Cellular and molecular mechanisms of tooth root development.

The tooth root is an integral, functionally important part of our dentition. The formation of a functional root depends on epithelial-mesenchymal interactions and integration of the root with the jaw bone, blood supply and nerve innervations. The root development process therefore offers an attractive model for investigating organogenesis. Understanding how roots develop and how they can be bioengineered is also of great interest in the field of regenerative medicine. Here, we discuss recent advances in understanding the cellular and molecular mechanisms underlying tooth root formation. We rev

Rehabilitation to crown-root fracture by fragment reattachment with resin-modified glass ionomer cement and composite resin restoration.

Tooth fragment reattachment in cases of enamel/dentin fracture is based on adhesive protocols and is indicated because of its technique simplicity. Complex cases of root fracture or extensive coronal damage demand the integration of biological properties with the mechanical characteristics of the restorative materials. This paper is a report from one patient with a favourable result that addresses the clinical procedures involved in the association of resin-modified glass ionomer cement and composite resin for cases of coronal fracture of anterior teeth with invasion of the periodontal biologi

Root reshaping: an integral component of periodontal surgery.

It is the aim of this article to present a surgical option to the traditional method of returning lost biologic width where invasion of the junctional and/or connective tissue attachment associated with a tooth has occurred. The alternative to conventional osseous resection involves reshaping the existing tooth surface in combination with conservative removal of the supporting alveolar bone to create the width needed for the restoration to be biologically acceptable. This procedure accomplishes several goals: (1) minimum supporting bone is removed; (2) deleterious root surface anatomy, such as

[Pulpectomy. Why? When?].

There are many circumstances under which the condition of the pulp calls for root canal work, due to the real or potential risk of pain, infection, inflammation or functional difficulties (acute, irreversible pulpitis, chronic pulpitic conditions, necrosis and its consequences, etc.). Other circumstances exist in which injury to the pulp is not necessarily irreversible: initial pulpitis, iatrogenic fracture of the pulp, traumatisms, dystrophic deteriorations. The high success rate for endodontic therapy could argue for elimination of impaired or necrose-prone pulp. In fact, however, unsuccessf

Mandibular Buccal Bifurcation Cyst (MBBC): A New Microsurgical and Guided Tissue Regeneration Approach-A Case Report.

BACKGROUND: Mandibular buccal bifurcation cyst (MBBC) is an inflammatory cyst that typically manifests during early childhood and is positioned buccal to the furcation area of the mandibular first molars. Clinically, it manifests as a buccal inclination of the crown without associated symptoms or dental vitality loss. The diagnosis is confirmed through a combination of clinical, radiological, surgical, and histological findings. CASE REPORT: A case of a 9-year-old boy with MBBC is presented. Molar 4.6 exhibits a buccal inclination of the crown and a palpable, nonpainful hard swelling on the v

Three-dimensional partitioning and quantification of orthodontic root resorption via automatic root extraction from cone-beam computed tomography.

BACKGROUND: External root resorption (ERR) during orthodontic treatment is a common concern, and accurate quantification is crucial for assessing outcomes and minimizing long-term complications. This study aims to quantify ERR using automated root extraction from cone beam computed tomography (CBCT), combined with a novel root partitioning method and enhanced through the integration of intraoral scans for improved accuracy. METHODS: Thirty-six patients with malocclusion were included and divided into two groups. Root extraction was performed on CBCT images using artificial intelligence (AI),

Influence of restorative materials on the mechanical properties of maxillary first molars with different degrees of cryptic fractures and defects: A finite element analysis.

This study aimed to apply finite element analysis to evaluate the effects of pile materials with different elastic moduli and cement materials on the stress distribution between the remaining tooth tissue and cryptic fracture defects. A three-dimensional finite element model was established for 20 maxillary first molars with hidden fissures and mesial tongue-tip defects. Two levels of hidden cracks and three types of pile and adhesive materials were used in the design. The stress distribution and maximum stress peak in the remaining tooth tissue and crack defects were determined by simulating

Permian hypercarnivore suggests dental complexity among early amniotes.

The oldest known complex terrestrial vertebrate community included hypercarnivorous varanopids, a successful clade of amniotes with wide geographic and temporal distributions. Little is known about their dentition and feeding behaviour, but with the unprecedented number of specimens of the varanopid Mesenosaurus from cave deposits in Oklahoma, we show that it exhibited serrations on the tooth crowns, and exceptionally rapid rates of development and reduced longevity relative to other terrestrial amniotes. In contrast, the coeval large apex predator Dimetrodon greatly increased dental longevity…

Is it a cystic or a cyst-like condition? Discussing the etiology of an unusual large mandibular lesion in a Roman skeleton from Quinta da Torrinha/Quinta de Santo António (Almada, Portugal).

Cystic and cyst-like lesions of the jaw are a recurrent finding in routine dental radiography but not in paleopathology. This paper describes a large oval osteolytic cavity (23 × 14 mm) observed in the mandible of a middle-aged female unearthed from the Roman necropolis of Quinta da Torrinha/Quinta de Santo António (Almada, Portugal, 3rd-5th centuries AD). The lesion was located in the body of the mandible, inferior to the alveolus of the first left molar. The associated tooth exhibited a carious lesion that destroyed the tooth crown and exposed the pulp cavity. The osteolytic lesion presented…

Restoration of Long Standing Traumatized Teeth: A Case Report.

Children are uniquely susceptible to craniofacial trauma. Injuries to the teeth occur often as a result of falls and sport activities. The pulp often gets infected after dental trauma resulting in to numerous complications. The authors present a case report of successful restoration of traumatized teeth with open apex which were weakened due to long standing infection and internal resorption. Initially antibiotic combination of 3- mix was used to disinfect the root canals. One tooth is treated with conventional endodontic treatment and the other tooth with open apex and perforation is managed

[Only a denture sore? Causes of a possible denture sore from a maxillary complete denture].

This case presentation documents the treatment sequence of a 74 years old patient who complained about a sore spot of the palatal mucosa underneath the complete denture. The intraoral examination revealed a dark spot, redness and swelling of the mucosa around this spot and halitosis. The mucosa exhibited a perforation of 3 x 10 mm in diameter. A radiographic 3-D picture showed an impacted canine tooth, which was partly covered by the palatal bone. Firstly the denture base was relieved and the swelling gradually disappeared. Then a biopsy was taken for histological analysis to exclude any malig

Calcific metamorphosis: a challenge in endodontic diagnosis and treatment.

Calcific metamorphosis (CM) is seen commonly in the dental pulp after traumatic tooth injuries and is recognized clinically as early as 3 months after injury. Calcific metamorphosis is characterized by deposition of hard tissue within the root canal space and yellow discoloration of the clinical crown. Opinion differs among practitioners as to whether to treat these cases upon early detection of CM or to observe them until symptoms or radiographic signs of pulpal necrosis are detected. In this review, the clinical, radiographic, and histopathologic appearance of CM is described; a review of th

Patterns of permanent incisor, canine and molar development in modern humans, great apes and early fossil hominins.

OBJECTIVE: The objectives of this study were to quantify the variation in coincident stages of incisor, canine and molar eruption and tooth formation in modern humans and great apes and then to ask if any early fossil hominins showed a dental development pattern beyond the human range and/or clearly typical of great apes. DESIGN: Four stages of eruption and 18 stages of tooth development were defined and then scored for each developing tooth on radiographs of 159 once-free-living subadult great apes and on orthopantomographs of 4091 dental patients aged 1-23 years. From original observations,

Occurrence of invasive cervical resorption after the completion of orthodontic treatment.

The invasive cervical resorption (ICR) is an irreversible and progressive loss of tooth hard tissues involving coronal and root dentine that usually affects single permanent teeth. The aim of this study was to present the pattern of ICR lesions detected in three patients with no contributory medical and dental anamnesis, at different time periods after orthodontic tooth movements. Conventional radiographs and cone-beam computed tomography (CBCT) images were obtained by X-Mind™ (Satelec), respective 3D Accuitomo (Morita). The present clinical study confirmed that orthodontic forces might be a k

Validation of the Basic Erosive Wear Examination.

The Basic Erosive Wear Examination (BEWE) is a practical index for screening tooth wear, using a 4-point ordinal scale (0-3). The highest score is recorded in each sextant and a total score (or BEWE sextant cumulative) is calculated per subject. This study aims to investigate if the BEWE sextant cumulative score compares to one comprising a percentage score from all tooth surfaces and as a highest BEWE per subject. The aim is to assess the validity of this score. A total of 350 subjects were recruited from hospital and general practice in south-east England. Buccal, occlusal and lingual/palata

Prevalence of pre-eruptive intracoronal dentin defects from panoramic radiographs.

PURPOSE: This investigation stemmed from preliminary clinical observations from a school dental clinic, which suggested that a proportion of clinically undetected, radiolucent lesions on radiographs may originate as pre-eruptive intracoronal dentin defects. This study investigated the prevalence of such defects in orthopantomograms from a group of children and young adults. METHODS: A total of 1281 orthopantomograms with 11,767 unerupted permanent teeth were examined. RESULTS: The prevalence of intracoronal dentin defects was 3% by subjects, and 0.5% by teeth; the highest prevalence being no

Developmental malformation of primary and permanent dentition: a rare sequel of trauma.

Dentofacial injuries that occur prior to the eruption of primary teeth can result in developmental disturbances not only in the primary but also in permanent dentition. Here we report a rare case of long term sequelae of trauma in a female child of 4 to 5 months of age which resulted in dilaceration and impaction of maxillary primary central incisors and subsequent enamel hypoplasia of the permanent maxillary central incisors.
