of Graves disease, poses significant management challenges due to potential disfigurement, visual impairment, and decreased quality of life. Uncertainties remain about the optimal treatment approach, especially regarding TED duration and its impact on outcomes. OBJECTIVE: This meta-analysis evaluates the effects of various treatments on inflammatory markers and severity endpoints in TED, stratified by disease duration, distinguishing between treatments initiated within the first 6 months (initial phase) and those ini

Pharmacological and toxicological studies on domiodol, a new mucolytic agent and expectorant.

The efficacy and toxicity of a compound containing organically bound iodine, 4-hydroxymethyl-2-indomethyl-2,3-dioxolane (domiodol), were evaluated. After oral and intravenous administration domiodol increased respiratory tract excretion indirectly measured on the basis of bronchial secretion in rats. Based on Kasé's method for quantitatively collecting respiratory tract fluid from rabbits, oral domiodol at the dose of 50-200 mg/kg induced a dose-related increase in respiratory tract fluid. In addition, oral domiodol (50 and 100 mg/kg) significantly reduced the viscosity of sputum collected qua

SARS-CoV-2 Vaccine-induced Thyroiditis: Safety of Revaccinations and Clinical Follow-up.

CONTEXT: The number of reported cases with severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) vaccine-induced subacute thyroiditis (SAT) and Graves' disease (GD) is growing. However, active debate continues about managing such side effects and the safety of repeat or booster doses of the vaccines in such cases. OBJECTIVES: This study aims to present long-term clinical follow-up of SARS-CoV-2 vaccine-induced SAT or GD cases and provide data regarding the safety of revaccinations. METHODS: Patients diagnosed with SARS-CoV-2 vaccine-induced SAT or GD were included. Data regarding the

Interleukin-6: a marker of thyroid-destructive processes?

Increased serum interleukin-6 (IL-6) concentrations have recently been reported in patients with subacute thyroiditis and in some patients with amiodarone-induced thyrotoxicosis, possibly because of cytokine release from damaged thyroid cells. In this study, serum IL-6 levels were determined by an enzyme-linked immunosorbent assay method in 18 patients given percutaneous intranodular ethanol injection (PIEI) for autonomously functioning thyroid nodule, 12 patients treated with radioactive iodine (RAI) for Graves' disease or toxic adenoma, and 23 patients submitted to fine needle aspiration (FN

[Anti-NAE autoantibodies and clinical spectrum in Hashimoto's encephalopathy].

Hashimoto's thyroiditis (HT) is the most common disorder affecting the thyroid gland. Encephalopathy associated with abnormal thyroid functions, such as myxedema encephalopathy, is treatable. Hashimoto's encephalopathy (HE) was recognized as a new clinical disease based on an autoimmune mechanism associated with HT, and steroid treatment has been successfully administrated. Recently, we discovered serum autoantibodies against the NH2-terminal of a-enolase (NAE) as a specific diagnostic marker for HE. We analyzed these serum anti-NAE autoantibodies and the clinical features in 84 cases of HE. T

Non-paraneoplastic limbic encephalitis associated with anti-glutamic acid decarboxylase antibodies.

Limbic encephalitis (LE) is a neurological syndrome that may present in association with cancer, infection, or as an isolate clinical condition often accompanying autoimmune disorders. Here we have characterized the clinical and laboratory features of two patients presenting with subacute onset, and chronic evolution, of anterograde amnesia and drug-resistant epilepsy associated with thyroid autoimmunity and in absence of tumoral pathology despite long follow-up. Antibodies against onconeural antigens, voltage gated potassium channel and glutamate receptors, which may accompany paraneoplastic

Physiologic and pathologic calcifications and ossifications in the face and neck.

The aim was to give a systematic presentation of physiologic and pathologic calcifications and ossifications in the face and neck with a special emphasis on clinical relevance. In a sometimes subacute setting one should recognize specific calcifications which often lead to important diagnoses such as fungal sinusitis or sclerosing labyrinthitis. In a more chronic situation intraocular calcifications in small children are pathognomonic for retinoblastoma. Juxtatumoral sclerosis of the laryngeal cartilage in laryngopharyngeal carcinoma is usually caused by tumor infiltration of the cartilage res

Identification of thyroid hormone residues on serum thyroglobulin: a clue to the source of circulating thyroglobulin in thyroid diseases.

Thyroglobulin (Tg) present in the serum of normal individuals and patients with thyroid disorders could be partly newly synthesized non-iodinated Tg and partly Tg containing iodine and hormone residues originating from the lumen of thyroid follicles. With the aim of examining the contribution of the latter source of Tg to the elevation of serum Tg concentration in thyroid pathophysiological situations, we devised a procedure to identify thyroxine (T4) and tri-iodothyronine (T3) residues on Tg from unfractionated serum. A two-step method, basedon (i)adsorption of Tg on an immobilized anti-human

Lyme Disease as a Potential Precursor to Anti-N-Methyl-D-Aspartate Receptor Encephalitis: A Case Report.

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a complex neuroinflammatory condition often triggered by underlying factors such as viral infections or malignancies, although idiopathic cases are not uncommon. Here, we present a unique case of a 27-year-old female patient diagnosed with anti-NMDAR encephalitis, where Lyme disease emerges as a plausible yet underrecognized trigger. The patient initially presented to the emergency department (ED) with acute psychosis and was later found to have positive Borrelia burgdorferi immunoglobulin G (IgG) titers, antibodies against the NR

Endocrine Involvement in COVID-19: Mechanisms, Clinical Features, and Implications for Care.

Coronavirus 2019 (COVID -19) has rapidly emerged as a global pandemic with multi-system involvement. Involvement of the endocrine system is expected in COVID-19 as the interplay between severe acute respiratory syndrome corona virus-2 (SARS CoV-2) and the endocrine system occurs at multiple levels. The widespread presence of ACE-2 receptors on various tissues suggests scope for direct viral infection. The interactions via the activation of inflammatory mediators and indirect immune-mediated damage are also postulated. Evidence so far suggests that COVID-19 can cause functional hypopituitarism

[The development of a sensitive radioimmunoassay for H-TSH and its clinical application (author's transl)].

Using a commercially available h-TSH RIA kit (Daiichi), a sensitive RIA method was developed. When pooled serum from patients with active Graves' disease was used as a carrier, combined applications of a delayed addition of 125-I TSH and utilization of X2 diluted anti-TSH antibody resulted in the consistent detection of 0.156 microunits/ml level of serum TSH. Further, 0.078 microunits/ml level could also be detected when 200 microliter of serum samples were applied. However, usage of 200 microliter sample volume should be determined after further studies concerning possible non specific effect

The utility of liver transplantation to treat acute liver failure caused by adult-onset Still's disease: case reports.

PURPOSE: Adult-onset Still's disease (AOSD) is an inflammatory condition commonly complicated by mild liver dysfunction. However, severe liver failure is rarely reported. We report three cases of severe acute hepatic failure (ALF) associated with AOSD. We encountered three cases of acute liver failure (ALF) with encephalopathy. RESULTS: Case 1 was a 75-year-old female, who was started on a steroid (prednisolone, PSL) to treat AOSD; this was gradually tapered. Two months later, severe ALF developed. She died despite an increase in the PSL dose and artificial liver support. Case 2 was a 26-year

Patients with a relapsing course of steroid-responsive encephalopathy associated with autoimmune thyroiditis exhibit persistent intrathecal CD4+ T-cell activation.

BACKGROUND AND PURPOSE: Steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT) is a rare condition defined by encephalopathy with acute or subacute onset, the presence of serum anti-thyroid antibodies, and reasonable exclusion of alternative causes. Despite having strong response towards corticosteroid treatment, some patients exhibit a chronic-relapsing course and require long-term immunosuppression. Markers for early identification of those patients are still absent. Thus, we aimed to characterise clinical as well as laboratory parameters of our local SREAT cohort.

Diagnosing Mitochondrial Disorder without Sophisticated Means.

Mitochondrial disorders (MIDs) require biochemical or genetic investigations for being diagnosed. In some cases, however, the diagnosis can be suspected upon the syndromic phenotype or upon clinical presentation and family history, as in the following case. The patient was a 74-year-old male admitted for worsening of pre-existing left-sided ptosis and ophthalmoparesis after a birthday party. The history was positive for arterial hypertension, hypertrophic cardiomyopathy with systolic dysfunction, diabetes-type 2, mild renal insufficiency, thyroiditis, and polyneuropathy. Hyperthyroidism-Induced Dural Venous Sinus Thrombosis: A Case Report.

Cerebral dural venous sinus thrombosis is an uncommon and potentially life-threatening cerebrovascular disorder with a broad and often nonspecific clinical spectrum, frequently leading to diagnostic delay. We describe a young woman with no prior comorbidities who presented with a subacute history of progressively worsening headache associated with visual disturbances and pulsatile tinnitus. Neuroimaging with computed tomography and magnetic resonance venography demonstrated acute thrombosis of the right transverse dural venous sinus without parenchymal infarction or hemorrhage.
