Open Airway Surgery in a Paraplegic: The Importance of an Adequate Cough.

OBJECTIVES: To describe a case of open airway surgery with postoperative respiratory complications in a paraplegic woman and to review the unique respiratory physiology seen in patients with a history of cervical or thoracic spinal cord injury (SCI). METHODS: Case report and literature review. RESULTS: We describe the case of a 25-year-old paraplegic who developed tracheal stenosis after tracheotomy, eventually requiring tracheal resection and re-anastomosis. Her postoperative course was complicated by mucus plugging and severe atelectasis, necessitating reintubation. After extubation, the p…

Relapsing polychondritis with initial presentations of recurrent negative-pressure pulmonary edema and acute respiratory failure.

Relapsing polychondritis is a rare autoimmune disease causing inflammation in cartilaginous structures and other tissues throughout the body. Negative-pressure pulmonary edema (NPPE) due to laryngeal swelling from relapsing polychondritis is rare and has not been reported. Here, we report a case of relapsing polychondritis in an 18-y-old female who presented with recurrent NPPE and acute respiratory failure, which was diagnosed initially as ARDS during the influenza season. She underwent an emergent tracheotomy to relieve the upper airway obstruction resulting from severe laryngeal edema. A ch…

Management of congenital subglottic hemangioma: trends and success over the past 17 years.

OBJECTIVE: To evaluate the success and complications of various treatment options of congenital subglottic hemangioma. STUDY DESIGN AND SETTINGS: Reported cases were grouped by treatment modalities and corresponding outcome evaluated. RESULTS: From 1986 through 2002, 372 patients were reported in 28 series. Carbon dioxide laser had 88.9% success rate yet 5.5 % significant subglottic stenosis. It shortened the tracheotomy duration by 13.7 months. Corticosteroids were not that beneficial (useful in only 24.5%) with 12.9% side effects. Intralesional corticosteroids were successful in 86.4% with…

A comparative analysis of open surgery vs endoscopic balloon dilation for pediatric subglottic stenosis.

IMPORTANCE: Minimally invasive endoscopic techniques are an appealing alternative to open surgical management of pediatric subglottic stenos … spital from 2006 through 2012. MAIN OUTCOMES AND MEASURES: Success was defined as decannulation or tracheotomy avoidance. Additional outcomes were total number of procedures and number of unplanned procedures. Univariate χ2 analyses and multivariate regression analyses were performed to identify patient and disease factors statistically associated with succe …

Sometimes tracheotomy may be necessary. Tracheotomy is also highly complicated in MPS patients. [Anesthesia and Surgery of the Trachea].

This article aims to review the current anaesthetic management of tracheal resections.Apart from the "traditional" approach of induction of general anaesthesia with conventional tracheal intubation and cross-field intubation or jet ventilation during the resection phase, there has lately been a trend towards less invasive techniques.Regional anaesthesia, laryngeal mask airways and preservation of spontaneous ventilation are among the new anaesthetic approaches. Current data suggest potential advantages compared with conventional tracheal intubation.Extracorporeal membrane oxygenation may provi

Balloon dilatation of pediatric subglottic laryngeal stenosis during the artificial apneic pause: experience in 5 children.

INTRODUCTION: Balloon dilatation is a method of choice for treatment of laryngeal stenosis in children. The aim of procedure in apneic pause is to avoid new insertion of tracheostomy cannula. PATIENTS AND METHODS: The authors performed balloon dilatation of subglottic laryngeal strictures (SGS) in 5 children (3 girls and 2 boys) without tracheotomy. Two of them with traumatic and inflammatory SGS had a tracheal cannula removed in the past. The other 3 children with postintubation SGS had never had a tracheostomy before. The need for tracheostomy due to worsening stridor was imminent for all o…

[Cancer or syphilis laryngis, politics or diagnostic failure--problems of Emperor Frederick's III disease].

The case history of the German emperor Frederick III, who died of laryngeal cancer in 1888, is briefly reported. In spite of fact, that the German laryngologists the patient's pathologic changes diagnosed as laryngeal cancer in early stage, the ultimate diagnosis was made too late for surgery to have any effect. This delay was due to the differences in opinions between the attending laryngologists; particularly between that of the English prominent laryngologist Morell Mackenzie and the Germans, prof. Gerhardt and prof. Bergmann. The roles of the British Queen Victoria as well as her daughter,

Effect of expiratory glottic constriction on lung volume and pattern of breathing in adult dogs.

We examined the effect of laryngeal constriction on the pattern of breathing in 4 anaesthetized adult mongrel dogs. By means of a T-shaped tracheostomy tube the larynx could be repeatedly excluded or included in the breathing circuit. Marked expiratory activity of the thyroarytenoid muscle (TA), the main glottic constrictor, was induced by injection of 100-400 ml of air into the pleural space or by inhalation of histamine aerosol (2 dogs) which resulted in rapid shallow breathing. In 21 pairs of runs in the 4 dogs switching from tracheostomy breathing to oral breathing decreased mid-expiratory

[Bilateral choanal atresia in the newborn infant. Role of the anesthesiologist].

Congenital bilateral atresia of the choana is a rare anomaly. It is a closure of the posterior nares by a membranous and/or osseous partition. It causes early respiratory distress in the neonate. The history and presenting symptoms seldom cannot simulate those of severe respiratory distress in the newborn. The anaesthesist can easily make the diagnosis when he observes a "cyclic pattern of respiratory distress": chest retraction and cyanosis which disappear with crying. The purpose of this present case is to emphasize both anaesthetic and surgical approaches to the management of this type of l

[The critical point in laryngeal and tracheal obstruction (author's transl)].

Laryngeal and tracheal obstruction can be more fully assessed using plethysmography. Experiences with 46 patients show that airway resistances exceeding 150 mm H2O/l/s always indicate a critical situation and these patients require urgent tracheostomy or intubation. - Three patients with glottic tumours and one patient with glottic oedema were tracheotomized because of the danger of asphyxiation. In these, resistances of between 107.4 and 150 mm H2O/l/s were found. Airway resistance exceeding 100 mm H2O/l/s are therefore critical indicators in patients with glottic oedema and laryngeal tumours
