[Latent tuberculosis infection: recommendations for preventive therapy in adults in Germany].

The immunologic mechanisms of latent tuberculosis (TB) infection are complex and hitherto not completely understood. The lifelong risk of an immunocompetent individual of developing active TB after infection with M. tuberculosis is 5-10 % and highest during the first two years after infection. Various factors may considerably increase the risk of developing active TB, e. g., immunosuppressive disease or immunosuppressive medication. However, the development of active TB may be avoided by preventive chemotherapy, the therapy of choice being isoniazid over a 9-month period. Alternative treatment

Nifedipine-induced hyporeactivity in delayed hypersensitivity skin tests.

Four patients and three healthy volunteers were submitted to delayed hypersensitivity skin tests to candidin, tuberculin, streptokinase/streptodornase (SK/SD) and mumps antigen before and after a 1 week treatment with nifedipine 10 mg q. 8h. A decrease in the scores of induration and erythema was observed, especially in response to SK/SD antigen challenge. We conclude that nifedipine may induce an anergy status in delayed hypersensitivity skin tests.

[Latent tuberculosis infection after an internship in India].

BACKGROUND: On returning from a period in a country where tuberculosis is endemic, e.g. a medical internship abroad, screening for tuberculosis takes place. The tuberculin skin test (Mantoux test) and the interferon-γ release assay test are available for this purpose. CASE DESCRIPTION: We describe a 23-year-old female medical intern who was treated preventively with a tuberculostatic drug due to a latent tuberculosis infection (LTBI) following an internship in India. This treatment was prematurely discontinued due to increased transaminases (ALT, AST) and physical side effects that impacted n

Cecal Tuberculosis Mimicking Submucosal Tumor.

A 41-year-old man presented with abdominal fullness in late August 2012. Abdominal CT showed ileus caused by stenosis of the ileum and an enlargement of the ileocecal lymph nodes. Colonoscopy showed a steep elevated protruding tumor in the cecum, with multiple ulcerative lesions on top. A pathological analysis of the lesions confirmed chronic inflammatory infiltration and epithelioid granuloma. The findings of a tuberculin skin test and QuantiFERON-TB-Gold test were positive. As a result, we treated the patient for tuberculosis of the cecum. After 4 months of treatment, colonoscopy confirmed t

Herpes zoster in Japanese patients with systemic lupus erythematosus.

We examined retrospectively the incidence of herpes zoster among 58 Japanese patients with systemic lupus erythematosus (SLE), 42 patients with systemic sclerosis (SSc) and 21 patients with autoimmune bullous diseases (AIBD). The incidence of herpes zoster was significantly higher in SLE (46.6%) than in SSc (9.5%) or AIBD (4.7%) (P < 0.001). The rate of positive delayed-type skin test with varicella zoster virus antigens was significantly lower in SLE than in SSc [4/16 (25.0%) vs. 11/12 (91.7%), P < 0.001]. In addition, the positive rate of skin test with purified protein derivatives of tuberc

Recommendations on the Diagnosis and Treatment of Tuberculosis Infection: SEPAR/SEIMC/Spanish Ministry of Health Consensus Statement.

All strategies on the pathway to tuberculosis (TB) elimination prioritise, among other measures, addressing tuberculosis infection (TBI), particularly the identification of individuals and population groups who should be candidates for tuberculosis preventive treatment (TPT). In Spain, a TBI test is required before recommending TPT. An interferon-gamma release assay (IGRA) is preferred over the tuberculin skin test (TST), although the latter may be used in settings where IGRAs are not available or when deemed necessary to increase diagnostic sensitivity. On the other hand, new skin tests emplo

Diagnosis and management of tuberculosis infection in inclusion health populations in London.

BACKGROUND: Tuberculosis in the UK is more prevalent in people with social risk factors- e.g. previous incarceration, homelessness - and in migrants from TB endemic countries. The management of TB infection is part of TB elimination strategies, but is challenging to provide to socially excluded groups and the evidence base for effective interventions is small. METHODS: We evaluated a TB infection screening and treatment programme provided by a peer-led service (Find&Treat) working in inclusion health settings (e.g. homeless hostels) in London. IGRA (interferon-gamma release assay) testing and

Invited Commentary: The Winding Road to Identifying the Annual Rate of Tuberculosis Infection.

Many persons with immunological tests indicating Mycobacterium tuberculosis infection, such as tuberculin skin tests or interferon-γ release assays, are at risk of progression to tuberculosis disease. Persons whose tests revert to negative may no longer be at such risk. Therefore, identifying the rate of test reversion, potentially indicating cure of M. tuberculosis infection, is an important area of investigation. In their accompanying article (Am J Epidemiol. 2023;192(12):1937-1943), Schwalb et al. extract data on test reversion from prechemotherapy literature and construct a model to predic

Tuberculosis and childhood cancer - A review of literature.

Tuberculosis and malignancy are major public health problems in developing countries like India and causes significant morbidity and mortality. Mycobacterium tuberculosis is an aerobic acid-fast bacilli which is an important pathogen especially complicating clinical status of paediatric oncology patients and treatment of infection with this bacilli is challenging in this subpopulation of patients because of ongoing immunosuppression and relative lack of published guidelines. Atypical presentations of tuberculosis in children also complicate the diagnosis and management. All the more, in tuberc

THE ROLE OF INTERFERON-GAMMA RELEASE ASSAYS IN DIAGNOSIS OF LATENT TUBERCULOSIS INFECTION IN CHILDREN.

Despite numerous published papers, diagnosis of latent tuberculosis infection (LTBI) in children is still an undefined area. The importance of this topic lies in the fact that one third of the world's population is infected with Mycobacterium (M.) tuberculosis. The majority of infected individuals are LTBI cases which make a reservoir for future active tuberculosis (TB) patients. The gold standard for LTBI detection is still undetermined and this is due to the effect of various confounding factors on existing diagnostic tests. Until a decade or so ago, throughout the last century, tuberculin s

Comparison of IGRA and TST in the diagnosis of latent tuberculosis among women of reproductive age in South India.

BACKGROUND: Latent tuberculosis infection (LTBI) is a mycobacterial infection defined on the basis of cellular immune response to mycobacterial antigens. The tuberculin skin test (TST) and the Interferon-Gamma Release Assay (IGRA) are the two tests currently used to establish the diagnosis of LTB. Literature suggests that a study regarding tuberculosis (TB) infection among women of reproductive age group is limited. METHODS: Female household contact, married, aged 18-49 years underwent written consent form and are screened for LTBI using the TST and IGRA. Participants are injected with TST [5

The role of IGRA in the diagnosis of tuberculosis infection, differentiating from active tuberculosis, and decision making for initiating treatment or preventive therapy of tuberculosis infection.

OBJECTIVES: The World Health Organization estimated that a quarter of the global population is infected by Mycobacterium tuberculosis (Mtb). A better control of tuberculosis (TB) is based on the ability to detect Mtb infection, identifying the progressors to TB disease, undergoing to preventive therapy and implementing strategies to register the infections and treatment completion. DESIGN: we reviewed the literature regarding the tests available for TB infection diagnosis, the preventive therapies options and the cascade of care for controlling TB at a public health level. RESULTS: current t

Evaluation of a lateral-flow nanoparticle fluorescence assay for TB infection diagnosis.

BACKGROUND: Programmatic management of TB infection is a critical component of the WHO End TB Strategy. Interferon-gamma release assays (IGRAs) overcome some limitations of the tuberculin skin test, but implementation of IGRA testing in low-resource settings is challenging.METHODS: In this feasibility study, we evaluated performance of a novel digital lateral-flow assay, the QIAreach® QuantiFERON® TB (QIAreach-QFT) test, against the QuantiFERON®-TB Gold Plus (QFT-Plus) assay. A population with a mix of risk factors for TB infection (111 donors) were sampled over multiple days. A total of 207 i
