Human African trypanosomiasis: How do the parasites enter and cause dysfunctions of the nervous system in murine models?

In this review we describe how Trypanosoma brucei brucei, a rodent pathogenic strain of African trypanosomes, can invade the nervous system, first by localization to the choroid plexus, the circumventricular organs (CVOs) and peripheral ganglia, which have fenestrated vessels, followed by crossing of the blood-brain barrier (BBB) into the white matter, hypothalamus, thalamus and basal ganglia. White blood cells (WBCs) pave the way for the trypanosome neuroinvasion. Experiments with immune deficient mice show that the invasion of WBCs is initiated by the toll-like receptor 9, followed by an aug

African animal trypanosomiasis as a constraint to livestock health and production in Karamoja region: a detailed qualitative and quantitative assessment.

BACKGROUND: Nagana (African Animal Trypanosomiasis-AAT) and tick-borne diseases (TBDs) constrain livestock production in most parts of sub-Saharan Africa. To this realisation, Uganda government set up an African trypanosomiasis (AT) control unit, which among other activities generates national tsetse control priority maps using apparent tsetse density data. Such maps underestimate mechanically transmitted AAT and thus ought to be refined using actual AT prevalence data. We therefore set out to generate up-to-date cattle and donkey trypanosomiasis prevalence data as well as find out the constra…

Overview of the Diagnostic Methods Used in the Field for Human African Trypanosomiasis: What Could Change in the Next Years?

Sleeping sickness is a parasitic infection caused by two species of trypanosomes (Trypanosoma brucei gambiense and rhodesiense), transmitted by the tsetse fly. The disease eventually affects the central nervous system, resulting in severe neurological symptoms. Without treatment, death is inevitable. During the first stage of the disease, infected patients are mildly symptomatic and early detection of infection allows safer treatment (administered on an outpatient basis) which can avoid death; routine screening of the exposed population is necessary, especially in areas of high endemicity. The

Proteomic selection of immunodiagnostic antigens for human African trypanosomiasis and generation of a prototype lateral flow immunodiagnostic device.

BACKGROUND: The diagnosis of Human African Trypanosomiasis relies mainly on the Card Agglutination Test for Trypanosomiasis (CATT). While this test is successful, it is acknowledged that there may be room for improvement. Our aim was to develop a prototype lateral flow test based on the detection of antibodies to trypanosome antigens. METHODOLOGY/PRINCIPAL FINDINGS: We took a non-biased approach to identify potential immunodiagnostic parasite protein antigens. The IgG fractions from the sera from Trypanosoma brucei gambiense infected and control patients were isolated using protein-G affinity…

Identification of a κ-opioid agonist as a potent and selective lead for drug development against human African trypanosomiasis.

A resazurin-based cell viability assay was developed for phenotypic screening of the LOPAC 1280 'library of pharmacologically active compounds' against bloodstream forms of Trypanosoma brucei in vitro identifying 33 compounds with EC(50) values <1 μM. Counter-screening vs. normal diploid human fibroblasts (MRC5 cells) was used to rank these hits for selectivity, with the most potent (<70 nM) and selective (>700-fold) compounds being suramin and pentamidine. These are well-known antitrypanosomal drugs which demonstrate the robustness of the resazurin cell viability assay. The most selective nov

Endemic type of animal trypanosomiasis is not associated with lower genotype variability of Trypanosoma congolense isolates circulating in livestock.

In order to verify whether the low impact on livestock production in endemic areas is related to a low number of trypanosome strains circulating in livestock, 37 Trypanosoma congolense isolates collected from cattle in 11 sites in an endemic trypanosomiasis area in Eastern Zambia were characterised for genotype variability using a modified amplified fragment length polymorphism technique (AFLP). Isolates were further cloned to evaluate the occurrence of mixed infections in individuals. The results obtained revealed a high genotype diversity (94.6%) among these isolates. Apart from one site, al…

Evaluation of cryptolepine and huperzine derivatives as lead compounds towards new agents for the treatment of human African trypanosomiasis.

The alkaloid cryptolepine (1) and eight synthetic analogues (2-8) were assessed for in vitro activities against Trypanosoma brucei. Four of the analogues were found to be highly potent with IC50 values of less than 3 nM and three of these were assessed against T. brucei brucei infection in rats. The most effective compound was 2, 7-dibromocryptolepine (7); a single oral dose of 20 mg/kg suppressed parasitaemia and increased the mean survival time to 13.6 days compared with 8.4 days for untreated controls. In addition, four huperzine derivatives (9-12) were shown to have in vitro antitrypanosom

A new format of the CATT test for the detection of human African Trypanosomiasis, designed for use in peripheral health facilities.

OBJECTIVES: To test the reproducibility and thermostability of a new format of the Card-Agglutination Test for Trypanosomiasis (CATT) test for Human African Trypanosomiasis (HAT), designed for use at primary health care facility level in endemic countries. METHODS: A population of 4217 from highly endemic villages was screened using the existing format of the CATT test (CATT-R250) on whole blood. All those testing positive (220) and a random sample of negatives (555) were retested in the field with the new format (CATT-D10). Inter-format reproducibility was assessed by calculating kappa. [Epidemiological, clinical and sociodemographic characteristics of human African trypanosomiasis (HAT) in and around Kinshasa, Democratic Republic of Congo].

BACKGROUND: Despite efforts to control human African trypanosomiasis (HAT) in the field, this infection remains prevalent in endemic or epidemic form in most of its traditional habitats. In the Democratic Republic of Congo (DRC), HAT has extended beyond rural areas to reach large cities such as Kinshasa. The objective of this study was to analyse the characteristics of trypanosomiasis patients (cases) in Kinshasa and to compare them to those of healthy controls. METHODS AND POPULATION OF STUDY: This case-control study allowed us to compare case patients and controls for some epidemiologic, cli…

Combination chemotherapy with a substance P receptor antagonist (aprepitant) and melarsoprol in a mouse model of human African trypanosomiasis.

Drug therapy for late-stage (encephalitic) human African trypanosomiasis (HAT) is currently very unsatisfactory with the most commonly used drug, melarsoprol, having a 5% overall mortality. There is evidence in a mouse model of HAT that Substance P (SP) receptor antagonism reduces the neuroinflammatory reaction to CNS trypanosome infection. In this study we investigated the effects of combination chemotherapy with melarsoprol and a humanised SP receptor antagonist aprepitant (EMEND) in this mouse model. The melarsoprol/aprepitant drug combination did not produce any clinical signs of illness i…

[Human African trypanosomiasis in the mangrove forest in Guinea: epidemiological and clinical features in two adjacent outbreak areas].

The purpose of this study carried out in two adjacent areas of the coastal mangrove forest of Guinea (Dubreka and Boffa) was to screen the population for disease, provide information on human African trypanosomiasis (HAT, a.k.a. sleeping sickness) and compare the epidemiologic and clinical features with those of outbreak areas in the Ivory Coast where more data is currently available. Cases of HAT were confirmed by parasitological testing after active medical work-up (91 of 9637 patients examined). Five cases were confirmed in patients in treatment centers. Of the first 57 cases admitted for t…

[Cost of a national program to control human African trypanosomiasis in the high Mbomou region, Central African Republic].

An outbreak of human African trypanosiaisis is ongoing in the High Mbomou area of the Central African Republic. This area is located on the Sudanese border approximately 1,100 kilometers from the capital city of Bangui. According to current estimates, the cost of implementing the National Human African Trypanosomiasis Program is 754,000 United States Dollars, i.e., 4.1 dollars per protected inhabitant. However actual conditions in the field suggest that this estimate should be revised. Special field conditions include constant refugee movement across the border, lack of accurate epidemiologica…

Effects of urbanization on transmission of human African trypanosomiasis in a suburban relict forest area of Daloa, Côte d'Ivoire.

The epidemiological risk of human African trypanosomiasis transmission was evaluated from entomological parameters (apparent trap density, female teneral rates, daily survival rates, proportion of human feeds) of tsetse (Glossina palpalis palpalis) populations in the town of Daloa, Côte d'Ivoire. High tsetse densities were found in the town outskirts, where the calculated risk of transmission was greatest. Environmental changes brought about by urbanization did not result in the disappearance of tsetse, or the interruption of sleeping sickness transmission. The few cases of sleeping sickness d…

[Contribution of a mathematical model in the control of a parasitosis: the case of human African trypanosomiasis due to Trypanosoma brucei gambiense].

Trypanosoma brucei gambiense sleeping sickness transmitted by tsetse flies (Glossina spp.) is lethal if not treated adequately. The endemicity was generally well under control in the sixties. However, since the seventies the disease is returning in most of its old foci, with alarming endemic levels in several areas. Mathematical modelling provides a rational basis for finding the optimal strategies to control these recrudescences. We present a deterministic model of the basic transmission of trypanosomiasis between human and vector hosts in natural situations. The parameters were quantified on…

Evaluation of Procyclic Agglutination Trypanosomiasis Test (PATT) for the immunodiagnosis of Trypanosoma brucei rhodesiense sleeping sickness in Kenya.

Documented sera from 156 patients admitted to Alupe Sleeping Sickness Hospital in Western Kenya were tested to determine the potential usefulness of Procclic Agglutination Trypanosomiasis Test (PATT) for the diagnosis of Trypanosoma brucei rhodesiense African sleeping sickness. A total of 490 serum samples were tested, including 42 controls. Anti-trypanosome antibodies were detected in 99% of the sera taken prior to trypanocidal drug therapy. Antibody levels remained high during course of treatment. In cured cases antibodies declined to negative or low levels 4 months to one year after treatme…

Effects of African trypanosomiasis on brain levels of dopamine, serotonin, 5-hydroxyindoleacetic acid, and homovanillic acid in the rabbit.

Serotonin (5-HT) levels fell by 21% in the mid-brain-thalamus-hypothalamus (MTH) region of the rabbit brain after chronic infection with the protozoan Trypanosoma brucei gambiense. 5-HT did not decrease significantly in the caudate/putamen (CP) or the pons/medulla (PM) region. 5-Hydroxyindoleacetic acid (5-HIAA) levels were unchanged in the MTH and caudate/putamen (CP) but increased by 17% in the pons/medulla (PM) after infection. Dopamine (DA) levels rose by 19% and homovanillic acid (HVA) by 33% in the PM during infection. DA and HVA tended to be lower in the CP of infected rabbits, but the

Human trypanosomiasis in south-east Uganda. A further study of the epidemiology of the disease among fishermen and peasant cultivators.

During the past two decades there has been an increase in the incidence and spread of sleeping-sickness due to Trypanosoma rhodesiense throughout the north-eastern shore area of Lake Victoria; this increase has been associated with heightened finishing activity and increasing and irregular settlement of the tsetse-fly belt of south-east Uganda. The author describes a number of epidemiological factors affecting the occurrence of the disease among fishermen, placing emphasis on the correct development of the local fishing industry to avoid, on the one hand, depleting the fish population and, on t
