A Decade of the National Center for HIV, Viral Hepatitis, STD, and TB Prevention's Epidemiologic and Economic Modeling Agreement.

In 2014, the Centers for Disease Control and Prevention (CDC) expanded capacity to conduct infectious disease and economic modeling through the National Center for HIV, Viral Hepatitis, STD, and TB Prevention Epidemiologic and Economic Modeling Agreement (NEEMA). NEEMA brought together CDC scientists, academic partners, and public health practitioners at state and local levels to use epidemiologic and economic modeling to support the understanding of the efficiency, outcomes, cost-effectiveness, return on investment, and impact of programs and policies related to the center's disease foci, pri

[Inequity in infant vaccination coverage in Colombia 2000 and 2003].

OBJECTIVE: Identifying inequity in childhood vaccination coverage in towns throughout Colombia for both immunisations scheme and type of vaccine. METHODS: An ecological study using secondary information for 2000 and 2003. Three indicators were measured: (1) gaps in coverage (understood as being the differences between municipal coverage and that at national level), (2) Gini coefficient and (3) Lorenz's curve. RESULTS: Decreasing national vaccination coverage was found, dropping from 78.8% in 2000 to 66.8% in 2003. The number of towns having gaps in their coverage also increased during the tw

Prevalence of liver function test derangements in adult tuberculosis patients initiated on a daily fixed combination regimen: a prospective study from Kerala, India.

Anti-tuberculosis drug-induced hepatotoxicity is a major challenge in tuberculosis (TB) management, particularly in high-burden countries like India. Early liver function test (LFT) derangements during daily fixed-dose combination (FDC) therapy may reflect subclinical hepatocellular stress and guide timely intervention; however, evidence from Indian programmatic settings remains limited. To determine the prevalence, severity, demographic associations, biochemical trends, and clinical outcomes of early (two-week) LFT abnormalities in adult TB patients initiating daily FDC therapy under the Nati

Rifampicin-warfarin interaction leading to macroscopic hematuria: a case report and review of the literature.

BACKGROUND: Rifampicin remains one of the first-line drugs used in tuberculosis therapy. This drug's potential to induce the hepatic cytochrome P450 oxidative enzyme system increases the risk of drug-drug interactions. Thus, although the presence of comorbidities typically necessitates the use of multiple drugs, the co-administration of rifampicin and warfarin may lead to adverse drug events. We report a bleeding episode after termination of the co-administration of rifampicin and warfarin and detail the challenges related to international normalized ratio (INR) monitoring. CASE PRESENTATION:

Tumor Necrosis Factor-Alpha (TNF-α) Inhibitor Treatment in Hidradenitis Suppurativa: A Population-Based Retrospective Cohort Study of Comorbid Risks.

Monoclonal antibodies targeting tumor necrosis factor-alpha (TNF-α) have transformed the management of moderate-to-severe hidradenitis suppurativa (HS). However, their broader safety profile remains underexplored. Using the TriNetX database, 15,416 patients with HS treated with TNF-α inhibitors (infliximab, adalimumab, certolizumab pegol, golimumab) and 201,737 untreated controls were identified. Untreated controls were defined as patients with HS who had no documented exposure to TNF-α inhibitors. This group may have included patients receiving alternative systemic therapies such as antibioti

A scoping review on the risk of tuberculosis in specific population groups: can we expand the World Health Organization recommendations?

Since 2015, the World Health Organization (WHO) has recommended prioritising testing and treatment of tuberculosis (TB) infection (TBI) in 11 high-risk groups. With new options emerging for TB preventive treatment, we conducted a scoping review, in consultation with the WHO's Global Tuberculosis Programme, to explore the evidence for other population groups at potentially high risk of progression to active TB. We searched six databases for preprints and articles published between 2000 and August 2022. 18 out of 33 668 screened records were included (six meta-analyses and 12 original research s

Variation of clinical manifestations according to culprit drugs in DRESS syndrome.

PURPOSE: Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a rare but serious condition that systematically damages various internal organs through T-cell-mediated immunological drug reactions. We aimed to investigate whether clinical manifestations of DRESS syndrome differ according to culprit drugs. METHODS: We retrospectively analyzed data from 123 patients with probable/definite DRESS syndrome based on the RegiSCAR criteria (January 2011 to July 2016). The data were obtained from the Korean Severe Cutaneous Adverse Reaction Registry. Causality was assessed using th

[Infections in patients affected by rheumatologic diseases associated to glucocorticoid use or tumor necrosis factor-alpha inhibitors].

A great diversity of infectious agents can affect patients that use steroids at immunosuppressive doses or tumor necrosis factor alpha (TNF-alpha) antagonists. The list of participating microorganisms is more restricted in the case of anti TNF-alpha blockers. Overlapping agents include intracellular bacteria, Mycobacterium tuberculosis, geographic fungal agents that have the ability to establish granulamotous infections, herpes zoster, and reactivation of chronic hepatitis B virus infection. An important conceptual issue for these infections is the existence of a threshold prednisone daily dos

Infectious diseases of Afghan immigrants in the United States: review of published reports.

Infectious diseases of immigrants may differ from patients born and resident in the same country, especially if immigrants from Africa or Asia live in Europe or North America. Because the available information is limited published reports of infections of Afghan immigrants in the United States and other countries were analysed. Four reports from the US and 15 reports from other countries were identified [7, (46.7%) Pakistan, 5 (33.3%) Iran, 1 (6.7%) United Kingdom, 1 (6.7%) Germany, 1 (6.7%) Israel)]. Reports from the US were case reports or case series of infections with gastro-intestinal par

Immune reconstitution disease: recent developments and implications for antiretroviral treatment in resource-limited settings.

PURPOSE OF REVIEW: We review literature published over the past 2 years regarding the immunopathogenesis, epidemiology, clinical spectrum and outcomes of immune reconstitution disease in patients receiving antiretroviral treatment. Particular attention is drawn to data relevant to resource-limited settings. RECENT FINDINGS: In high-income countries, immune reconstitution disease occurs in association with a largely predictable spectrum of pathogens, including the herpesviruses, Mycobacterium tuberculosis and Mycobacterium avium complex, Cryptococcus neoformans and hepatitis viruses. [Inflammatory reconstitution immune syndrome associated to antiretroviral therapy and meningeal cryptococcosis].

The objective of high activity antiretroviral therapy (HAART) in patients with AIDS, is to obtain immune restoration. This means a reduction of the viral load and restitution of the CD4 cell count. A decreased rate of HIV replication improves both the number and function of CD4 cells. Nevertheless, this treatment sometimes results in the reappearance of previous symptoms from treated conditions due to opportunistic infections (ie: tuberculosis, criptococcosis, hepatitis, Pneumocystis jirovesi, toxoplasmosis, etc) or non infectious condition such as sarcoidosis, Graves disease or Kaposi sarcoma

Treatment completion and costs of a randomized trial of rifampin for 4 months versus isoniazid for 9 months.

There is little published information regarding treatment completion, safety, and efficacy of rifampin administered daily for 4 months-a recommended alternative to 9 months of isoniazid for therapy of latent tuberculosis infection. In an open-label randomized trial at a university-affiliated respiratory hospital, consenting patients whose treating physician had recommended therapy for latent tuberculosis infection were randomized to daily self-administered rifampin for 4 months or daily self-administered isoniazid for 9 months. Of 58 patients randomized to rifampin, 53 (91%) took 80% of doses,

Hepatotoxicity and tuberculosis treatment outcomes in chronic liver disease.

BACKGROUND: The treatment of tuberculosis (TB) is known to cause liver injury, however, there is limited data to guide optimal treatment for patients with chronic liver disease. METHODS: We undertook a retrospective case series of patients with chronic liver disease and TB disease. The primary objective was to determine if there was a difference in the incidence of drug-induced liver injury (DILI) in patients with cirrhosis versus those with chronic hepatitis. Additionally, we sought to compare TB treatment outcomes, type and duration of therapy, and incidence of adverse events. RESULTS: We

Implementing guidelines on the prevention of opportunistic infections in inflammatory bowel disease.

INTRODUCTION: Opportunistic infections are a key safety concern in the management of patients with inflammatory bowel disease (IBD). Despite the existence of international guidelines, many gastroenterologists have not adopted routine screening and vaccination. The aim of this study was to modify clinical behaviour by use of a simple screening tool. METHODS: A screening and vaccination proforma for hepatitis B, varicella, Influenza, Pneumococcus, human papillomavirus, tuberculosis, hepatitis C and HIV was provided to each participating gastroenterologist. Gastroenterologists were surveyed for

Health status assessment of drug addicts in Alexandria.

The case-control study was conducted among addicts in Alexandria to determine the different health problems among them, as well as to study the correlation between addiction and these problems. One hundred drug addicts and eighty apparently healthy controls were included in this study. The whole sample was subjected to the following: filling a predesigned questionnaire sheet, clinical and electrocardiogram (ECG) examination, measurements of Body Mass Index (BMI), complete blood picture and antistreptolysin O titre (ASO), testing for hepatitis B surface antigen (HBsAg) and antibody to hepatitis

Health Care for Immigrant Children: Discussing Birth Country During Pediatric Appointments.

Information about a child's country of birth (also called nativity) is highly relevant to pediatric care, as it informs the recommendations pediatricians will make regarding screening for serious conditions such as elevated blood lead levels, viral hepatitis, and tuberculosis. However, pediatricians may be uncertain how to respectfully inquire about a child's birth country. These uncertainties have been amplified in the current US political climate in which many immigrant families have heightened fear of detention and deportation. We suggest pediatricians ask about nativity when it shapes heal
