Syphilis is a sexually transmitted disease caused by Treponema pallidum. The signs and symptoms of syphilis vary depending on which of the four stages it presents. The primary stage of syphilis classically presents with a painless ulcer (chancre). We report a case of the extragenital chancre on the nipple which is examined from skin biopsy and immunohistochemistry. This case showed that it is important to identify the special site's pruritus erythema by pathology and serological examination.

We present a case of a penile lesion with a clinical appearance similar to Mondor penile disease (thrombosis of the dorsal vein of the penis) or penile sclerosing lymphangitis. Laboratory evaluation, however, showed a solid lesion, with no vascular component to Doppler ultrasonography and no treponema to immunohistochemistry. Histological and serological tests were compatible with secondary syphilis. The authors reinforce the need for the inclusion of syphilis in the differential diagnosis of penile cord injuries.

Syphilis is a well-known cause of foetal death. We describe a Danish case of foetal death in week 31 + 0 in an Iraqi-born woman. The foetus had ascites. Blood samples were positive for IgG and IgM, for syphilis, and immunohistochemical staining of the placenta showed Treponema pallidum in relation to the vessels of the umbilical cord. Congenital syphilis should be kept in mind even though it is rare in Denmark.

Congenital syphilis has varied manifestations in first two years of life. A case of 4-month-old child who was presented with painless swelling of finger and toes is reported. Family history was suggestive of syphilis in parents and one sibling. VDRL test in serum was positive in 1:128 dilution. Treponema pallidum haemagglutination test was positive. The child was treated with crystalline penicillin and responded favourably.

We analyzed prevalence rates of syphilis (positive Treponema pallidum hemagglutinin antigen/T. pallidum particle antigen and venereal disease research laboratory test) among police officers in Guinea-Bissau from 1990 to 2010 and found a significant decline from 4.5% to 0.4% (P = 0.0065). Our results are in line with other recent reports from West Africa. More research is needed to identify the reasons for this decline.

Secondary syphilis, the hematogenous spread of Treponema pallidum, usually occurs 4-10 weeks after initial exposure. The skin is involved in 70% of cases, with maculopapular, vesiculobullous, and ulcerative morphologies possible at presentation, making the diagnosis of secondary syphilis challenging given its ability to mimic many other conditions. We present an atypical case of secondary syphilis that closely resembled erythema multiforme (EM) clinically and histologically.

Patients undergoing cardiac transplantation are routinely screened for prior exposure to Treponema pallidum, the bacterium implicated in syphilis. Although acute syphilis typically presents with a painless genital chancre, chronic syphilis can manifest in protean forms and linger undiagnosed. Left untreated, syphilis can cause multi-system disease, particularly irreversible neurologic damage. We describe a case of cutaneous secondary syphilis, neurosyphilis and luetic hepatitis in a cardiac transplant patient.

Lues maligna is a rare presentation of an infection with Treponema pallidum. Here we report three lues maligna infections with severe dermatological manifestations in a single HIV-1 infected individual. Despite the start of highly active antiretroviral therapy and a substantial increase in CD4 cell count after the first episode, he developed consecutive episodes. We assume a specific immunological predisposition to react to T. pallidum in this patient.

Studies were carried out on the comparative value of passive haemaglutination, cardiolipin and treponema tests in a total number of 4327 sera syphilitic patients in various stages of the disease and apparently normal subjects or free of syphilis. The results demonstrated the specificity and sensitivity of passive haemagglutination in all the stages of syphilis. In view of the qualities and simplicity of the test, the author recommends it as a screening test combined with a micromethod using the cardiolipin antigen (flocculation test).
